The City of Sanford requires payment of a plan review fee at the time of permit submittal. A copy of the executed contract is required in order to
calculate a plan review charge and will be considered the estimated construction value of the job at the time of submittal. The actual construction value
will be figured based on the current ICC Valuation Table in effect at the time the permit is issued, in accordance with local ordinance. Should calculated
charges figured off the executed contract exceed the actual construction value, credit will be applied to your permit fees when the permit is issued.

or under the time and material rider and contractor is paid for such work. When deemed necessary to move aerials for proper construction, they shalt be moved at
Owner's risk and expense. It is expressly agreed by Owner that Contractor shall not be liable for any injuries to persons or property caused by leaking of such roof, or
for damages caused by weather, acts of God. fires, gales, structural defects, acts of owner, owners agents or employees. or damage or misalignment of antennae or
satellite and owner hereby waives all such claims. (14) If customer stops crew from wonting on items spoiled out in the contract and reflected in the crew's work order,
then customer agrees to pay for their time charged at the agreed upon man hour rate mentioned herein above. (15) Deposits made on execution hereof are refundable
only at the discretion of the contractor, unless a court of law determines that there is a material breach of the contract by contractor. (16) This agreement must be
ratified by the home office of the contractor, before contractor is bound. (17) 'Chis written agreement is the only and entire contract covering the subject matter hereof
and no representations, oral or written, have been made to Cheer other than those herein.contained. Tune is of the essence of this agreement. Owner and Contractor

WARRANTY INFORMATION
SPECIFICATION
IN CONSIDERATION OF THE PURCHASE AND USE OF ITS PRODUCTS ON THE AFOREMENTIONED INSTALLATION, ADVANCED COMMERCIAL
ROOFING SYSTEMS, INC. ("ACRS") REPRESENTS TO THE CUSTOMER DESCRIBED PREVIOUSLY THAT SUBJECT TO THE TERMS, CONDITIONS AND
LIMITATIONS STATED HEREIN, ACRS WILL REPAIR ANY LEAK(S) M THE ACRS ROOFING SYSTEM UP TO, BUT NOT EXCEEDING, THE CUSTOMER'S
ORIGINAL COST OF THE INSTALLED ROOFING SYSTEM
TERMS AND CONDITIONS
1) THE ROOFING SYSTEM .MUST BE INSTALLED BY AN ADVANTAGE SYSTEM APPROVED ROOFING CONTRACTOR.
2) CUSTOMER SHALL .NOTIFY ACRS 'WITHIN THIRTY (30) DAYS OF THE DISCOVERY OF ANY LEAK(S) IN THE ROOFING
SYSTEM.
3) DURING THE TERM OF THIS SERVICE WARRANTY, ACRS, ITS.AGENTS OR EMPLOYEES SHALL HAVE FREE ACCESS TO THE
ROOF DURING REGULAR BUSINESS HOURS.
4) ACRS SHALL HAVE NO OBLIGATION UNDER THIS WARRANTY UNTIL ALL BILIS FOR INSTALLATION, SERVICE AND ACRS
SUPPLIES HAVE BEEN PAID IN FULL TO ADVANCED COMMERCIAL ROOFING SYSTEMS, INC., 1924 NORTH ELM STREET,
MUNCIE, INDIANA 47303.
5) IF, UPON INSPECTION, ACRS DETERMINES THAT THE LEAK(S) IN THE ROOFING SYSTEM ARE CAUSED BY DEFECTS IN THE
ACRS ROOFING MATERIAL OR BY DEFECTIVE WORKMANSHIP OF THE ADVANTAGE SYSTEMS APPROVED ROOFING. ,
CONTRACTOR, CUSTOMER'S REMEDY AND ACRS LIABILITY SHALL BE LIMITED TO ACRS REPAIR OF THE ACRS ROOFING
SYSTEMS SUBJECT TO THE COST LIMITATION SET FORTH ABOVE.
6) ACRS FAILURE AT ANY TIME. TO ENFORCE ANY OF THE TERMS OR CONDITIONS STATED HEREIN SHALL. NOT BE
CONSTRUED AS A WAIVER OF SUCH TERMS AND CONDITIONS.
7) ACRS WILL CONTRACT WITH AN ADVANTAGE SYSTEMS APPROVED ROOFING CONTRACTOR TO PERFORM SERVICE
WARRANTY WORK ON THE ACRS SPECIFIED SYSTEM.
8) SHOULD THE BUILDING BE SOLD, THE APPLICABLE FEE MUST BE PAID TO ACRS WITHIN 30 DAYS OF CLOSING TO
TRANSFER WARRANTY TO NEW CUSTOMER, IF ELECTED.
9) ANY .[TEM INSTALLED BY ACRS THAT IS NOT A ROOFING PRODUCT SHALL CARRY NO MORE THAN A ONE YEAR LABOR
AND MATERIAL WARRANTY.
10) WARRANTY SHALL TAKE EFFECT AT DATE OF JOB COMPLETION, EXCLUDING ALUMINUM COATING.
EXCLUSIONS
ACRS WILL NOT BE RESPONSIBLE FOR LEAK(S) OR CONSEQUENTIAL DAMAGES CAUSED BY ANY ONE OR COMBINATIONS OF THE
FOLLOWING:
1) NATURAL DISASTERS INCLUDING, BUT NOT LIMITED TO, FLOODS, LIGHTNING, HAIL, GALES, TORNADOES, HURRICANES
AND EARTHQUAKES.
2) ANY ACT OF NEGLIGENCE, ACCIDENT OR MISUSE, INCLUDING, BUT NOT LIMITED TO. VANDALISM, CIVIL DISOBEDIENCE.
ACTS OF WAR, IMPROPER OR INADEQUATE MAINTENANCE.
3) METAL WORK OR OTHER MATERIALS USED WITH THE ROOFING SYSTEM, BUT NOT FURNISHED BYACRS THAT LEAKS)
OR CAUSES LEAK(S).
4) ALTERATIONS OR REPAIRS MADE ON OR THROUGH THE ROOFING SYSTEM OR FLASmNGS AFTER DATE OF COMPLETION
OF THE ROOFING SYSTEM WITHOUT WRITTEN AUTHORIZATION FROM ACRS, UNLESS INSTALLED BY AN ADVANTAGE
SYSTEMS APPROVED ROOFING CONTRACTOR IN A .MANNER ACCEPTED IN WRITING BY .ACRS.
5) IMPROPER APPLICATIONS OR FAILURE OF ANY COMPONENT UNDERLAYING THE ROOF MEMBRANE OR FLASHINGS, SUCH
AS INSULATION, STRUCTURAL DECK, VAPOR BARRIERS, EXPANSION JOINTS OR FASTENERS.
6) ACRS SHALL NOT BE RESPONSIBLE FOR LEAK(S) OR CONSEQUENTIAL DAMAGE CAUSED BY CHANGE IN THE ORIGINAL
PRINCIPAL USAGE TO WHICH THE BUILDING IS PITT, UNLESS SUCH CHANGE IS APPROVED IN WRITING BY ACRS.
7) UNDER NO CIRCUMSTANCES SHALL ACRS BE LIABLE FOR DAMAGE TO INTERIOR OF BUILDING, CONSEQUENTIAL
DAMAGE, LOSS OF PROFITS OR RENTS, CONTENTS OF BUILDING, ROOF INSULATION, ROOF DECK(S) OR OTHER BASE
OVER WHICH ACRS IS APPLIED.
8) DISCHARGE OF VEGETABLE, MINERAL, ANIMAL OILS, GREASES, SOLVENTS, OR CHEMICALS SUCH AS INDUSTRIAL
WASTES, UPON THE ROOF SURFACE
9) ACRS IS NOT RESPONSIBLE FOR MISALIGNMENT OF AERIALS AND WAIVES ALL SUCH CLAIMS.
ACTION
IF LEAK(S) SHOULD OCCUR FROM ANY CAUSE, THE CUSTOMER SHALT. NOTIFY ACRS PROMPTLY, CONFIRMING SUCH NOTICE IN WRITING,
ACRS WILL INSPECT THE ROOF, IF ANY LEAK(S) ARE FOUND TO HAVE BEEN CAUSED BY CONDITIONS COVERED M THE SERVICE WARRANTY
AS STATED ABOVE, ACRS WILL HAVE THE REPAIRS MADE AND WILL PAY FOR SUCH REPAIRS SUBJECT TO THE TERMS. CONDITIONS AND
LIMITATIONS OF THIS SERVICE WARRANTY. IF ACRS DETERMINES THAT THE CAUSE OF LEAK(S) IS NOT COVERED UNDER THE TERMS,

to be brought up to code, confirm with Walter that this work is scheduled
and or completed. There is also a tree that needs cut back before we get
there. Replace the gravel stop with metal drip -edge and replace the
gutters with combination drip -edge gutters. Sweep loose gravel and
install the Flexbond roof over the old gravel roof.Retro-fit the roof drains.
Temporary repairs made to leaks in showroom prior to new roof project
starts. This is part of the contract with the customer.

Quality Assurance documentation changes, or provisions of the Code that relate to the product change. Acceptance of
this Evaluation Report by the named client constitutes agreement to notify Robert Nieminen, P.E. if the product
changes or the referenced Quality Assurance documentation changes. Trinity) ERD requires a complete review of this
Evaluation Report relative to updated Code requirements with each Code Cycle.

4. PRODUCT DESCRIPTION:
The following roof covers are fully adhered to Approved substrates as outlined in the Limitations / Conditions of Use
herein.
4.1 Polybrite 70 or KM Acryl 15 Liquid Applied Roof Cover: A liquid applied acrylic roof system consisting of the
following components.
4.2 PG 800 Asphalt Emulsion Roof Coating: An asphalt -based, clay emulsion used as a base coat.
4.3 Polybrite 76 Base Coat or KM Acryl 30: A water-based, liquid -applied elastomeric base coat.
4.4 Polyglass Polyester Fabric Mat or KM Polyester Fabric: A stitchbond polyester fabric for use as
reinforcement.
4.5 Polybrite 70 Premium Grade Elastomeric Roof Coating or KM Acryl 15: A white or tinted, water-based,
liquid -applied elastomeric coating.

S. LIMITATIONS:
5.1 This is a building code evaluation. Neither Trinity I ERD nor Robert Nieminen, P.E. are, in any way, the Designer
of Record for any project on which this Evaluation Report, or previous versions thereof, is/was used for
permitting or design guidance unless retained specifically for that purpose.
5.2 This Evaluation Report is not for use in FBC HVHZ jurisdictions.
5.3 Refer to a current Approved Roofing Materials Directory for fire ratings of this product.
5.4 For steel deck installations, foam plastic insulation shall be separated from the building interior in accordance
with FBC 2603.4 unless the exceptions stated in FBC 2603.4.1 and 2603.6 apply.
5.5 The roof system evaluation herein pertains to above -deck roof components. Roof decks and structural
members shall be in accordance with FBC requirements to the satisfaction of the Authority Having
Jurisdiction. Load resistance of the roof deck shall be documented through proper codified and/or FBC
Approval documentation.
5.6 For recover installations, the existing roof shall be examined in accordance with FBC 1511.
5.7 For mechanically attached insulation or membrane or strip -bonded insulation, the maximum design pressure
for the selected assembly shall meet or exceed the Zone -1 design pressure determined in accordance with FBC
Chapter 16. Zones 2 and 3 shall employ an attachment density designed by a qualified design professional to
resist the elevated pressure criteria. Commonly used methods are ANSI/SPRI WD1, FM Loss Prevention Data
Sheet 1-29, Roofing Application Standard RAS 117 and Roofing Application Standard RAS 137. Assemblies
marked with an asterisk* carry the limitations set forth in Section 2.2.10.1 of FM Loss Prevention Data Sheet
1-29 (January 2016) for Zone 2/3 enhancements.
5.8 For assemblies with all components fully bonded in place, the maximum design pressure for the selected
assembly shall meet or exceed critical design pressure determined in accordance with FBC Chapter 16. No
rational analysis is permitted for these systems.
5.9 For mechanically attached insulation or membrane over existing roof decks, fasteners shall be tested in the
existing deck for withdrawal resistance. A qualified design professional shall review the data for comparison
to the minimum requirements for the system. Testing and analysis shall be in accordance with ANSI/SPRI FX -
1 or Testing Application Standard TAS 105.
5.10 For bonded insulation or membrane over existing substrates in a re -roof (tear off) or recover installation, the
existing deck or existing roof surface shall be examined for compatibility with the adhesive to be installed. if
any surface conditions exist that bring system performance into question, field uplift testing in accordance
with ANSI/SPRI IA -1, ASTM E907, FM Loss Prevention Data Sheet 1-52 or Testing Application Standard TAS
124 shall be conducted on mock-ups of the proposed new roof assembly.
5.11 For bonded insulation or membrane over existing substrates in a recover installation, the existing roof system
shall be capable of resisting project design pressures on its own merit to the satisfaction of the Authority
Having Jurisdiction, as documented through field uplift testing in accordance with ASTM E907, FM Loss
Prevention Data Sheet 1-52 or Testing Application Standard TAS 124.
5.12 Metal edge attachment (except gutters), shall be designed and installed for wind loads in accordance with FBC
Chapter 16 and tested for resistance in accordance with ANSI/SPRI ES -1 or Roofing Application Standard RAS
111, except the basic wind speed shall be determined from FBC Figure 1609.3(l), 1609.3(2) or 1609.3(3).
5.13 All products in the roof assembly shall have quality assurance in accordance with FAC Rule 61G20-3.

6. INSTALLATION:
6.1 Polyglass Liquid Applied Roof Systems shall be installed in accordance with Polyglass USA published
installation instructions, subject to the Limitations / Conditions of Use noted below.
6.2 System attachment requirements for wind load resistance are set forth in Appendix 1. "MDP" = Maximum
Design Pressure is the result of testing for wind load resistance based on allowable wind loads, and reflects
the ultimate passing pressure divided by 2 (the 2 to 1 margin of safety per FBC 1504.9 has already been
applied). Refer to FBC 1609 for determination of design wind loads.
7. BUILDING' PERMIT REQUIREMENTS:
As required by the Building Official or Authority Having Jurisdiction in order to properly evaluate the installation of
this product.

3. Unless otherwise noted, insulation may be any one layer or combination of polyisocyanu rate, polystyrene, wood fiberboard, perlite, mineral wool, gypsum -based roof board or asphalt -based roof board that meets
the QA requirements of F.A.C. Rule 61G20-3 and is documented as meeting FBC 1505.1 and, for foam plastic, FBC Chapter 26, when Installed with the roof cover.
4. Unless otherwise noted, insulation adhesive application rates are as follows. Ribbon or bead width is at the time of application; the ribbons/beads shall expand as noted in the manufacturer's published instructions:
Dow INSTA-STIK Quik Set Commercial Roofing Adhesive (D -IS): Continuous 0.75 to 1 -Inch wide ribbons, 12 -inch o.c.
Y OMG OlyBond 500 (013500): Continuous 0.75 to 1 -inch wide ribbons, 12 -inch o.c. (PaceCart or SpotShot).
S- Note: When multiple layers(s) of insulation and/or coverboard are installed in ribbon -applied adhesive, board joints shall be staggered.
Note: The maximum edge distance from the adhesive ribbon to the edge of the insulation board shall be not less than one-half the specified ribbons spacing.

8.' For assemblies with all components fully bonded in place, the maximum design pressure for the selected assembly shall meet or exceed critical design pressure determined in accordance with FBC Chapter 16, and no
rational analysis is permitted.
9. For mechanically attached components over existing decks, fasteners shall be tested in the existing deck for withdrawal resistance. A qualified design professional shall review the data for comparison to the
minimum requirements for the system. Testing and analysis shall be in accordance with ANSI/SPRI FX -1 or Testing Application Standard TAS 105.
10. For existing substrates in a bonded recover or re -roof installation, the existing roof surface or existing roof deck shall be examined for compatibility and bond performance with the selected adhesive, and the existing
roof system (for recover) shall be capable of resisting project design pressures on its own merit to the satisfaction of the Authority Having Jurisdiction, as documented through field uplift testing in accordance with
ANSI/SPRI IA -1, ASTM E907, FM Loss Prevention Data Sheet 1-52 orTesting Application Standard TAS 124.
11. "MDP" = Maximum Design Pressure is the result of testing for wind load resistance based on allowable wind loads. Refer to FBC 1609 for determination of design wind loads.
