Description of Change: 3rd St. Storm Sewer Repair / Replacement (24" Storm Sewer
Option)
Reason for Change: It was discovered that the storm sewer located along the south
side of 3 rd St. west of the Cloud Branch drainage system was not
connected to the new box culvert that was constructed as part of
the Former Sanford Gasification Plant Remedial Construction. It
is required that new storm sewer piping extending from west of
the box culvert, through the ISS area, and connecting to the box
culvert be installed in order to restore the 3 rd St. storm sewer
system outfall to the Cloud Branch drainage system.

WAIVER This Change Order constitutes full and mutual accord and satisfaction for the
adjustment of the Contract Price and Contract Time as a result of increases or decreases in
cost and time of performance caused directly and indirectly from the change. Acceptance of
this Waiver constitutes an agreement between OWNER and CONTRACTOR that the Change
Order represents an equitable adjustment to the Agreement and that CONTRACTOR shall
waive all rights to file a Contract Claim or claim of any nature on this Change Order. Execution
of this Change Order shall constitute CONTRACTOR's complete acceptance and satisfaction
that it is entitled to no more costs or time (direct, indirect, impact, etc.) pursuant to this Change
Order.

NOTES
1. Dewatering is not included — in the event it is found to be needed, it will be conducted at an additional cost as
follows:
18" or 24" RCP outside the ISS area $4,128.00
18" or 24" RCP inside the ISS area $480.00
2. All dump fees & environmental paperwork or fees generated by the ISS removal operation will be paid for and
provided by others.
3. The rip rap is available from Cemex In their Cocoa, FL plant. They are the Florida distributors for Aggregates USA &
they ship the product down here from GA by rail. Therefore, our price includes picking up this material from Cocoa,
FL in lieu of Macon, GA
4. As requested, this price does not include erosion control at individual inlets & silt fence at the project site proper.
5. This price includes an allowance of $400 for geotechnical testing ( com(action, proctor, etc.)
6. This price includes removal & replacement of the private sign. This sign is fairly old, therefore, BBC takes no
responsibility of its integrity in case it falls apart upon removal. We will take all due care in the process of its removal
& replacement.
7. A trench box will be utilized for installation of all pipe & structures.
8. The trenched area through the ISS material is priced at 17' long, 9' deep & 6' wide. This is because earth fill will have
to be utilized at the trench bottom & sides to provide a compacted base for the pipe, especially at the pipe bell area.
The original request for pricing calls for a 17' long x 8' deep x 3' wide trench @ the ISS. Those dimensions do not
allow for enough room for compaction or compaction equipment. 37 CY (truck measure) of earth fill is included here
for backfill for the area the ISS occupied.
9. Both palm trees (approx. 30'— 40' tall) are to be removed by others and are not included in this cost.
10. As requested, trucking of removed ISS material to the disposal site will be by Star Trucking and that cost is included
above.
11. We anticipate this scope of work to take about 30 calendar days to complete upon authorization to proceed.
Therefore, we will need a 30 day extension of the original Upsala contract time. The authorization to proceed will be
needed by 211113 in order to complete the work within 30 days of the current subst. completion date of the Upsala
project (2127113). Starting the 3' St. work by 211113 gives until 3127113 to get the 3` St. work completed.
12. Rip Rap at existing MES North of 3' St. is not included.
