commenced prior to the issuance of a permit and that all work will be pertbrmed to meet standards of all laws regulating construction
in this jurisdiction. I understand that a separate permit. must be secured for electrical work, plumbing, signs, wells, pools,
furnaces, boilers, heaters, tanks, and air conditioners, etc.

NOTICE: In additiontothe requirements of this permit, there may be additional restrictions applicable to this property that may be found
in the public records of this county, and there may be additional permits required from othergovernmentalentities such as water management districts,
state agencies, or federal agencies. Acceptance of
permit is verification that I will notify the owner of the property of the requirements of Florida Lien Law, FS 713. The City
of Sanford requires payment of plan review fee at the time of permit submittal. A copy of the executed contract is required in carder
to calculate a plan review charge and will be considered the estimated construction value of tire job at the time of submittal. The actual
construction value will be figured based on the current ICC Valuation 'fable in effect at the time the permit is issued, in accordance with
local ordinance. Should calculated charges figured off the executed contract exceed the actual construction value, credit will
be applied to your permit fees when the permit is issued. OWNER'S

TERMS & CONDITIONS OF AGREEMENT
1. Purchaser hereby accepts the equipment and service described above and agrees to pay Facemyer Air Conditioning and Heating the price
shown above.
2. All equipment and material are guaranteed by Facemyer Air Conditioning and Heating to be as specified. All work will be completed in a
workmanlike manner according to normally accepted practices.
3. Materials and work in addition to that described herein will be furnished only on Purchaser's authorization and will be paid by Purchaser
as an extra charge.

4. Upon failure to pay any sums due hereunder, Purchaser agrees to pay Facemyer Air Conditioning and Heating interest at the rate of one
and one half percent (1'h%) per month (annual rate of 18%) on all outstanding balances.
5. Facemyer Air Conditioning and Heating shall not be liable for any default caused by events beyond its control, including but not limited
to, fire, flood, strikes, accidents, or delays affecting this work or other operations in which it is involved, directly or indirectly.
6. Purchaser shall permit Facemyer Air Conditioning and Pleating reasonable access to the property on which equipment is to be installed.
Title to all provided equipment remains with Facemyer Air Conditioning and Heating until all amounts due thereon are paid in full, whether
such equipment is affixed to the realty or not, and shall remain personal property and be deemed sever -able without injury to the freehold. On
any payment default by Purchaser, or if in Facemyer Air Conditioning and Heating's judgment, reasonably exercised, its equity appears to be
imperiled, then, Facemyer Air Conditioning and Heating may without further notice enter the premises and remove or resell the equipment,
and Purchaser shall be liable for any deficiency or loss sustained by Facemyer Air Conditioning and Heating in connection therewith.
7. Once equipment is connected at Purchaser's property, Purchaser assumes all risk of loss or damage to such equipment and shall insure
same fully to protect all interests of Facemyer Air Conditioning and Heating, cost of insurance to be paid by Purchaser. Facemyer Air
Conditioning and Heating carries liability insurance and Worker's Compensation Insurance.
8. Facemyer Air Conditioning and Heating provides a one-year limited labor warranty. Equipment or system failure due to lack of proper
maintenance service or abuse is expressly excluded. Normal maintenance check-ups and filter replacements are the responsibility of
Purchaser. All other warranties, expressed or implied, are the responsibility of the manufacturer of the equipment, parts, or materials used in
connection with the services.
9. There are no warranties, expressed or implied, for existing equipment, ductwork, or other materials not installed by Facemyer Air
Conditioning and Heating
10. All warranty work will be performed during Facemyer Air Conditioning and Heating's normal working hours, 8:OOAM to S:OOPM,
Monday through Friday.
11. Purchaser is responsible for all costs and reasonable attorney fees incurred by Facemyer Air Conditioning and Heating in connection with
any action or proceeding (including arbitration and appeals) arising out of this agreement, including collection of any outstanding amounts
due, whether or not suit is filed.
12. Except as provided herein Facemyer Air Conditioning and Heating makes no other representations or warranties, either express or
implied, including, but not limited to, any implied warranties of merchantability or fitness for a particular purpose Facemyer Air
Conditioning and Heating expressly disclaims all other warranties. Facemyer Air Conditioning and Heating's maximum liability hereunder
shall consist of refunding all moneys paid to it by Purchaser hereunder subject to removal and return to Facemyer Air Conditioning and
Heating of all equipment provided hereunder. Under no circumstances will Facemyer Air Conditioning and Heating be liable to Purchaser or
any other person for any damages, including, without limitation, any indirect, incidental, special, or consequential damages, expenses, cost,
profits, lost savings or earnings, lost or corrupted data, or other liability arising out of or related to this agreement, or the services or
equipment provided hereunder.
13. This agreement shall be governed and construed solely according to the internal laws of the State of Florida, without reference to any
conflicts of laws.
14. This agreement is the complete and exclusive statement of the agreement between Purchaser and Facemyer Air Conditioning and Heating
and it supersedes all prior oral and written proposals and any prior or subsequent communications pertaining to the subject matter hereof.
Governor Rick Scott recently signed House Bill 535, which includes important updates to sections 489.1401-143,F.S., permitting payments
to injured residential consumers from the Florida Homeowners Recovery Fund for both Division I and Division II scopes of work.
Effective July I, 2016, all licensed contractors, both Division I and Division 11, must ensure that the Florida Homeowner Construction
Recovery Fund Notice is contained in their residential contracts.
According to section 489.1425(1): Each agreement or contract for repair, restoration, improvement, or construction to residential real
property must contain a written statement explaining the consumer's rights under the recovery fund, except where the value of all labor and
materials does not exceed $2,500. The written statement must be substantially in the following form:
