in the following manner:
Remove existing drivowq and, relocate to better circulate trafficflow.
Remove 40 L.F. of low curb and replace with high curb.
Construct one (1) 18t Commercial concrete driveway on southern end of
property vdth 201 radius on south and 10' radius, on the; .north side.
Remove 32 L.F. of high curb and replace t:iith low curb.
Concrete to be 3000 lb. F.S.I.
All disturbed areas to be sodded.
Pavement symbol arrovrs to be installed for entrance and exit.
PRIOR TO EXCAVATING, CONTACT
This permit void if not completed within THE CLERK.OF THE CIRCUIT COURT

The driveway(s) will be constructed in accordance with resolution,; adopted by the Department of
Transportation and covered in its "Driveway Regulations Coveripg Private Entrances and Exits for State
Maintained Roads:'

or injuries that may occur by reason .of this construction of said faci4ty. ;
The applicant binds and obligates himself to conform to the above description and attached sketch
