Application is hereby made to obtain a permit to do the work and installations as indicated. I certify that no work" or installation has
commenced prior to the issuance of a permit and that all work will be performed to meet standards of all laws regulating construction
in this jurisdiction. I understand that a separate permit must be secured for electrical work, plumbing, signs'. wells, pools,
furnaces, boilers, heaters, tanks, and air conditioners, etc.

NOTICE: In addition to the requirements of this permit, there may be additional restrictions applicable to this property that may be
found in the public records of this county, and there may be additional permits required` from other governmental entities such as water
management districts, state agencies, or federal agencies.
Acceptance of permit is verification that I will notify the owner of the property of the requirements of Florida Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee at the time of permit submittal. A copy of the executed contract is required
in order to calculate a plan review charge and will be considered the estimated construction value of the job at the time of submittal.
The actual construction value will tie figured based on the current ICC Valuation Table in effect at the time the.permit is issued, in
accordance with local ordinance. Should calculated charges figured off the executed contract exceed the actual construction value,
credit will be applied to your permit fees when the permit is issued.
OWNER'S AFFIDAVIT: I certify that all of the foregoing information is,accurate and that all work will
be done in compliance with all applicable laws regulating construction and zoning.

4. Electrical shall be installed to standards of the National Electric Code and is guaranteed free from short circuits for one year. No
other warranties are expressed or -implied in this Contract.
5. Electric Contractor is not responsible or liable in any way, for any part of the existing electrical system.
6. Electric Contractor shall make holes in the building to gain access for routing cables & conduits and shall not be held liable in any
way for labor or costs incurred for painting, texturing, repairing walls, ceilings or floors.
7. Time and materials work shall becharged $50.00 an,hour plus materials, Servicecalls.shall be charged X8 M -tor. -the Est
half-hour, then $50.00 per hour. $150.00 is minimum charge.
8. Permits, inspections and/or plans required for this work are the responsibility and liabilty of the Electrical Contractor.

9. Change orders: Any alteration or deviation from the above specifications involving extra costs will be made only upon written
agreement, and will become an extra charge above the contract price to be paid immediately.
10. Delay: Electric Contractor will be excused for any delay beyond our control. Thesedelays may include, but are not limited to Acts
of God, labor disputes, inclement weather, acts of owner or public authority, or other unforseen contingencies.
11;: Right:to, Stop, Work: If any payment under. this Contract is not made•when due; Electric Contractor may suspend work on,the job•:
until such time as all payments have been made. Any failure to make payment is subject to a claim against the property.
12. It is agreed that payment to Electric Contractor shall not be withheld due to any delay or failed payments to the party submitted to
in section 1.
13. Malicious mischief and vandalism on the job is the responsibility of the party submitted to in section 1. Electric Contractor shall
not he.responsible.for any damage xesulting therefrom. Party submitted to shall carry needed insurance.
14. Overdue accounts will be charged a late charge at the rate of 1.5% per month, or the maximum rate permitted by law, whichever is.
less.
15. In the event any party to this Contract commences any action, legal or otherwise, to collect the contract price, the prevailing party
shall be entitled to recover attorneys fees and all other costs incurred in connection with the action.
16. This Proposal shall be effective for thirty days from the date first set forth in this Contract. If this Proposal is not accepted within

17. The language of all parts of this Contract shall in all cases be construed as a whole, according to its fair meaning, and not strictly
for or against any of the parties. No provision shall be deemed dependant upon any other.
18. If any provision of this Contract is held by an arbitrator or court to be unenforceable, invalid or illegal, said provision shall be
deemed to be deleted and shall not affect the validity of the remaining provisions of this contract.
19. Substitution of Materials: Electric Contractor may substitute,materials without notice to the Owner/General in order to allow work

20. Party submitted to shall give written notice of a back charge to Electric Contractor within ten days of the act giving rise to a back
charge. If no such notice is given within the ten day period the back charge shall not be allowed.
21: In the event party.submitted to does not request work to begin and/or the project is not in good condition to commence the work
within 30 days after the acceptance of this Contract, Electric Contractor shall have the option to terminate this Contract.
