In talking with your DPBR staff at your Call Center, the suspension was rescinded after fine payment today, April 17, 2018, and was placed on suspension February 12, 2018. From Mr. Adam's email request for a DOH inspection below received by my staff on April 3, it appears that this contractor was conducting unlicensed work during the time frame of license suspension. All of the items on the inspection report must be corrected or you must be granted variance approval before we can issue an initial operating permit. The meetings are the second Wednesday of odd months (upcoming meeting is May 9, 2018 1 have also attached the notice). You must contact the variance board coordinator to find out the last date for submission to be added to the agenda. You will need to submit to the coordinator: 8 copies of the completed variance application, 8 copies of supporting documentation for the variance, a check made out to the Florida Department of Health for $300, and a letter of authorization to speak on behalf of the owner of the project.
