NOTICE: In addition to the requirements of this hermit, there may be additional restrictions applicable to this property that may be
found in the public records of this county, and there may be additional permits required from other governmental entities such as water
management districts, state agencies, or federal agencies.
Acceptance of permit is verification that I will notify the owner of the property of the requirements of Florida Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee at the time of permit submittal. A copy of the executed contract is required
in order to calculate a plan review charge and will be considered the estimated construction value of the job at the time of submittal.
The actual construction value will be figured based on the current ICC Valuation Table in effect at the time the permit is issued, in
accordance with local ordinance. Should calculated. charges figured off the executed contract exceed the actual construction value,
credit will be applic&to your permit fees when the permit is issued.

Purpose of Visit and Overview:
On Thursday, May 10, 2018 at 10:30 AM, at your requested, we visited your home to review the
contract work for the installation of an I-beam so that a column could be removed. We submitted
drawings for the work to be done. There are a few minor things that need to be done for the work to
pass inspection by the County.
Observations and Comments:
1. The 3 joists and beam the run perpendicular to the I-beam must be shimmed using metal shim and
bar stock so that the joists and beam at resting on the steel I-beam. Presently, there are gaps
between each member.
2. There is slight motion in the column. The reason is that the column next to the sliding door is
secured at the bottom (floor end) but not at the top.. Use two "L" brackets bolted to the web of the
steel I-beam and set to be against the concrete block wall. The "L" brackets can be 4" X 2" X 3/8"
thick. Bolt them using 2- 3/16" dia. steel bolts only enough to pass through the web and hold the 2
"L" brackets in place. At the concrete block end use 2-Tapcon 3/16" dia. Screws to secure the
column at the top.
3. There was no footing set at the screen door end. Therefore, build a footing, as called out in the
drawing at that end. Since there is a footing at the inner end, there will be no need to build another
footing at that end.
Conclusion:
We certify that the above will be sufficient for the finalizing of the work in this home. This letter must
be presented to the County for their final approval before making changes.

ARTICLE 1: PARTIES
This Agreement is made and entered into on this April 3, 2018, by and between Amod Saxena (Hereinafter referred
to as "Owner" and Central Florida Renovations, LLC (Hereinafter referred to as "Contractor"). In consideration of
the mutual promises contained herein, Contractor agrees to perform the following work:

Contractor.

1. People Authorized to Sign Change Orders: The following people are authorized to sign Change
Orders: Amod Saxena (Owner) Chuck Semans (CFL Renovations)
ARTICLE 4: CHANGES IN THE WORK
During the course of the project, Owner may order changes in the work (both additions and deletions). The
Contractor will determine the cost of these changes and submit a written change order to Owner for a written
approval. The owner may, at his discretion, have the Contractor perform the change order on a time and materials
basis.
ARTICLE 5: RATES CHARGED FOR TIME AND MATERIAL WORK
Journey Carpenter: $65 per hour, Labor: $35 per hour, Contractor; $100 per hour, Subcontractor: Amount charged
by Subcontractor. Contractor will charge for profit and overhead at a rate of 17% on all work performed on a Time
and Material basis., on both materials and labor rates as set forth in this paragraph. If at any time the Owner takes
over this Project, all Actual Project Time and materials will revert to Time and Materials work and billed. This will
have included all Profit and Overhead at 17% as well.
ARTICLE 6: DEVIATIONS FROM SCOPE OF WORK IN AGREEMENT DOCUMENTS
Any alterations or deviation from the Scope of Work referred to in the Agreement Documents involving extra costs
of materials or labor (including any overage on ALLOWANCE work) will be executed upon a written Change Order
issued by Contractor and should be signed by Contractor and Owner prior to the commencement of any additional
Work. This written Change Order will become an extra charge over and above the Total Agreement Price.
ARTICLE 7: ESCALATION CLAUSE FOR SPECIFIED BUILDING MATERIALS
The contract price for this construction project has been calculated based on the current prices for the component
building materials. However, the market for the building materials that are hereafter specified is considered to be
volatile, and sudden price increases or decreases could occur. The Contractor agrees to use his best efforts to obtain
the lowest prices from available suppliers but should there be an increase or decrease in prices of these specified
materials that are purchased after execution of contract for use on this construction project, the Owner agrees to pay
that cost increase to the Contractor, or any decrease will be credited to the Owner. Any claim by the Contractor for
payment of cost increase, as provided above, shall require written notice delivered by the Contractor to the Owner
stating the increase cost, building material or materials in question, and the source of supply, supported by invoices
or bills of sale.

C. PAYMENT OF CHANGE ORDERS

Payment for each Change Order is due upon acceptance of Change Order and submittal of invoice by Contractor to
Owner,
0. ADDITIONAL PAYMENTS FOR ALLOWANCE WORK AND RELATED CREDITS
Payment for work designated in the Agreement as ALLOWANCE work has been initially factored into the Total
Agreement Price and Payment Schedule set forth in this Agreement. If the actual cost of the ALLOWANCE work
exceeds the line item ALLOWANCE amount in the Agreement, the difference between the cost and the line item
ALLOWANCE amount stated in the Agreement will be added to the Total Agreement Price. If the cost of the
ALLOWANCE Work is lees than the ALLOWANCE line item amount listed in the Agreement, a credit will be
issued to Owner after all billings related to this particular line item ALLOWANCE work has been received by
Contractor. This credit will be applied toward Final Payment owing under the Agreement. Contractor profit,
overhead and any supervisory labor will not be credited back to Owner for ALLOWANCE work.
E. FINAL AGREEMENT PAYMENT
Final Agreement payment is due and payable upon "Substantial Completion" (Not final Completion) of all work
under contract, "Substantial Completion" is defined as being the point at which a Certificate of Occupancy is issued,
or Final Inspections have passed.
F. HOLD BACK FROM FINAL PAYMENT FOR PUNCH LIST WORK
At time of making the Final Agreement Payment, Owner may hold back 150% of the value of all punch list items.
Owner and Contractor will place a fair and reasonable value on each Punch List item at time of walk-through wi

Owner. Contractor and owner will then execute the Punch List Form. This 150% hold back for Punch List items
assures Owner that all Punch List work will be completed by Contractor in a timely manner.
G. PAYMENT FOR COMPLETED PUNCH LIST WORK
Payment far completed Punch List work items is due and payable with submittal of invoice for those completed
items, even though entire punch list may not be completed.
H. INTEREST CHARGES
Interest in the amount of 1.5% per month will be charged on all late payments under this Agreement. "late
Payments" are defined as any payment not received within 7 days of invoice from Contractor.
ARTICLE 9: MISCELLANEOUS CONDITIONS
A. CONFLICT OF DOCUMENTS:
If any conflict should arise between the Plans and this "Agreement and attached Specifications", then the terms and
conditions of this "Agreement and attached Specifications" shall be controlling and binding upon the parties to this
Agreement.
S. INSTALLATION OF OWNER -SUPPLIED FIXTURES AND MATERIALS:
Contractor cannot warranty any Owner -Supplied materials or fixtures (whether new or used). If Owner -supplied
materials and fixtures fail due to defect in the materials or fixtures themselves. Contractor will charge for all labor
and materials required to repair or replace both the defective materials or fixtures, and any surrounding work that is
damaged by these effective materials.
C. Control and Direction of Employees and Subcontractors:
Contractor or his appointed Supervisor shall be the sole supervisor of Contractor's Employees and Subcontractors
Owner must not order or request Contractor Employees and Subcontractors to make changes in the work. All
changes in the work are to be -first discussed with Contractor and then performed per the written Change Order
process as set forth in this Agreement.
D. Owner Coordination with Contractor:
Owner agrees to promptly furnish Contractor with all details and decisions about unspecified construction finishes,
and to consent to or deny changes in the Scope of Work that may arise so as not to delay the progress of Work.
Owner agrees to furnish Contractor with continual access to the job site.
E. Contractor Not to be Relied Upon as Architect, Engineer or Designer:
The Contractor is not an architect, engineer or designer. Contractor is not being hired to perform any of these
services. To the extent that Contractor makes any suggestions in these areas, The Owner acknowledges and

If the Contractor encounters conditions at the site that are (1) subsurface or otherwise concealed physical conditions
that differ materially from those indicated in the Contract Documents or (2) unknown physical conditions of an
unusual nature that differ materially from those ordinarily found to exist and generally recognized as inherent in
construction activities of the character provided for in the Contract Documents. The Owner will be notified in
writing as soon as possible and no later the 7 working days of the find.
ARTICLE 15: SUBCONTRACTORS
The Contractor may use subcontractors to perform specific portions of this contract that the Contractor cannot
perform with his personnel. The Owner may specify a subcontractor they wish to receive a proposal from for this
contract; however, the Contractor shall not be required to subcontract with anyone he has reasonable objection to. If
a subcontractor is selected he must carry all appropriate licenses and insurances required by local, city and state
licensing boards. The Contractor will verify once all documents are received. At that time a subcontract must be
written, reviewed and approved by Contractor and Subcontractor. Once approved and signed work will commence.
The contractor will not allow any Subcontractor or Subcontractor employees on premises unless first getting their
workers comp certificate naming Owner as a named insured.
Article 16: Warranty
Contractor provides a warranty on all Contractor and Subcontractor supplied labor and materials supplied on this
project for a period of one year following substantial completion of all work. No warranty is provided by Contractor
on any materials furnished by the Owner for installation. No warranty is provided on any existing materials that are
moved and / or reinstalled by the Contractor within the dwelling (including any warranty that existing/used materials
will not be damaged during the removal and reinstallation process). One year after substantial completion of the
project, the Owner's sole remedy (for materials and labor) on all materials that are covered by the manufacturer's
warranty is strictly with the manufacturer, not with the Contractor.
ARTICLE 17: ENTIRE AGREEMENT, SEVERABILITY, AND MODIFICATIONS
This Agreement represents and contains the entire agreement between parties. Prior discussions or verbal
representations by Contractor or Owner that are not contained in this Agreement are not part of this Agreement. In
the event that any provision of this Agreement is at any time held by a court to be invalid or unenforceable, the
parties agree that all other provisions under this Agreement will remain in full force and effect. Any further
modification of this Agreement must be made in writing and executed by Owner and Contractor in order to be valid
and binding upon the parties.
ARTICLE 18: NOTICE AND OPPORTUNITY TO REPAIR
Prior to any arbitration or any legal actions, the Owner must give the Contractor notice and opportunity to repair NY
defects. Owner shall describe the claim in writing in reasonable detail delivered to the Contractor and not mIT

that Contractor suggestions are merely options that the owner may want to review with appropriate design
professional for consideration. Contractor's suggestions are not a substitute for a professional engineering,
architectural, or design services, and are not to be retied on as such by Owner.
ARTICLE 10: WORK STOPPAGE AND TERMINATION OF AGREEMENT FOR DEFAULT
Contractor shall have the right to stop ail work on the project if payments are not made to Contractor in accordance
with the Payment Schedule in this Agreement, or if Owner repeatedly fails or refuses to furnish Contractor with
access to the job site and 1 or product selections or information necessary for the advancement of Contractor's work.
Simultaneous with stopping work on the project, the Contractor must give Owner written notice of the nature of
Owner's default and must also give Owner 14-day period in which to cure this default.
If work is stopped due to any of the above reasons (or for any other material breach of agreement by Owner) for a
period of 14 days, and the Owner has failed to take significant steps to cure his default, then Contractor may,
without prejudicing any other remedies Contractor may have, give written notice of termination of the Agreement to
Owner and demand payment for all completed work and materials ordered through the date of work stoppage, and
any other toss sustained by Contractor, including Contractor's Profit and Overhead at the rate of 17% on the balance
of the incomplete work under this Agreement. Thereafter, Contractor is relieved from all other contractual duties.
including all Punch List and Warranty Work.
Article 11: Lien Releases
Upon request by Owner, Contractor and Subcontractors will issue appropriate lien releases when payment for such
items has.been received from Owner.
ARTICLE 12: INSURANCE & LICENSES
Contractor warrants that he is duly licensed by the State of Florida, holding license #CBC1266696. The Contractor
shall provide Workman's Compensation and General Liability Insurance. Owner shall be furnished with insurance
certificates as named insured. Owner shall maintain "Builder's Risk" insurance that provides coverage to the work
in progress during the project against damage caused by fire, vandalism, and Acts of God such as hurricanes, floods,
etc.
ARTICLE 13: DISPUTE RESOLUTION AND ATTORNEY FEES
Any controversy or claim arising out of or related to this Agreement involving an any amount $5.000 or less will be
handle in small claims court in the state of Florida. Any amount larger than $5,000 must be settled by binding
arbitration administered by the American Arbitration Association in accordance with the Construction industry
Arbitration Rules. Judgement upon award may be entered in any Court having jurisdiction thereof. Prevailing party
in any legal proceeding related to this Agreement shall be entitled to payment of reasonable attomey's fees, cost and
expenses.

ARTICLE 1: PARTIES
This Agreement is made and entered into on this April 3, 2018, by and between Amod Saxena (Hereinafter referred
to as "Owner" and Central Florida Renovations, LLC (Hereinafter referred to as "Contractor"). In consideration of
the mutual promises contained herein, Contractor agrees to perform the following work:

specified in each written Change Order) Every change order needs to be approved and signed prior to work being
performed and / or billed.
C. Expiration of This Agreement
This Agreement will expire 30 days after the date above if not accepted in writing by Owner and returned to
Contractor.

1. People Authorized to Sign Change Orders: The following people are authorized to sign Change

ARTICLE 4: CHANGES IN THE WORK
During the course of the project, Owner may order changes in the work (both additions and deletions). The
Contractor will determine the cost of these changes and submit a written change order to Owner for a written
approval. The owner may, at his discretion, have the Contractor perform the change order on a time and materials
basis.
ARTICLE 5: RATES CHARGED FOR TIME AND MATERIAL WORK
Journey Carpenter: $65 per hour, Labor: $35 per hour, Contractor; $100 per hour, Subcontractor: Amount charged
by Subcontractor, Contractor will charge for profit and overhead at a rate of 17% on all work performed on a Time
and Material basis., on both materials and labor rates as set forth in this paragraph. If at any time the Owner takes
over this Project, all Actual Project Time and materials will revert to Time and Materials work and billed. This will
have included all Profit and Overhead at 17% as well.
ARTICLE 6: DEVIATIONS FROM SCOPE OF WORK IN AGREEMENT DOCUMENTS
Any alterations or deviation from the Scope of Work referred to in the Agreement Documents involving extra costs
of materials or labor (including any overage on ALLOWANCE work) will be executed upon a written Change Order
issued by Contractor and should be signed by Contractor and Owner prior to the commencement of any additional
Work. This written Change Order will become an extra charge over and above the Total Agreement Price.
ARTICLE 7: ESCALATION CLAUSE FOR SPECIFIED BUILDING MATERIALS
The contract price for this construction project has been calculated based on the current prices for the component
building materials. However, the market for the building materials that are hereafter specified is considered to be
volatile, and sudden price increases or decreases could occur. The Contractor agrees to use his best efforts to obtain
the lowest prices from available suppliers but should there be an increase or decrease in prices of these specified
materials that are purchased after execution of contract for use on this constriction project, the Owner agrees to pay
that cost increase to the Contractor, or any decrease will be credited to the Owner. Any claim by the Contractor for
payment of a cost increase, as provided above, shall require written notice delivered by the Contractor to the Owner
stating the increase cost, building material or materials in question, and the source of supply, supported by invoices
or bills of sale.

C. PAYMENT OF CHANGE ORDERS
Schedule of Values
Payment for each Change Order is due upon acceptance of Change Order and submittal of invoice by Contractor to
Owner.
D. ADDITIONAL PAYMENTS FOR ALLOWANCE WORK AND RELATED CREDITS
Payment for work designated in the Agreement as ALLOWANCE work has been initially factored into the Total
Agreement Price and Payment Schedule set forth in this Agreement. If the actual cost of the ALLOWANCE work
exceeds the line item ALLOWANCE amount in the Agreement, -the difference between the cost and the line item
ALLOWANCE amount stated in the Agreement will be added to the Total Agreement Price. If the cost of the
ALLOWANCE Work is less than the ALLOWANCE line item amount listed in the Agreement, a credit will be
issued to Owner after all billings related to this particular line item ALLOWANCE work has been received by
Contractor. This credit will be applied toward Final Payment owing under the Agreement. Contractor profit,
overhead and any supervisory labor will not be credited back to Owner for ALLOWANCE work.
E. FINAL AGREEMENT PAYMENT
Final Agreement payment is due and payable upon "Substantial Completion" (Not Final Completion) of all work
under contract. "Substantial Completion" is defined as being the point at which a Certificate of Occupancy is issued,
or Final Inspections have passed.
F. HOLD BACK FROM FINAL PAYMENT FOR PUNCH LIST WORK
At time of making the Final Agreement Payment, Owner may hold back 150% of the value of all punch list items.
Owner and Contractor will place a fair and reasonable value on each Punch List item at time of walk-through with

Owner. Contractor and owner will then execute the Punch List Form. This 150% hold back for Punch List items
assures Owner that all Punch List work will be completed by Contractor in a timely manner.
G. PAYMENT FOR COMPLETED PUNCH LIST WORK
Payment for completed Punch List work items is due and payable with submittal of invoice for those completed
items, even though entire punch list may not be completed.
H. INTEREST CHARGES
Interest in the amount of 1.5% per month will be charged on all late payments under this Agreement. "late
Payments" are defined as any payment not received within 7 days of invoice from Contractor.
ARTICLE 9: MISCELLANEOUS CONDITIONS
A. CONFLICT OF DOCUMENTS:
If any conflict should arise between the Plans and this "Agreement and attached Specifications", then the terms and
conditions of this "Agreement and attached Specifications" shall be controlling and binding upon the parties to this
Agreement.
B. INSTALLATION OF OWNER -SUPPLIED FIXTURES AND MATERIALS:
Contractor cannot warranty any Owner -Supplied materials or fixtures (whether new or used). If Owner -supplied
materials and fixtures fail due to defect in the materials or fixtures themselves, Contractor will charge for all labor
and materials required to repair or replace both the defective materials or fixtures, and any surrounding work that is
damaged by these effective materials.
C. Control and Direction of Employees and Subcontractors:
Contractor or his appointed Supervisor shall be the sole supervisor of Contractor's Employees and Subcontractors.
Owner must not order or request Contractor Employees and Subcontractors to make changes in the work. All
changes in the work are to be first discussed with Contractor and then performed per the written Change Order
process as set forth in this Agreement.
D. Owner Coordination with Contractor:
Owner agrees to promptly furnish Contractor with all details and decisions about unspecified construction finishes,
and to consent to or deny changes in the Scope of Work that may arise so as not to delay the progress of Work.
Owner agrees to furnish Contractor with continual access to the job site.
E. Contractor Not to be Relied Upon as Architect, Engineer or Designer.
The Contractor is not an architect, engineer or designer. Contractor is not being hired to perform any of these
services. To the extent that Contractor makes any suggestions in these areas, The Owner acknowledges and agrees

If the Contractor encounters conditions at the site that are (1) subsurface or otherwise concealed physical conditions
that differ materially from those indicated in the Contract Documents or (2) unknown physical conditions of an
unusual nature that differ materially from those ordinarily found to exist and generally recognized as inherent in
construction activities of the character provided for in the Contract Documents. The Owner will be notified in
writing as soon as possible and no later the 7 working days of the find.
ARTICLE 15: SUBCONTRACTORS
The Contractor may use subcontractors to perform specific portions of this contract that the Contractor cannot
perform with his personnel. The Owner may specify a subcontractor they wish to receive a proposal from for this
contract; however, the Contractor shall not be required to subcontract with anyone he has reasonable objection to. If
a subcontractor is selected he must carry all appropriate licenses and insurances required by local, city and state
licensing boards. The Contractor will verify once all documents are received. At that time a subcontract must be
written, reviewed and approved by Contractor and Subcontractor. Once approved and signed work will commence.
The contractor will not allow any Subcontractor or Subcontractor employees on premises unless first getting their
workers comp certificate naming Owner as a named insured.
Article 16: Warranty
Contractor provides a warranty on all Contractor and Subcontractor supplied labor and materials supplied on this
project for a period of one year following substantial completion of all work. No warranty is provided by Contractor
on any materials furnished by the Owner for installation. No warranty is provided on any existing materials that are
moved and / or reinstalled by the Contractor within the dwelling (including any warranty that existing/used materials
will not be damaged during the removal and reinstallation process). One year after substantial completion of the
project, the Owner's sole remedy (for materials and labor) on all materials that are covered by the manufacturer's
warranty is strictly with the manufacturer, not with the Contractor.
ARTICLE 17: ENTIRE AGREEMENT, SEVERABILITY, AND MODIFICATIONS
This Agreement represents and contains the entire agreement between parties. Prior discussions or verbal
representations by Contractor or Owner that are not contained in this Agreement are not part of this Agreement. In
the event that any provision of this Agreement is at any time held by a court to be invalid or unenforceable, the
parties agree that all other provisions under this Agreement will remain in full force and effect. Any further
modification of this Agreement must be made in writing and executed by Owner and Contractor in order to be valid
and binding upon the parties.
ARTICLE IS: NOTICE AND OPPORTUNITY TO REPAIR
Prior to any arbitration or any legal actions, the Owner must give the Contractor notice and opportunity to repair any
defects, Owner shall describe the claim in writing in reasonable detail delivered to the Contractor and not more than

HOOD EXHAUST DUCT: Outside ventilation requires a. HOOD
EXHAUST DUCT. Read the following carefully.
EXHAUST CONNECTION: The hood exhaust has been designed to mate
with a standard 3-1/4" x 10" rectangular duct. If a round duct is required, a
rectangular -to -round transition adaptor must be used. Do not use less than
a 6" diameter duct.
REAR EXHAUST: If a rear or horizontal exhaust is to be used, care should be
taken to align exhaust with space between studs, or wall should be prepared
at the time it is constructed by leaving enough space between the wall studs to
accommodate exhaust.
MAXIMUM DUCT LENGTH: For satisfactory air movement, the total duct
length of 3-1/4" x 10" rectangular or 6" diameter round duct should not exceed
140 equivalent feet.
ELBOWS, TRANSITIONS, WALL AND ROOF CAPS, etc., present
additional resistance to airflow and are equivalent to a. section of straight
duct which is longer than their actual physical size. When calculating the total
duet length, add the equivalent length of all transitions and adaptors plus the
lengths of all straight duct sections. The chart below shows the approximate
feet of equivalent length of some typical ducts.
