NOTICE: In addition to the requirements of this permit, there may be additional restrictions applicable to this property that may be found in the public
records of this county, and there may be additional permits required from other governmental entities such as water management districts, state
agencies, or federal agencies.
Acceptance of permit is verification that I will notify the owner of the property of the requirements of Florida Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee at the time of permit submittal. A copy of the executed contract is required in order to
calculate a plan review charge and will be considered the estimated construction value of the job at the time of submittal. The actual construction value
will be figured based on the current ICC Valuation Table in effect at the time the permit is issued, in accordance with local ordinance. Should calculated
charges figured off the executed contract exceed [he actual construction value, credit will be applied to your permit fees when the permit is issued.

This license was suspended on your online site yesterday, and now today the license is current, active.
In talking with your DPBR staff at your Call Center, the suspension was rescinded after fine payment today, April 17,
2018, and was placed on suspension February 12, 2018.
From Mr. Adam's email request for a DOH inspection below received by my staff on April 3, it appears that this
contractor was conducting unlicensed work during the time frame of license suspension.
Can you call me? As I cannot get through to you. Also, can you send me a copy of the Board's FINAL ORDER for this
contractor?

Wyndham Preserve Pool Construction Inspection 4/6/2018 Good
Afternoon, I
have attached a copy of the inspection from today for the Wyndham Preserve Pool which was not constructed to meet
commercial code requirements. All of the items on the inspection report must be corrected or you must be granted
variance approval before we can issue an initial operating permit. I
have provided the following link to the Variance Application DH 4080 (if the link does not work you can copy and paste this
address in your web browser to access the file http://www.floridahealth.gov/environmental-health/swimming- pools/
documents/dh4080-O9.pdf). The
meetings are the second Wednesday of odd months (upcoming meeting is May 9, 2018 1 have also attached the notice).
You must contact the variance board coordinator to find out the last date for submission to be added to the agenda.
You will need to submit to the coordinator: 8 copies of the completed variance application, 8 copies of supporting
documentation for the variance, a check made out to the Florida Department of Health for $300, and a letter of
authorization to speak on behalf of the owner of the project.
