NOTICE: In addition to the requirements of this permit, there may be additional restrictions applicable to this property that may be
found in the public records ofthis county, and there may be additional permits required from other governmental entities such as water
management districts, state agencies, or federal agencies.
Acceptance ofpermit is verification that 1 will notify the owner ofthe property of the requirements of Florida Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee at the time of permit submittal. A copy of the executed contract is required
in order to calculate a plan review charge and will be considered the estimated construction value of the job at the time of submittal.
The actual construction value will be figured based on the current 1CC Valuation Table in effect at the time the permit is issued, in
accordance with local ordinance. Should calculated charges figured off the executed contract exceed the actual construction value,
credit will be applied to your permit fees when the permit is issued.

All permit application packages must be complete prior to acceptance. You must check each
box to the left or indicate n/a on this submittal. A complete application package shall include
the following:
PERMIT APPLICATION AND SUBMITTAL REOUIREMENTS
Building Permit Application completed, signed and notarized.
Floodplain development application completed and signed if any portion of the property is in a flood
hazard area as identified on the most current flood insurance rate map.
Copy of a contract, signed by the contractor and the property owner, indicating the documented
construction value
Application must include correct address and complete parcel I.D. number.
Contractor information is required to be included on the permit application (ifcontractor is applicant).
Applicant must include the name of the designated plan review contact person, their phone number and
either a fax number or email address on the Building Permit Application form.
Copy of the contractor's license issued by the State of Florida (if contractor is applicant).
A site specific notarized power of attorney shall be required from the licensed contractor if
he/she appoints an employee of his/her company to sign the permit application as the contractor.
Certificate of insurance indicating worker's compensation insurance coverage and naming the City of
Sanford as certificate holder, or a copy of a worker's compensation exemption issued by the State of
Florida (must be submitted with each application if contractor is the applicant).
Completed and signed Owner Builder Statement / Affidavit (if owner is applicant).
Two (2) copies of all applicable plans and related documentation.
An accurate, signed and sealed, property survey which shows all improvements on the subject property
and within 10 feet on adjacent parcels.
Plot plan showing location ofproposed improvement(s) and setbacks to property line(s). May need to
include infill lot requirements.

display cand be completed and certified by the builder to be accurate and correct before final approval of the building for occupancy. Florida law

to electric service systems to be turned off. A separate valve shall be provided to permit the energy supplied to the main burners)
of combustion types of service water heating systems to be turned off.
0 R403.4.4.2 Water heating equipment. Water heating equipment installed in residential units shall meet the minimum efficiencies of
Table C404 2 in Chapter 4 of the Florida Building Code. Energy Conservation, Commercial Provisions, for the type of equipment installed.
Equipment used to provide heating functions as part of a combination system shall satisfy all stated requirements for the appropriate water
heating category. Solar water heaters shall met the criteria Section R403.4.4.2.1.
R403.4.4.2.1 Solar water heating systems. Solar systems for domestic hot water production are rated by the annual solar energy
factor of the system. The solar energy factor of a system shall be determined from the Florida Solar Energy Center Directory of
Certified Solar Systems. Solar collectors shall be tested in accordance with ISO Standard 9806, Test Methods for Solar Collectors.
and SRCC Standard TM-1, Solar Domestic Hot Water System and Component Test Protocol. Collectors in installed solar water
heating systems should meet the following criteria:
1. Be installed with atilt angle between 10 degrees and 40 degrees of the horizontal; and
2. Be installed at an orientation within 45 degrees of true south.

Florida Building Code. Residential or Florida Building Code, Mechanical, as applicable, or with other approved means of ventilation.
Outdoor air intakes and exhausts shall have automate or gravity dampers that dose when the ventilation system is not operating.

0 R403.6 Equipment sizing. Heating and cooling equipment shall be sized in accordance With ACCA Manual S based on the
equipment loads calculated in accordance with ACCA Manual j or other approved heating and cooling calculation methodologies,
based on building loads for the directional orientation of the building. The manufacturer and model number of the outdoor and
indoor units (if split system) shall be submitted along with the sensible and total cooling capaeitles at the design conditions
describgd in Section R302.1. This code does not allow designer safety factors. provisions for future expansion or other factors
which affect equipment sizing. System sizing calculations shall not include loads created by local intermittent mechanical ventilation
such as,standard kitchen and bathroom exhaust systems.
R403.6.1.1 Cooling equipment capacity. Cooling only equipment shall be selected so that its total capacity is not less than the
calculated total load, but not more than 1.1S times greater than the total load calculated according to the procedure selected in
Section 403.6. or the closest available size provided by the manufacturers product lines. The corresponding latent capacity of the
equipment shall not be less than the calculated latent load.

equipment may be sized in accordance with good design practice.

R403.6.1.2.1 Heat pumps. Heat pumps sizing shall be based on the cooling requirements as calculated according to Section
R403.6.1.1 and the heat pump totai cooling capacity shall not be more than 1.15 times greater than the design cooling load.
R403.61.2.2 Electric resistance furnaces. Eiectric resistance furnaces shall be sized within 4 kW of the design
requirements calculated according to the procedure selected in Section R403.6 1.
R403.6.1.2.3 Fossil fuel heating equipment. The capacity of fossil fuel heating equipment with natural daft atmospheric bumers
shall not be ess than the design toad calculated in accordanco with Section R403.6.1.
0 R403.6.1.3 Extra capacity required for special occasions. Residences requiring excess cooling or heating equipment capacity on an
intermittent basis. such as 'anticipated additional loads caused by major entertainment events, shall have equipment sized or controlled to
prevent continuous space cooling or heating within that space by one or more of the following options:
1.' A separate cooling or heating system is utilized to provide cooling or heating to the major entertainment areas.
2. A variable capacity system sized for optimum performance during base load periods is utilized.
0 R403.7 Systems serving multiple dwelling units (Mandatory). Systems serving multiple dwelling units shall comply with Sections
C403 and C404 of the Commercial Provisions in lieu of Section R403.
lJ R403.8 Snow meli system controls (Mandatory). Snow and ice -melting systems, supplied through energy service to the building, shall
include automatic controls capable of shutting off the system when the pavement temperature is above 55'F, and no precipitation is falling

D R403.9 Swimming pools, Inground spas and portable spas )Mandatory). The energy requirements for residential pools and
inground spas shall be as specified in Sections R403.9.1 through R403 9.3 and in accordance with ANSI/APSP-15 The energy
requirements for portable spas shall be in accordance with ANSI/APSP-14.
0 R403.9.4. Pool and spa heaters. All pool heaters shall be equipped with a readily accessible on -off switch that is mounted
outside the heater to allow shutting off the heater without adjusting the thermostat setting.
R403.9.1.1 Gas and oil -fired pool and spa heaters. All gas- and oil -fired pool and space heaters shall have a minimum
thermal efficiency of 82 percent for heaters manufactured on or after April 16, 2013 when tested in accordance with ANSI Z 21.56.
Pool heaters fired by natural gas or LP gas shall not have continuously burning pilot lights.
R403.9.1.2 Heat pump pool heaters. Heat pump pool heaters shall 'nave a minimum COP of 4.0 when tested in accordance
with AHRI 1160, Table 2, Standard Rating Conditions -Low Air Temperature. A test report from an independent laboratory is
required to verity procedure compliance. Geothermal swimming pool heat pumps are not required to most this standard.
0 R403.9.2 Time switches. Time switches or other control method that can automatically turn off and on heaters and pumps according to a
preset schedule shall be installed on all heaters and pumps. Heaters. pumps and motors that have buiit in timers shall be deemed in
compliance with this equipment.
Exceptions:
1. ; Where public health standards require 24-hour pump operations.
2. Where pumps are required to operate solar- and wsste-heat-racovery pool heating systems.
3. Where pumps are powered exclusively from on-sito renewable generation.
0 R403.9.3 Covers. Heated swimming pools and inground permanently installed spas shall be equipped with a vapor -retardant cover on or
at the water surface or a liquid cover or other means proven to reduce heat Ices.
Exception: Outdoor pools deriving over 70 percent of the energy for heating from site-raccvered energy, such as a heat pump or
solar energy source computed over an operating season.
RR404.1 Lighting equipment (Mandatory). A minimum of 75 percent of the lamps in permanently installed lighting fixtures shall be
high -efficacy Lamps or a minimum of 75 percent of permanently installed lighting fixtures shall contain only high efficacy lamps.
Exception: Low -voltage lighting shall not be required to uhlae, high -efficacy lamps.

SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction
materials. The documentation submitted has been reviewed and accepted by Miami -Dade County RER -
Product Control Section to be used in Miami Dade County and other areas where allowed by the Authority
Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami -Dade County Product Control
Section (In Miami -Dade County) and/or the AHJ (in areas other than Miami -Dade County) reserve the right
to have this product or material tested for quality assurance purposes. If this product or material fails to
perform in the accepted manner, the manufacturer will incur the expense of such testing and the AHJ may
immediately revoke, modify, or suspend the use of such product or material within their jurisdiction. RER
reserves the right to revoke this acceptance, if it is determined by Miami —Dade County Product Control
Section that this product or material fails to meet the requirements ofthe applicable building code.
This product is approved as described herein, and has been designed to comply with the Florida Building
Code, including the High Velocity Hurricane Zone. -

APPROVAL DOCUMENT: Drawing No. MD-SH2O0-01, titled "Single Hung Window Installation'
sheets 1 through 8 of 8, dated 05/11/11, with revision C dated 04/22/16, prepared by manufacturer, signed and
sealed by Anthony Lynn Miller, P.E., bearing the Miami -Dade County Product Control Revision stamp with
the Notice of Acceptance number and expiration date by the Miami -Dade County Product Control Section.

Each unit shall bear a permanent label with the manufacturer's name or logo, city, state, model/
series, and following statement: "Miami -Dade County Product Control Approved", unless otherwise noted
herein. RENEWAL
of this NOA shall be considered after a renewal application has been filed and there has been no
change in the applicable building code negatively affecting the performance of this product. TERMINATION
of this NOA will occur after the expiration date or if there has been a revision or change in
the materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of
any product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to
comply with any section of this NOA shall be cause for termination and removal of NOA. ADVERTISEMENT:
The NOA number preceded by the words Miami -Dade County, Florida, and followed
by the expiration date may be displayed in advertising literature. If any portion of the NOA is displayed,
then it shall be done in its entirety. INSPECTION:
A copy of this entire NOA shall be provided to the user by the manufacturer or its distributors
and shall be available for inspection at the job site at the request of the Building Official. This
NOA revises and renews NOAN 15-0528.22 and consists of thispage 1 and evidence page E-1, as well

I. This product has been evatuoted and is incompliance with the 5th Edition (2014) Florida SuDding Code
FBC) shucturd requirements including the'Ngh VeWty Hurticone Zone' (HVHZ).
2. Product anchors shag be as listed and spaced as shown on details. Anchor embedment to base material
shag be beyond wag dressing or stucco.
3. When used in the 14VHT this product compges with Section 1626 of the Florida Sulding Code and does not
require on impact resistant covering.
4. When used in areas outside of the NVHT requiring wind borne debris protection this product comples with
Section 1609.1.2 of the FBC and does not require on Impact resistant covering. This product meets missle
level'W and includes Mind Zone 4 as defined In ASTM El996 andSection 1609.1.2.2 of the FBC.
5. For 2rt stud framing construction, anchoring of these units shag be the same as that shown for 2Ktuck

The following is a list of the areas of the submitted plans that contained violations of the codes adopted by
the City of Sanford and enforced by the Building Division. The violations noted must be addressed before
the plans can be approved. Changes to plans shall be submitted on the same size format as the original
submittal. Changes to construction documents that require an Architect or Engineer's seal must be
submitted with the appropriate seal. Provide two copies of affected plan sheets and/or supplemental
information as requested. Provide two copies ofaffected Plan streets and/or supplemental information as
reauested. Permit submittals will not be accepted witliout two copies.
COMMENTS:
1. The frame wall is required to be insulated to minimum R-13. The plans show R-11. Please revise
accordingly. As an alternative, the alteration can be calculated using the Simulation method and Form 405
to figure the R-value of the frame wall.

stud to the bottom plate.

5. Please provide two (2) copies of a gas riser for the gas stove that will be installed. Riser must include
length of piping, pipe size, pipe material, appliance BTU's, gas type, inlet pressure, pressure drop and
specific gravity in order to verify compliance with FBCR G2413.

and venting method.

7. Please provide a note on the plans that the entire home must be equipped with smoke detectors, located
as required for new construction. Smoke detectors are not required to be hard -wired or interconnected; the
use of 10-year lithium battery backup detectors is permitted.
