BY SIGNING THIS STATEMENT, I ATTEST THAT. (Initial to the left of each statement)
I understand that state law requires construction to be done by a licensed contractor and have applied for
an owner -builder permit under an exemption from the law. The exemption specifies that 1, as the owner of
the property listed, may act as my own contractor with certain restrictions even though I do not have at\' license.
I understand that building permits are not required to be signed by a property owner unless he or she is
responsible for the construction and is not hiring a licensed contractor to assume responsibility.
I understand that, as an owner -builder, I am the responsible party of record on a permit. I understand that I
may protect myself from potential financial risk by hiring a licensed contractor and having the permit filed
in his or tier name instead of my own name. I also understand that a contractor is required by law to be
licensed in Florida and to list his or her license numbers on all permit and contracts.
I understand that I may build or improve a one -family or two -fancily residence or a firm outbuilding. I
may also build or improve a commercial building if the costs do not exceed $75,000. The building or
residence must be for my own use or occupancy. It may not be built or substantially improved for sale or
lease. If a building or residence that I have built or substantially improved myself is sold or ]eased withinQ_ in I year after the construction is complete, the law will presume that I built or substantially improved it
for sale or lease, which violates this exemption.
I understand that, as the owner -builder, I must provide direct, onsite supervision of the construction.
I understand that I may not hire an unlicensed individual person to act as my contractor or to supervise
persons working on my building or residence. It is my responsibility to ensure that the persons whom I
employ have the licenses required by law and by city ordinance.
I understand that it is a frequent practice of unlicensed persons to have the property owner obtain an
owner -builder permit that erroneously implies that the property owner is providing his or her own labor
and materials. 1, as an owner -builder, may be held liable and subjected to serious financial risk for any
injuries sustained by an unlicensed person or his or tier employees while working on my property. My
homeowner's insurance may not provide coverage for those injuries. I am willfully acting as an owner -
builder and am aware of the limits of my insurance coverage for injuries to workers on my property,
I understand that I may not delegate the responsibility for supervising work to a licensed contractor who is
not licensed to perform the work being done. Any person working on my building who Is not licensed
must work under my direct supervision and must be employed by me, which means that I must
comply with laws requiring the withholding of federal income tax and social security contributions
under the Federal Insurance Contributions Act (FICA) and must provide workers' compensation

SERVICES DEPARTMENT
The Development Review Team (DRT) held a Public Meeting at 9 AM on September 25, 2018 to consider a Minor
Conditional Use to establish an accessory residential structure at 717 Santa Barbara Drive.
The project was presented at the September 18, 2018 meeting, As- part of the Minor Conditional Use process, the
standards for consideration were reviewed.
A. The conditional use must be consistent with the goals, objectives and policies of the Comprehensive Plan and the
Land Development Regulations.
The proposed use is consistent with the Land Development Regulations based on the use being an allowable use
in the City of Sanford and the use being permitted in this zoning classification after a minor conditional use. The
comprehensive plan states that supportive community facilities and accessory land uses may be located within
areas designated LDR-SF.
B.Public facilities and services including, but not limited to, roadways, park facilities, schools, police and fire
protection, drainage systems, refuse and disposal systems, water and sewer must be adequate to serve the
proposed use.
The site currently has City water service. The project is within an established area with other concurrent services
available.
C.The proposed development must not adversely affect known archeological, historical or cultural resources,
There are no known archeological, historical or cultural resources affected by this use.
D. The design of the proposed development must minimize adverse effects, including, but not limited to, visual,
noise and traffic impacts, of the proposed use on the surrounding properties.
Other properties in the area ore permitted to request a minor conditional use to establish an accessory
residential structure.
E.The conditional use must be compatible with the nature and intensity of the development surrounding the
premises and with the community character of the immediate vicinity of the parcel proposed for development.
The project is within an existing residential neighborhood and the use is limited to residential uses. The nature
and intensity of the additional use should remain consistent with its surroundings.
F.The location and topography of the premises proposed for development must make the use an appropriate use
which will not adversely affect the public interest.
Based on existing neighborhood conditions the request does appear to be in harmony with the general purpose
and intent of the land development regulations and not injurious to the neighborhood, or otherwise detrimental
to the public welfare.

Design Procedure- Based on the dimensions of the structure, appropriate wind loads are
determined using Chapter 16 of the FBC 2007 for roof cladding wind loads. These
component wind loads for roof cladding are compared to the allowable pressures
listed above. The design professional shall select the appropriate erection details
to reference in his drawings for proper fastener attachment to his structure and
analyze the panel fasteners for pullout and pullover. Support framing must be in
compliance with FBC Chapter 22 for steel, Chapter 23 for wood and Chapter 16
for structural loading.

1, Prior to the return, the distributor must inform Sun-Tek of the material to be returned, the reason for such action, together Win the Sun-'Iek invoice and the customer
order numbers (it available).
2, If the authorization to return material Is given and an 'RGA' number is Issued, the terms are as fottows,
A. RGKs vAll be valid for sixty (60) days from date of issuance.
B, Material shall be returned by "prepaid' freight.
C Material shalt be property packaged to assure damage free shipment.
D. Material shalt be subject to inspection and if reconditioning is not required, credit will be issued at the net price of the original order, less 25% handling
charge for those items that are returned and reflect no fault or error of Sun-Tak. Material that has been approved for returns and reflects a fautt or error of
Sun -Tok will be issued credit at the net price of the original order,
E. And goods falling to pass inspection will be credited at a net price of original order, less 25% handling charge and less a reconditioning charge If the item
can be reconditioned. If the Item cannot be reconditioned, no credit will be issued and a 25% charge back willapply (applicable only toapproved returns

2. Other special promodons receiving prior written approval from Sun-Tek Vitt be reimbursed at a rate of
50% of the cost devoted to Sun-Tek.

Sun-Tek will share in the cost of a distributor Open House Program up to 50% of the cost that is devoted to
Sun-Tek. For exampte, if seven suppliers are involved in the Open House, Sun-Tek's share will be 50% of 1/7
of the Open House costs that are documented. Documentation of all cost must be included with request for
reimbursement
1. Copy of paid invoices indicating items and amounts which are to be applied against the co-op account.
2. Copy of booth space contracts showing space cost and substaining documents to determine how much
of the total space rented was used for the purpose of promoting Sun-Tek Skylights i.e., one fourth, one
third, one half, etc. A picture of the display is preferred with the request.
3. Original invoice(s) of expenses for other special promotions involving the exctusive promotion of Sun-Tek
Skyrights.
4. Tear sheets and copies of agency billings must be submitted for cooperative advertising of Sun-Tek
products,
