All of the items on the inspection report must be corrected or you must be granted variance approval before we can issue an initial operating permit. The meetings are the second Wednesday of odd months (upcoming meeting is May 9, 2018 1 have also attached the notice). You will need to submit to the coordinator: 8 copies of the completed variance application, 8 copies of supporting documentation for the variance, a check made out to the Florida Department of Health for $300, and a letter of authorization to speak on behalf of the owner of the project.
