action be taken to collect any/all amounts due and unpaid, to include court cost and attorneys fees. Cosmetic repairs such as patching and painting are not
included in the proposal. Titan will make every effort in the performance of our work to prevent or minimize damage that could result from repairs. Titan is
not responsible for interior or exterior damage to windows, doors, tile, drywall, paneling, appliances, roofing, HVAC, landscaping or plumbing, etc. Any
additional costs incurred for removal and/or replacement of HVAC, electrical, plumbing, sewer, pool equipment, sprinkler system, utilities, or any other
underground unknowns are not included in this proposal, unless specified, and will be an additional expense. No allowance is included for dealing with
previous foundation repair efforts by others or design changes there of. Titan Complies with FLORIDA'S CONSTRUCTION LIEN LAW
(SECTIONS 713.001-713.37, FLORIDA STATUTES) If this contract should be breached after the allotted time frame, Titan Foundation Repair Specialist, LLC,
as the Contractor, is entitled to recover any and all applicable fees, (which include, but are not limited to, documenting fees, labor, travel, or

Please be aware that the Building Official requires an affidavit from the Engineer of record for the final
inspection for the stabilization of the building's foundation. No inspections will be performed` by the
Building Department. When work is complete, please provide the Building Department with a signed
and sealed affidavit from the Engineer of record indicating a positive conclusion for the final
inspection to complete this permit.
Whenever a permit is issued in reliance upon an affidavit or whenever the work to be covered by a
permit involves installation under conditions which, in the opinion of the building official, are
hazardous or complex, the building official shall require that the architect or engineer who signed the
affidavit or prepared the drawings or computations shall supervise such work. In addition, they shall be
responsible for conformity to the permit, provide copies of inspection reports as inspections are
performed, and upon completion make and file with the Building Official a written affidavit that the
work has been done in conformity to the reviewed plans and with the structural provisions of the
technical codes. In the event such architect or engineer is not available, the owner shall employ in his
stead a competent person or agency whose qualifications are reviewed by the Building Official. The
Building Official shall ensure that any person conducting inspections is qualified as a building
inspector under Part III of Chapter 468, Florida Statutes.

Investigation Findings
Sinkhole Geotech (SGT) was requested to check this residence to recommend the
appropriate remediation for the damage. SGT inspected the residence to study the nature
and potential causes of the damage. The damage to the residence is cosmetic in nature. To
the best of my knowledge, there no previous foundation work has been done. With only a
visual foundation inspection, the damage seem to be a result of long-term post construction
movement and for shrinkage or expansion cracks. The most significant damage is the
hairline cracks in two corners. The purpose of this Soil & Structure Assessment completed
by SGT for the project site was to evaluate the foundation and determine if the residence is
stabilized and habitable at this time. A two -stage engineering investigation was performed
which consisted of the elements listed below.
Stage I
• Performing a field inspection of the residential structure, selecting field test
locations, and reviewing existing information consisting of general subsoil
conditions in the project area and any other available project area related
information.
Stage II
• Evaluating the exterior and interior damage, if any,
• Obtaining representative soil samples from test locations, if needed,
® Grouting or filling all soil borings in general accordance with applicable guidelines,
as necessary,
s Reviewing soil samples of all selected samples, as necessary, to evaluate pertinent
engineering characteristics of the soil and assist in their classification, and;
o Compiling the field exploration data, providing an evaluation of sub -soil conditions
and summarizing comments, findings, and conclusions as they relate to the
completed repair, ongoing damages on the property.

The observed damage is consistent with differential movement of the foundation due to lack of
support from underlying soils. SGT was requested by the homeowner and/or his contractor to
provide a partial remediation plan. A remediation consists of 4 underpinning points as
requested by homeowner/homeowner contractor. The depth of the underpinning piers is a
function of the pressure exerted to barely lift the wall without damaging it. The underpinning
transfers the house load to more stable deeper soil and prevents further foundation
movement. Estimated total cost is in the range of $5,000 to $8,000.

Per your request, Sinkhole Geotech, Inc. (SGT) is pleased to submit the Foundation
Stabilization Report for monitoring partial underpinning pressure piers at the above
referenced site.
Per your request to provide underpinning pier plan to augment the foundation, SGT
monitored the partial underpinning plan. The underpinning transfers the house load to
more stable deeper soil and prevents further foundation movement.
The purpose of this report is to provide certification that SGT, as a third party, monitored
the partial foundation stabilization operation performed by Titan Foundation Repair
Specialist on behalf of the client or homeowner at the above referenced structure. This
report documents the pressure pier and the partial underpinning installation details of the
work completed. The approximate locations of the perimeter underpin piers are depicted
on the enclosed underpinning layout.
WHAT IS UNDERPINNING?
Underpinning is a process of augmenting the foundation underneath the building. There
are a variety of methods to accomplish this, each with its own benefits and drawbacks.
Generally, underpinning is a method of transferring the wall and roof loads on the
foundations to deeper soils or even to the limestone itself.

HOW DOES UNDERPINNING WORK?

Underpinning Involves attaching steel piers using pin brackets to the foundation of the
home, and driving those piers into deeper competent soils or to the limestone formation. If
correctly planed out and mounted, underpinning supplies the lift the structure or
foundation needs to meet a more satisfactory level, also providing a future support as a
preventative from settling of the underpinned area.
WHEN SHOULD UNDERPINNING BE USED?
There is a variety of reasons when underpinning should be used. When buildings are on
unpredictable land, there is often a possibility the foundation will become unstable. In this
situation, underpinning may be necessary. Here are a few examples of situations that would
require underpinning:
• Structure is not even: re -leveling the structure
• Cosmetic/structural cracks: closing of wide cracks caused by excessive settlement
• Soils have changed or cavities were created; bypass weak soil layers
• Soils have not been completely identified before construction began. pre -
construction underpinning
• The foundation may no longer be strong or stable enough for support of the
structure: foundation augmentation
• The structure's purpose may have changed: meeting the weight increase on the
foundation
• Structure needs additional foundation capacity to footings or mats with too little
bearing capacity (building movement or ground consolidation is excessive)
• Distressed or undermined Structure needs support; a catastrophic event (e.g.
erosion, landslides, burst pipes, mine subsidence, etc.)
• Adding load capacity to an existing foundation system to accommodate a seismic
retrofit or addition of new structure, building stories, etc.
• Cost considerations: homeowner preference to secure the foundation

At the Short Street Investments LLC Property, the underpin piers were hydraulically driven
down to refusal or a maximum pressure into the underlying soils. The pier depths ranged
from 16 feet to a maximum of 26 feet below foundation (see appendix B). The plan utilized
4 underpinning points as shown in the As -Built Plan,
The jacking of each pier was terminated at a pressure that was appropriate to avoid
damaging the structure based the condition of the foundation at each location. The piers
were jacked to pressure up to approximately 3,000 psi, which provides more than 24,000
pounds of foundation support per pier. When the piers reached the ultimate support depth,
each underpin bracket was sedured to the pressure pier with a 1/2" pin. The jacking depth

and pressures of each individual pier are presented in tabular form in the appendix of this
report.
In conclusion, to the best of our belief or knowledge, Titan Foundation Repair Specialist has
completed this partial underpinning installation in accordance with applicable standard
practice and the foundation has been stabilized and habitable at this time. Therefore, it is
also our opinion that there Is no visual evidence of new or continued settlement activity at
this corner location at this time. This documentation is limited to the partial underpinning
installation only and not intended to deal with problem behind the limits of this partial
underpinning repair. Our opinion is based on a reasonable professional probability from
similar conditions and similar locations. In addition, it is not intended to remediate soil
conditions that may or may not exist at the site such as buried debris, organic matter, or
improper foundation design or embedment. No structural analysis has been performed to
determine the structural integrity of covered structural members. SGT does not imply any
warranty to the effectiveness of work performed nor should any be implied, but only
accuracy of the information reported herein. We cannot be responsible for unforeseen soil
shifts, movements, or building cracking that may result as part of implementing this
stabilization process.
