NOTICE: In addition to the requirements of this permit, there may be additional restrictions applicable to this property that may be

Acceptance of permit is verification that I will notify the owner of the property of the requirements of Florida Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee at the time of permit submittal. A copy of the executed contract is required
in order to calculate a plan review charge and will be considered the estimated construction value of the job at the time of submittal.
The actual construction value will be figured based on the current ICC Valuation Table in effect at the time the permit is issued, in
accordance with local ordinance. Should calculated charges figured off the executed contract exceed the actual construction value,
credit will be applied to your permit fees when the permit is issued.

TIME FOR COMPLETION: The Work to be provided by the CONTRACTOR shall be substantially completed as
described In subsection 14.13 of the General Conditions, within ninety (90) calendar days- after the date
when the Contract Time begins to run as provided in subsection 2.2 of the General Conditions. The Work shall
be finally completed, ready for Final Payment in accordance with subsection 14.9 of the General Conditions,
within fifteen (15) calendar days after the actual date of Substantial Completion. Failure to meet the
completion time shall be grounds for Termination of both the Work Order and the Master Agreement for
Default.

WORK ORDER
TERMS AND CONDITIONS
a) Execution of this Work Order by the COUNTY shall serve as authorization for the CONTRACTOR to
provide, for the stated project, services as set out in the Scope of Services attached as Exhibit "A" to
the Master Agreement cited on the face of this Work Order and as further delineated in the attachments
listed on this Work Order.
b) Term: This work order shall take effect on the date of Its execution by the County and expires upon
final delivery, inspection, acceptance and payment unless terminated earlier in accordance with the
Termination provisions herein.
c) The CONTRACTOR shall provide said services pursuant to this Work Order, its Attachments, and the
cited Master Agreement (as amended, if applicable) which is incorporated herein by reference as If it
had been set out in its entirety.
d) Whenever the Work Order conflicts with the cited Master Agreement, the Master Agreement shall
prevail.
e) CONTRACT PRICE:
1) COUNTY shall pay CONTRACTOR for performance of the Work in accordance with the Contract
Documents on the basis of the Total Bid (Work Order Amount). The CONTRACTOR's total
compensation is DOLLAR ($) subject only to increases or decreases made in strict conformance with
the Contract Documents.
li) CONTRACTOR agrees to accept the Contract Price as full compensation for doing all Work, furnishing
all Materials, and performing all Work embraced in the Work Order Documents; for all loss or damage
arising out of performance of the Work and from the action of the elements or from any unforeseen
or unknown difficulties or obstructions which may arise or be encountered In the prosecution of the
Work until the Final Acceptance; and for all risks of every description connected with the Work.
III) The CONTRACTOR acknowledges that CONTRACTOR studied, considered, and included in
CONTRACTOR's Total Bid (Work Order Amount) all costs of any nature relating to:
1) performance of the Work under Central Florida weather conditions;
2) applicable law licensing, and permitting requirements,
3) the Project site conditions, Including but not limited to, subsurface site conditions;
4) the terms and conditions of the Contract Documents, including, but not limited to, the
indemnification and no damage for delay provisions of the Contract Documents.
iv) The CONTRACTOR acknowledges that performance of the Work will Involve significant Work adjacent
to, above, and In close proximity to Underground Facilities Including utilities which will require the
support of active utilities, as well as, the scheduling and sequencing of utility Installations, and
relocations (temporary and permanent) by CONTRACTOR.
1) In addition to the acknowledgements previously made, the CONTRACTOR acknowledges that
the CONTRACTOR's own study of Underground Facilities, utilities in their present, relocated
temporary and permanent) and proposed locations, and conflicts relating to utilities and
Underground Facilities.
2) The CONTRACTOR acknowledges that CONTRACTOR's Total Bid (Work Order Amount)
considered and included all of CONTRACTOR's costs relating to CONTRACTOR's responsibilities to
coordinate and sequence the Work with the work of the COUNTY and its own forces, the work of
other utility contractors and the work of others at the Project site.

f) PAYMENT PROCEDURES.
i) CONTRACTOR shall submit applications for payment in accordance with Section 14 of the General
Conditions. Applications for Payment will be processed by ENGINEER as provided in the General
Conditions.
II) Progress Payments. COUNTY shall make progress payments on the basis of CONTRACTOR's
applications for payment as recommended by ENGINEER, In with Section 14 of the General
Conditions.
III) Final Payment. Upon Final Completion and acceptance of the Work In accordance with subsection
14.9.1 of the General Conditions, COUNTY shall pay the remainder of the Contract Price as provided
In subsection 14.9.1.
g) ADDITIONAL RETAINAGE FOR FAILURE TO MAINTAIN PROGRESS ON THE WORK.
i) Retainage under the Contract Documents is held as collateral security to secure completion of the
Work.
ii) In the event that CONTRACTOR fails to physically mobilize to the Work required by Section 6.19 of
the General Conditions, then the COUNTY may withhold retainage to secure completion of the Work
In an amount equal to the product of the number of Days after the 31s' Day following the Date of
Commencement of Contract Time liquidated damage amount for Substantial Completion set forth in
Section 9 of this agreement. The additional retainage shall be withheld from the initial and each
subsequent Progress Payment. The additional retainage held under this subsection shall be released
to CONTRACTOR In the next Progress Payment following the ENGINEER's approval of a
supplementary Progress Schedule demonstrating that the requisite progress will be regained and
maintained as required by Section 6.19.2 of the General Conditions.
iii) COUNTY may withhold additional retainage In anticipation of liquidated damages equal to the product
of the number of Days after the scheduled Contract Time (Substantial Completion or Final
Completion) and the amount of liquidated damages set forth in this Agreement If CONTRACTOR is
behind schedule and it is anticipated by COUNTY that the Work will not be completed within the
Contract Time. The additional retainage, under this subsection, may at the COUNTY's discretion be
withheld from subsequent Progress payments. Any additional retainage held under this subsection
shall be released to CONTRACTOR in the next Progress Payment following the Project Manager's
approval of a supplemental Progress Schedule demonstrating that the requisite progress will be
regained and maintained as required by Sections 6.19.2 of the General Conditions.
h) LIQUIDATED DAMAGES.
i) The COUNTY and CONTRACTOR recognize that time Is essential to the performance of this
Agreement and CONTRACTOR recognizes that the COUNTY and Its traveling public will suffer financial
loss if the Work is not substantially completed as described in subsection 14.13 of the General
Conditions and within the time specified in this Agreement, plus any extensions thereof allowed in
accordance with Section 12 of the General Conditions. The parties also recognize the delays, expense
and difficulties involved in proving in a legal or alternative dispute resolution proceeding the damages
resulting from inconvenience to the traveling public Including traffic loading, intersection operations,
costs for time, costs of fuel and costs for some environmental Impacts (excluding actual delay
damages which may include, but are not limited to, engineering fees and inspection costs) suffered
by the COUNTY If the Work is not competed on time. Accordingly, CONTRACTOR and CONTRACTOR's
Surety agree to pay COUNTY as liquidated damages, and not as a penalty, —Two -Hundred Fifty
Dollars ($250.00) per Day for each Day CONTRACTOR exceeds the Contract Time for Substantial
Completion until the Work is Substantially Complete. It Is agreed that if this Work in not Finally
Completed In accordance with the Contract Documents, the CONTRACTOR shall pay the COUNTY as
liquidated damages for delay, and not as a penalty, one-fourth (1/4) of the rate set forth above.
il) The liquidated damages provided In this Section are intended to apply even if CONTRACTOR is
terminated, in default, or if the CONTRACTOR has abandoned the Work.

Nl r Floodplain development application completed and signed if any portion of the property is in a flood
hazard area as identified on the most current flood insurance rate map.
1 I/ Copy of a contract, signed by the contractor and he property owmr, iddicating the documented

Application must include correct address and complete parcel I.D. number.
Contractor information is required to be included on the permit application (if contractor is applicant).
Applicant must include the name of the designated plan review contact person, their phone number and
either a fax number or email address on the Building Permit Application form.
Copy of the contractor's license issued by the State of Florida (if contractor is applicant).
A site specific notarized power of attorney shall be required from the licensed contractor if
he/she appoints an employee of his/her company to sign the permit application as the contractor.
Certificate of insurance indicating worker's compensation insurance coverage and naming the City of
Sanford as certificate holder, or copy of a worker's compensation exemption issued by the State of
Florida (must be submitted with each application if contractor is the applicant).
1 Completed and signed Owner Builder Statement / Affidavit (if owner is applicant).
Two (2) copies of all applicable plans and related documentation.
C, An accurate, signed and sealed, property survey which shows all improvements on the subject property
and within 10 feet on adjacent parcels.
Plot plan showing location of proposed improvement(s) and setbacks to property line(s). May need to
include infill lot requirements.
Hand -drawn submittals must be submitted on plain white paper and include accurate dimensions, all
details that apply to the project, and must be legible.

I. This product has been evaluated and is in compliance wilh the 5th Edition (2014( Florida Building Code
FBC( structural requirements including the "High Velocity Hurricane Zone" (HVHZ(.
2. Product anchors shall be as listed and spaced as shown on details. Anchor embedment to base material
shall be beyond wag dressing or stucco.
3. When used In the "HVHr' this product complies with Section 1626 of the Florida Budding Code and does not
require an impact resistant covering.
4. When used in areas outside of the "HVHZ" requiring wind borne debris protection this product complies with
Section 1609.1.2 of the FOC and does not require an impact resistant covering. This product meets missile
level "D" and includes Wind Zone 4 as defined in ASTM El 996 and Section 1609.1.2.2 of the FBC.
5. For 2x stud framing construction, anchoring of these units shag be the some as that shown for 2x buck
masonry construction.
6. Site conditions that deviate from the details of Ihis drawing require further engineering analysis by a
licensed engineer or registered architect.
7. Oulswing configurations meet water infiltration requirements for "HVHr'.
8. Inswing configurations do not meet the water infiltration requiremenls for the "HVHT'. Inswing units shag be
Installed only in non -habitable areas or at habitable locations protected by an overhang or canopy such

SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction
materials. The documentation submitted has been reviewed and accepted by Miami -Dade County RER -
Product Control Section to be used in Miami Dade County and other areas where allowed by the Authority
Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami -Dade County Product Control
Section (In Miami Dade County) and/or the AHJ (in areas other than Miami Dade County) reserve the right
to have this product or material tested for quality assurance purposes. If this product or material fails to
perform in the accepted manner, the manufacturer will incur the expense of such testing and the AHJ may
immediately revoke, modify, or suspend the use of such product or material within their jurisdiction. RER
reserves the right to revoke this acceptance, if it is determined by Miami -Dade County Product Control
Section that this product or material fails to meet the requirements of the applicable building code.
This product is approved as described herein, and has been designed to comply with the Florida Building
Code, including the High Velocity Hurricane Zone.

sheets 1 through 12 of 12, dated 05/15/15, prepared by manufacturer, signed and sealed by Anthony Lynn
Miller, P.E., bearing the Miami -Dade County Product Control Section Approval stamp with the Notice of
Acceptance number and approval date by the Miami -Dade County Product Control Section.

LABELING: Each unit shall bear a permanent label with 'the manufacturer's name or logo, city, state,
model/series, and following statement: "Miami -Dade County Product Control Approved", unless otherwise
noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been
no change in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change
in the materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement
of any product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure
to comply with any section of this NOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami -Dade County, Florida, and
followed by the expiration date may be displayed in advertising literature. If any portion of the NOA is
displayed, then it shall be done in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its
distributors and shall be available for inspection at the job site at the request of the Building Official.

SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction
materials. The documentation submitted has been reviewed and accepted by Miami—Dade County RER—
Product Control Section to be used in Miami—Dade County and other areas where allowed by the Authority
Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami—Dade County Product
Control Section (In Miami—Dade County) and/ or the AHJ (in areas other than Miami—Dade County) reserve
the right to have this product or material tested for quality assurance purposes. If this product or material
fails to perform in the accepted manner, the manufacturer will incur the expense of such testing and the AHJ
may, immediately revoke, modify, or suspend the use of such product or material within their jurisdiction.
RER reserves the right to revoke this acceptance, if it is determined by Miami—Dade County Product
Control Section that this product or material fails to meet the requirements of the applicable building code.
This product is approved as described herein, and has been designed to comply with the Florida Building
Code, including the High Velocity Hurricane Zone.
