refer individually to one of the following corporations as evidenced on the signature page of this Agreement: "Lowe's Home Centers, Inc." (a North Carolina corpora-
tion and wholly owned subsidiary of Lowe's Companies, Inc.,:a North=Carolina corporation);"`Lbwe's HIW; I:nc." (a Washington corporation and wholly owned subsid-
iary of Lowe's Companies,_Inc., a North Carolina, corporation) or "The Contractor Yard, -Inc."_ (a North Carolina corporation and wholly owned subsidiary of Lowe's
Companies, Inc., a North Carolina corporation).
GENERAL DESCRIPTIONI_By this installed sales contra60he "Contract"), Customer•agreeS •fo purchase and Lowe's agrees to sell the goods andl_or materials

and any attached sketches,`materials lists, floor plans,,and%or specification sheets.
INSTALLATION SERVICES. Customer authorizes Lowe's (a) -to— arrange for the Installation Services to be performed by an experienced Installer (licensed when
legally'required);''(b) to issue`a work order to the Installer -to perform the InstallationServices,-(c) to inspect the Installer's work, and (d) to;pay the,installer after
completion of the work and after receipt of a certificate, signedanddated by Customer, `thatwo the:rkl`fas-been satisfactorily completed (the:"Certificateof Comple- tion"). Customer
understands that Lowe's will rely upon the Certificate of Completion in paying the Installer for the Installation Services. , GOODS. Lowe'
s will arrange for delivery c f the Goods to the Premises. Any surplus mate rials_upon completion of the..l'nstailation S_ ervices shall remain the property of Lowe'
s and shall be returned to Lowe's by the Installer. PRICE. The
Price owed by Customer to Lowe.°s covers the Goods, Installation Services, applicable,taxes,and the cost of purchasing any required building permit. The Price
assumes sound existing substructures, superstructure and points of attachments The`p'ric6 shall be -increased by the cost and reasonable profit to Lowe's of having
to provide additional Goods and/or Installation Services as a result of defective "substructures, superstructures, or points of, attachments. PAYMENT. Payment
of the Price, by Customer to Lowe's is due in full upon execution of the Contract. WAIVER OF
LIENS FOR CONTRACTS OF $3,000 OR'LESS. If INS Contract is for a Price $3 000.or less Lowe s'"on behalf of itself; its Installer, and any subcontractor(s),
hereby fully and unconditionally relinquishes,. waives and releases any andall m'edhanic's.If ' ' ', material" ' If' 'and other liens in he Prerriises which Lowe'
s might have or acquire, by operation of law or otherwise, as a result of h'is Contract. In addition, because responsibility for paying "its li sfallerbelongs toLowe'
s and not to Customer, Lowe's has required the Installer, on behalf of itself and any subcontractor(s), to.fully and unconditionallyielinquish, waive and release any and
all mechanic's liens, materialman's liens and other liens in the Premises which the Installer or,subcontractormight have or-,a_cquire.now;or in the future_, by operation of
law or otherwise, as a result of this Contract. ENTIRE AGREEMENT.
This Contract accurately states the entire agreement between Customer and Lowe's concerning, this sale of -.Goods and Installation Services and
replaces and supersedes;all prior agreements and understandings relating'therefo,both oral and written. Any additions or changes to this Ccntractmust be in
writing signed by Customer and Lowe's. , `t ; MANUFACTURER WARRANTY=
FOR GOODS / LOWE'S WARRANTY FOR INSTALLATION SERVICES. Customer is entitled to any warranty provided by a manufacturer of
the Goodssold: un rer.this Contract: The Installer will provide Customer,with any manufacturer consumer warranty information accompanying the Goods,'and
Customer, mayalso obtain.such information by contacting,Lowe's. Lowe's does not Warrant the Goods. L'owe's do iswarianithat-the Installation Services will be
performed by the Installer in a.good and "workmanlike manner." Lowe's warranty for Installation Services shall extend for a period of one year from the date the Certificate of
Completion is signed by Customer or for such greater period as may be required by applicable°law governing consumer warranties for workmanship'(the Warranty Period").
Customer must give Lowe's written notice within the Warranty Period of any warranty claim relating to Installation Services. Customer agrees that its sole
and exclusive rem ed"y:against.Lowe's for a warranty claim is reinstallation in a good and workmanlike manner, includinOhe`repa rYorreplacement of any -Goods if and
to the extent reasonably necessary to correct the defective Installation Services. Customer shall have no other, remedy against Lowe's for a warranty claim, including
without limitation remedy for loss or damage caused by normal wear and fear; loss`or damage vbhfch has not been reasonably mitigated, loss or
damage caused by acts of God, incidental or consequential damages for lost profits, sales, injuries to persons or property, or any other incidental or consequential
damages. Lowe's
warranty for Installation Services shall be in lieu of any other warranty expressed or implied, including without -limitation -any implied warranty of merchantability
or fitness for a particular purpose. In connection with anywarrantyclaim, Customer agrees, at no cost to Lowe s, io.prepare the. Premises; including the removal
and replacement of fixtures incident to the repair,area, so tti'at Lowe's, or its designee, can correct the defective Installation Services without undue delay. LICENSES
PERMITS SAFETY
RULES BUILDING CODES ZONING ORDINANCESAND OTHER LAWS: Lowe's shallee'responsit le to:Customer for obtain, ing any and all
licenses and building permits which are legally required to perform the Contract. If Customer obtains permits on his own;.Customer,will be_precluded from claiming against certain
state guaranty funds relating to home improvements. Lowe's shall also be responsible to Customer for the Contract's being performed in compliance with all
applicable safety rules and all existing building codes, zoning ordinances and oth&-.laws: Lowe'9!9hali not be responsible for-`ariy one existing violations of safety rules,
building codes, zoning, ordinances or,other.laws •,rfia,.,ctrange occurstosany applicable safety.rule; buitding.ccde;zoning ordinance or other law which requires additional
Goods and/or Installation Services to perform the Contract, Customer agrees to payLowe_s the -costa and reasonape profit for such additional Goods and Installation
Services and to execute a resulting change order or new replacement. Contr cjas,requested by Lowe's. CUSTOMER'S WARRANTY AGAINST
VIOLATIONS OF EASEMENTS, COVENANTS, AND THIRD PAR Y RIGHTS Customer warrants that performance of this Contract by Lowe's

agrees to pay Lowe's the cost and reasonable profit for such additional Goods andInstallation Services and to execute a resulting change order or
new replacement Contract as requested by Lowe's. ARBITRATION. If -Customer has
any questions or complaints regarding the Contract, Customer -may contact the Lowe's Manager or Installed Sales Specialist whose n e and
telephone numberappear,'elsewhere on,the Contract: All disputes;and claims between Customer and Lowe's concerning this;Contract which either party believes cannot be
resolved informally, including without+imitation any warranty claims, shall be resolved by binding arbitration conducted by a single arbitrator under the auspices, rules
and procedures'of`the;Ame-rican ArbitratibWAssociation and Jn•;accordance:with applicable -federal and 'State arbitratiori'statutesr'The arbitration shall be held
in the city or county where the -Premises are located was made- or. in -such -other-convenient location" asthe-parties may- mutually agree. No discovery^'shall tie 4ilowed
except as maybe agreed to in writing by the parties. Either paitq`maydemandarbitration; and the arbitrator's final -award 9hall'be' issued within ninety (90) days
after service of the arbitration demand on the other party. REMEDY FOR BREACH. if
Customer breaches this Contract, Lowe's, or -its assigns; -shall be entitled to recover the greater of liquidated damages in the amount oQ0% of the total .
Contract price or such actual damages as Lowe's, or its assignees, prove. Also, if Customer fails'to pay Lowe's in accordance with this Contract; Lowe's shall be
entitled to recover its legal costs, including reasonable attorney's fees, in connection with arbitrating, obtaining judgment on an arbitration award, of otherwise pursuing Customer for
collection. Customer agrees that Lowe's reasonable attorney's fees shall constitute 15% of the amount owed under the Contract; arbitration award_or judgment.
UNAVOIDABLE DELAY OR FAILURE
IN PERFORMANCE EXCUSED. Any delay or failure by Lowe's or its Installer in performing this Contract because of strike, fire, flood, acts of
God, inability to obtain Goods, or any other causes beyond the reasonable control of Lowe's or its Installer shall be excused and shall not be breaches of this Contract. --
MISCELLANEOUS. This Contract shall
be interpreted under and governed by the law of North Carolina. If any provision of this Contract is, contrary. to any law to which it is subject;
such unlawful provision shall be ineffective, without invalidating the.other provisions which -shall, remain in,full forge and effect ; . , All parties to this
Agreement acknowledge that if thisAgreement is executed; or tothe extent ;this Agreement is to.be performed, within the states of Alaska Arizona, California, Colorado, Hawaii, Ida
ho,"Montana,; Nevada, Oregon;• Utah; -Washington, Wyoming,, it is an Agreement.exclus vely with Lowe's HIW, Inc. and no rights or remedies shall be available
from any other Lowe's entity. If this Agreement is executed, or. to the extent this Agreement is to be performed, outside of Alaska, Arizona, California; Colorado, Hawaii, Idaho,
Montana, Nevad1. a, Oregon, Utah; Washington, Wyoming it is an Agreement with Lowe's Home Centers, Inc: and no rights or remedies,shall be available

NOTICE OF ACCEPTANCE: SPECIFIC CONDITIONS
1. SCOPE
1.1 This renews the Notice of Acceptance No. 00-0321.26 which was issued on April 28, 2000. It
approves a residential insulated door, as described in Section 2 of this Notice of Acceptance,
designed to comply with the South Florida Building Code (SFBC), 1994 Edition for Miami -Dade
County,. for the locations where the pressure requirements, as determined by SFBC Chapter 23, do
not exceed the Design Pressure Rating values indicated in the approved drawings.
2. PRODUCT DESCRIPTION
2.1 The Series Entergy 6-8 S-W/E Inswing Opaque Double Residential Insulated Steel Doors with

compliance with the following documents: Drawing No 31-1029-EW-I, Sheets 1 through 6 of 6,
titled "Premdor (Entergy Brand) Double Door with Sidelites in Wood Frames with Bumper
Threshold (Inswing);" prepared by manufacturer, dated 7/29/97 with revision C dated0I/11i00, bearing
the Miami=Dade County Product Control approval stamp with the Notice of Acceptance numberandapprovaldatebytheMiami -Dade County Product Control Division. These documents shall
hereinafter be referred to as the approved drawings. j
3. LIMITATIONS 3.
1 This approval applies to single unit, applications of pair of doors and single door only, as shown in approved
drawings. Single door units shall include all components described in the active leaf of i this
approval.. 3.
2 'Unit shall be installed only at locations protected by a canopy or overhang such that the angle between
the edge of canopy or overhang to sill is less than 45 degrees. Unless unit is installed in he

LABELING

Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and following statement: "

NOTICE OF ACCEPTANCE: STANDARD CONDITIONS
1. Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed
and the original submitted documentation, including test supporting data, engineering documents,
are no older than eight (8) years.
2. Any and all approved products shalt be permanently labeled with the manufacturer's name, city,
state, and the following statement. Miami -Dade County Product Control Approved", or as
specifically stated in the specific conditions of this Acceptance.
3. Renewals of Acceptance will not be considered if: 1
a. There has been a change in the South Florida Building Code affecting the evaluation of this
product and the product is not in compliance with the code changes.
b. The product is no longer the same product (identical) as the one originally approved.
C. If the Acceptance holder has not complied with all the requirements of this acceptance,
including the correct installation of the product.
d. The engineer who originally prepared,. signed and sealed the required documentation initially
submitted, is no longer practicing the engineering profession.
4. Any revision or change in the materials; use,,,and/or manufacture of the product or process shall
automatically be cause for termination of this Acceptance, unless prior written approval has been
requested (through the filing of a revision application with appropriate fee) and granted by this i
office.
5. Any of the following shall also be grounds for removal of this Acceptance:
a: Unsatisfactory performance of this product or process.
Misuse of this Acceptance as an endorsement of any product, for sales, advertising or anyb• P
other purposes.
6. The Notice of Acceptance number preceded by the words Miami -Dade County, Florida, and
e followed by the expiration' date may be displayed in advertising literature. If any portion of the
Notice of Acceptance is displayed, then it shall be done in its entirety.

7. A copy of this Acceptance as well as approved drawings and other documents, where it applies,
shall be provided to the user by the manufacturer or its distributors and shall be available for
inspection at the job site at all time. The engineer needs not reseal the copies.
8. Failure to comply with any section of this Acceptance shall be cause for termination and removal of
Acceptance.
9. This Notice of Acceptance consists of pages 1, 2 and this last page 3.
