Scope of Work to be Performed:
1. All areas around staging area and dump trailers will be secured with caution tape to insure proper
safety for residents.
2. Project will be staged as to not disrupt daily traffic flow of residents.
3. Tarp landscape in work area to prevent damage during project
4. Provide full time job foreman to oversee and supervise all work.
S. Remove and replace one layer of roof.
6. Remove roof down to decking and re -nail decking 6" on center per code.
7. Inspect all decking for rotten, damaged or deteriorated wood. All deteriorated wood will be replaced
at an additional charge of $5S.00 per sheet of plywood Installed, and SS.75 per linear foot of deck
boards and fascia board installed.
8. Furnish and install new lead boots and new plastic lead boot covers to protect from squirrels.
9. Furnish and Install new goose neck vents to replace existing. All vents will be 26-gauge factory
painted metal.
10. Re -flash chimney's as needed.
11. Furnish and install 22 new off ridge vents to replace existing old aluminum off ridge vents on building.
12. Furnish and Install 30 lb. felt underlayment over entire roof and nail all dry -in to meet state and
county high wind code.
13. Furnish and install Certainteed shingle starters.
14. Furnish and install Certainteed pre-cut Hip and Ridge cap.

Customer and Agent agree to allow Oviedo Roofing Enterprises Inc. to use the Water and Power on site for the
duration of the roof project. Disclosure
Property owner or agent will provide Oviedo Roofing and anyvendor or supplier with access tojob site to
facilitate trucks and equipment. We are not responsible forgutters, screens, screen enclosures and cracks in Disclosure
ceilings. Customer will secure all pictures, paintings, lights and cover any furniture as needed. This price is based
on our trucks being able to back up to the building. However, we are not responsible for any cracks or damage Owners
to the driveway or sidewalks. Oviedo Roofing Enterprises, Inc is not responsible for any damages to cars, Initials
including damages such as the punctures, dents, broken or cracked glass, or scratches to the paint, when
customers cars are left In driveway or close to our work area. In addition we are not responsible for damage
caused by falling debris. If you do not want us to use the driveway we will have to charge extra and Oviedo
Roofing will not be responsible for draln fields, yard Irrigation, plumbing, landscape or septic systems. Oviedo
Roofing Enterprises, Inc will be responsible for any Intentional negligence on their part.
Oviedo Roofing will not be responsible for any interior repairs ordamage or any environmental Issues Disclosure
discovered during or as part of roof replacement. Owners Initials
Removal of all solar panels and relating piping will be the responsibility of the owner or agent. Disclosure
Price based on one layer of roofing. If additional layers are found each layer will be removed at $30.00 per Disclosure

All paperwork will be posted at the job site (permit, N.O.C., and dry -in affidavit) and must remain posted until Disclosure

This estimate is based on a check/cash price. We offer the use of Visa, MasterCard, and Discover, with the
agreement you will be charged the fee that we must pay the merchant. Financing programs are also available. Disclosure

Contractors work will be warranted by Contractor In accordance with its standard warranty. Contractor shall
not be liable for special, punitive, incidental, consequential damages or subrogation. The acceptance of this
