ORDINANCE NO. '?riL11-
AN ORDINANCE OF THE CITY OF SAl\TfORD, FLORIDA,
AMENDING ORDINANCE NO. 3630, TO AMEND HUMAN
RESOURCES MANUAL PERSONNEL RULES AND
REGULATIONS, CHAPER II, SECTION 2.45, HEALTH
INSURANCE; PROVIDING FOR SEVERABILITY, CONFLICTS,
AND EFFECTIVE DATE.
WHEREAS, it is the desire of the City Commission of the City of Sanford, Florida, to amend
Human Resources Manual Personnel Rules and Regulations, Health Insurance.
NOW, THEREFORE, BE IT ENACTED BY THE PEOPLE OF THE CITY OF SANFORD,
FLORIDA:
SECTION 1: The City Commission adopts statements above as if fully set forth herein.
SECTION 2: That the Human Resources Manual Personnel Rules and Regulations, Health
Insurance, which is attached hereto as Exhibit "A" is incorporated herein by reference.
SECTION 3: SEVERABILITY: If any section or portion of a section of this ordinance
proves to be invalid, unlawful or unconstitutional, it shall not be held to impair the validity, force or
effect of any other section or part of this ordinance.
SECTION 4: CONFLICTS: That all ordinance or part of ordinances in conflict herewith be
and the same are hereby revoked.
SECTION 5: EFFECTIVE DATE: That this ordinance shall become effective October 1

2.45 HEALTH INSURANCE
(A) On the first day of the month after completing thirty (30) calendar days of service
with the City of Sanford, regular full-time and regular part-time employees are
provided with City paid health insurance. Once the application is submitted to
Human Resources, the City pays the employee portion as determined by the City
Commission of health insurance that is equal to the premium of the least expensive
plan option between the City's HMO Plan Option and the POS Plan Option. The
selection by the employee of a plan option requiring a higher premium payment that
is different from the City's portion shall be borne by the employee. Employees
desiring insurance coverage for their dependents, under the City's plan, may obtain
such dependent coverage at their own expense.
(B) Each FY the City Commission will determine the rates the City will contribute towards
health insurance coverage. If the cost of employee coverage exceeds the amount
determined by the City Commission, then the difference shall be borne by the
employee and/or retiree receiving health care coverage
-fA)(C) An employee if hired before October 1, 2002 will have met the criteria for eligibility
for continuation of city-paid health insurance at the time of retirement upon
completion of twenty-five (25) years' service to the City of Sanford. Employees hired
after October 1, 2002 who retire with twenty-five (25) years of service to the City
shall not be eligible for any city paid health insurance in accordance with the
foregoing retirement eligibility criteria in this paragraph.
(0) This will entitle the retiring employee to the same health insurance coverage as is
afforded regular employees at the samo city and employee contribution rates that
m::ty bo in effect and the total premium charged to the retiring employee will not
exceed the total premium charged to the active employee.
(E) Retirees receiving city health insurance coverage upon reaching age 65 will be
required to enroll in Medicare Part B coverage.
(F) Employees retiring for disability reasons and who meet the criteria for disability
retirement as prescribed by the Florida Retirement System (F. R.S.) or who meet the
criteria for disability retirement as prescribed by the Police or Fire Pension Plan are
eligible for continued City-paid health insurance.
This will ontitleThe retiring employee retiring for approved for disability reasons will
be entitled to the same health insurance coverage as is afforded regular employees
at the somo employeo contribution rate that may bo in effoct and the total premium
will not exceed the total premium charged to the active group. Employees retiring for
disability reasons shall have submitted to the City, proof of application for disability
retirement coincident with the employee's date of termination.

fGtfG) Upon attainment of age sixty-five (65) all entitlement to City-paid health insurance will
cease,
Whenever a retiree becomes ineligible for continuation of City-paid health insurance
due to attainment of age sixty-five (65), he/she or his/her eligible dependents may
elect to continue this coverage at his/her own expense under the provisions of
existing "COBRA" Law and Florida Statute 112.0801.
tf))(H) With respect to the foregoing paragraph this policy became effective October 1,
1991 and would be applicable to all retirements occurring on or after that date.
Current retirees will remain eligiblo f-or City paid hoalth insuranco as is affordod
regular omployees ot tho same omployee contribution rate that is in eff-ect.
