south side at a location confirmed by General Contractor. As shown on drawings.
The air handler serving Meeting Rooms100-102 will be located above the acoustical
ceiling in Corridor 103.
The (2) air handlers for Multi -Purpose room 106 and for Meeting Room 107 will both
be located in Room 106. one on the south east side of the room and one on the north

Scope of work
Install new Trane 4 Ton XV20i High Efficiency Variable Speed heat pump system_
with (3) zones. Includes (1) 950 Wi-Fi enabled thermostat and (3) individual room
wall sensors. Outdoor unit to mount (in concrete pad and attached at each corner with
equipment provided bracket. Concrete pad provided by General Contractor.
Install new Trane 3 Ton XR17 2 speed heat pump system with (2) zone dampers.
Includes (2) 824 thermostats with (1) bypass damper. Outdoor unit to mount on
concrete pad and attached at each corner with equipment provided brackets. Concrete
pad provided by the General Contractor.
Install new Trane 4 Ton XR14 heat hump system. Includes(]) 824 thermostat..
outdoor unit to mount on concrete pad and attached, at each corner. with equipment
provided bracket. Concrete pad provided'by general contractor.
Will furnish (2) new intake louvers and connect to new equipment. Louvers to be
installed by General Contractor.
Install (1) new exhaust fan above mop sink and pipe work to new louver. Pro Tech
will furnish new louver for general contractor to install.
Furnish & install all low voltage thermostat control wiring.,
Supply and return duct design and installation -
Install (3) 5 inch media filters; one per unit.
Install condensate PVC lines and copper refrigerant lines for each unit. Condensate
lines to drain to grass area at exterior of building.
System startup will be performed by a Pro -Tech certified technician and will be
checked to ensure all manufacturer specs are being met through quality assurance,
analysis.
Cut and patch any necessary,hole_s at the time of installation. No exterior metal panel
patch work included.
Sweep, dust, and vacuum at the end of each day's work.
All installation debris from site will be removed and placed in onsite dumpster i
provided by General Contractor.
NOT INCLUDED 1N SCOPE OF WORK
Cost of electrical work is not included in our proposal.

1. Subcontractor agrees to perform all labor in a workmanlike manner, to prosecute said work in accordance with Contractors schedule and to complete all work
in a prompt and professional manner. Subcontractor is required to clean up scope of work lob debris on a daily basis, failure to maintain this Policy will result In
McKee Construction Co. hiring dean up workers which will result in a backcharge to the subcontractor.
2. Subcontractor further agrees that he has in full force and effect this date and will continue in force during the course of his work, Workmen's Compensation
Insurance, as required by law, and Public Liability and Property Damage Insurance as is reasonably necessary to insure against the risk arising out of this
contract. A certificate of said insurance Is to be received by the Contractor within three (3) days following the execution of this contract. In no case will any work
be commenced prior to receipt of the Certificate by the Contractor. "Prior to the completion of work operations on this property, -none of the above insurance Is to
be cancelled, changed, or allowed to lapse until 30 days prior written notice has beer given to the Contractor."
3. In addition to standard manufacturers warranties for any equipment installed by Subcontractor, Subcontractor guarantees all work against defects from the
use of Inferior or defective materials, equipment or workmanship for one year from the date of occupancy by the owner. All repairs or changes required in
connection with this Guarantee will be performed promptly at the Subcontractors expense.
4. Subcontractor required to acknowledge McKee Construction Co. safety policies manual on site, and adhere to all such policies. McKee Construction Co.
requires hard hats to be worn at all times on jobsite, failure to adhere to this policy will result in the immediate expulsion of violators from jobsite. The
subcontractor agrees to furnish McKee Construction Co. with Material Safety Data Sheets for all hazardous materials that will be used on this project by
subcontractor or his sub -subcontractors.
5. The subcontractor Is to comply with all current OSHA regulations. Safety for the Subcontractors employees is the responsibility of the Subcontractor.
Subcontractor shall maintain a competent person on site during the course of the subcontractors work. In the event that the Subcontractor Is in violation on any
OSHA safety regulations, all loses, damages, and expenses incurred including OSHA fines Incurred as a result of said violation shall be home_ by and charged
against the Subcontractor.
6. Mckee Construction Co.adfiere's to a drug free work environment. All companies and their employees who perform work under this subcontract agreement on
McKee Construction Co. jobsites are subject to random drug testing as a requirement of this contract If refusal or positive testing takes place McKee
Construction Co. reserves the right to ban from performing work under this contract individual or individuals that violate this policy.
7. McKee Construction Co. requires the subcontractor to have on jobsite at all times at least one English speaking employee. Violation of this agreement will
result in immediate termination of this subcontract If termination of contract occurs. subcontractor agrees to cover any cost overage's that may take place in
completing the scope of work.
8. As part of the acceptance of this Subcontract, Subcontractor will provide to McKee Construction Co. a list of all principal's and key employee's home and
cellular phone numbers. Monthly Pay request will not be paid until these numbers have been supplied to McKee Construction Co. If at any time the Subcontractor
fails to respond in a timely manner, McKee Construction Co. has the right to terminate said contract as described in paragraph 7.
9. Subcontractor Is responsible for the maintenance of their scope of work items until Certificate of Occupancy is secured.
10. This Subcontract Agreement can be terminated by McKee Construction Co. if Subcontractor makes any false statement to McKee Construction Co., Including,
but not limited to, any false statement which would subject Subcontractor to criminal charges pursuant to Section 713.35, Flolyda Statues.
11. Jobsite meetings will be scheduled regularly. Subcontractor shall have at -least one employee at a timely scheduled jobsite, meeting for but not limited to the
following reasons: safeey, job schedule, quality of work or failure to perform.

Maintenance
Periodic system maintenance is necessary fir this system to operate correctly and keep
the manufactures warranties in force. We \\ ill update the church's maintenance
agreement so they will know the additional cast.
Terms of Agreement
In the event payment is not made promptly in accordance with agreed teens, it shall be the
Seller's option _to_charge.a service'charge.not excee 'din" I V, per.month._The.first_----
service charge will become due 30 days from the date of the billing of our amount due on
the job. In the event of collection by Attorney, all Attornevs', court costs, and other legal
fees shall be borne by the Buyer: in the event of non-payment, Buyer agrees to allow.
Seller on the premises to remove the equipment installed. This investment proposal shall j
be binding upon the heirs, successors. or assigns of the parties hereto.
It is understood that the title of all the products and equipment covered by this agreement
remains solely in the Seller, until the entire purchase price has been paid in full and the I
manner of installation and/or attachment to any equipment and/or any portion of the
building structure in -which the installation is made shall not in any manner the Seller's
title.

Ci Building Permit Application completed, signed and notarized. Application must include correct address
and complete parcel I.D. number.
Copy of a contract, signed by the contractor and property owner, indicating the documented construction
value
2' Copy of the contractor's license issued by the State of Florida (if contractor is applicant).
A site specific notarized power of attorney shall be required from the licensed contractor if
he/she appoints an employee of his/her company to sign the permit application as the contractor.
Certificate of insurance indicating worker's compensation insurance coverage and naming the City of
Sanford as certificate holder, or a copy of a worker's compensation exemption issued by the State of
Florida (must be submitted with each application if contractor is the applicant).

Please see below and the attached sheet for the panel calc.
This is a high leg system so the load is not evenly balanced across three phases. In this case, we determine which phase
is more heavily loaded (A phase = 23.2 kva), we then convert kva to VA (23,200va), and then divide this number by 120
volts (Phase to neutral voltage)
23.2 kva * 1000 = 23,200VA
23,200/120 volts = 193 amps

This is a high leg system so the load is not evenly balanced across three phases. In this case, we determine which phase
is more heavily loaded (A phase = 23.2 kva), we then convert kva to VA (23,200va), and then divide this number by 120
volts (Phase to neutral voltage)
23.2 kva * 1000 = 23,200VA
23,200/120 volts = 193 amps
