The front corner of the garage was damaged by a tree impact during the recent
hurricane. The location of this damage is identified in the site plan provided as
Attachment "A". The damage to the Meara residence located at 111 Brierwood Dr.,
Sanford, FL (32771) includes several of the manufactured roof trusses and a section of
the masonry wall. In accordance with the FLORIDA BUILDING CODE 5TH EDITION
2014) EXISTING BUILDINGS, the following classification is established for the repair of
this identified tree impact damage. In accordance with Section 502.1, the proposed
effort "is to be considered a repair". Further, since this structure has not sustained
substantial structural damage" in accordance with Section 404 Repairs, Section (B)
404.4 applies. This section provides that "For damage less than substantial structural
damage, repairs shall be allowed that restore the building to its pre -damage state,
based on material properties and design strengths applicable at the time of original
construction. New structural members and connections used for this repair shall comply
with the provisions of FLORIDA BUILDING CODE 5TH EDITION BUILDING for new
buildings of similar structure, purpose and location." The following specific steps detail
the requirements for the repair of this damaged structure.

ENGINEERING REPAIR DIRECTION
This Engineering Repair Direction provides for the repair of the damaged structures in
full compliance with the requirements of the FLORIDA BUILDING CODE 5T" EDITION
2014), including the FLORIDA BUILDING CODE, BUILDING, the FLORIDA BUILDING
CODE, RESIDENTIAL and the FLORIDA BUILDING CODE, EXISTING BUILDINGS.
The following specific directions are provided for the repair and restoration of the
damage identified in the exterior bearing wall structure and roof of the Meara residence
located at 111 Brierwood Dr., Sanford, FL (32771). The specific directions are provided
relative to the wall section shown in Attachment "B" and the partial floor plan provided
as Attachment "C
Shorinq — Prior to demolition and removal of the damaged structures, shoring
must be provided to ensure that the roof, roof structures and the precast garage
door header are adequately supported during the repair and restoration.
Demolition — Upon removal of the damaged portions of the roof structure and the
damaged section of masonry wall as identified in Attachment "C", the full extent
of the damaged roof and masonry wall structures will be identified. All the
structurally damaged wood material is to be removed and replaced. Based on the
preliminary evaluation, this will involve the removal and replacement of a number
of the manufactured wood roof trusses, the masonry wall between the garage
door opening and the front corner as well as a minimum of four feet of masonry
material along the front wall adjacent to this damaged corner.
Masonry Wall Rebuild — The damaged masonry material as identified in
Attachment "C" is to be removed from the lintel to the slab. The damaged
masonry concrete block material is to be removed such that only full 16" concrete
block masonry units are removed, providing for an interlocking of the rebuilt wall
structure into the existing undamaged masonry wall structure. Specifically, the
precast garage header is not to be cut allowing for the full 8" bearing on the
rebuilt section of masonry wall between the opening and the corner. As shown in
the Attachment, vertical cells are to be reinforced and grouted solid at the edge
of the garage door opening, the corner and at the front of the wall, where the new
concrete block units are knitted into the existing wall structure. The reinforcement

in these cells is to be #5 rebar, that is epoxied into the existing undamaged
foundation and tied into the new reinforced lintel structure. The replaced section
of lintel structure is to be grouted solid and reinforced with #5 rebar, which has
been secured a minimum of 5" into the existing lintel employing SIMPSON SET
Epoxy or equivalent. All of the rebar is to be tied in contact and provided with in
the case of the vertical reinforcement a minimum overlap of 8" and in the
horizontal overlaps in the lintel course a minimum overlap of 25". Upon
completion of the structural repair, the exterior of the wall is to be finished with
stucco with a minimum masonry thickness of 1/2" in compliance with the
FLORIDA BUILDING CODE 5T" EDITION (2014) RESIDENTIAL
Roof — The damaged roof trusses are to be removed and replaced. The new roof
trusses are to be secured employing SIMPSON MTSM16 twist straps with (7)
10d X 1'h" Nails and (4) %" X 2Y4" TITENs. The new roof sheeting is to be'h"
CDX Plywood or'/2" OSB material. The sections of sheeting are to be installed
such that as a minimum they span three trusses in the horizontal direction and
are a minimum of 24" in the vertical direction. The roof sheeting is to be secured
employing 8d Code Compliant Ring Shank Nails, 4" on center on the edges and
8" on center in the field.
Roof Covering — The new roof covering is to be installed over Code compliant
underlayment to match the existing roof shingles.
Soffit and Fascia — Soffit and fascia are to be repaired and finished to match the
existing materials and finishes.
GENERAL SPECIFICATION
The repair and restoration of this single family residential structure has been designed
in accordance the requirements of the FLORIDA BUILDING CODE 5TH EDITION,
BUILDING, CHAPTER 16, DESIGN CRITERIA and ASCE 7.

All connections have been checked to withstand all applicable loads.
All details and sections are intended to be typical and shall be construed to apply to
any similar situation elsewhere on the project, except where a different detail is
shown.
All concrete blocks shall be standard weight blocks conforming to ASTM C90, Grade
N1 with Fm = 1000 PSI, laid in running bond with Type S mortar.
Where shown, cores of concrete block masonry shall be filled with pea gravel
concrete with a minimum compressive strength of 3000 PSI at 28 days. Slump shall
be 8 to 10 inches.
In masonry headers extend the bottom steel reinforcement a minimum of 6 inches
into pilaster or poured column and wire tie the vertical reinforcement steel.
All reinforcing steel shall be new billet steel conforming to ASTM 615 Grade 40.
All reinforcing steel shall be provided with a minimum of 3 inches of concrete cover,
when installed below grade and 2 inches of concrete cover above grade.
All continuous vertical or horizontal rebar in footings, beams and other concrete shall
be spliced where necessary or desirable by wiring together in contact. The length of
5 bar laps shall be a minimum of 25 inches (40 bar diameters).

SCOPE:
This Evaluation Report is issued under Rule 61G20-3 and the applicable rules and regulations governing the use of
construction materials in the State of Florida. The documentation submitted has been reviewed by Robert Nieminen,
P.E. for use of the product under the Florida Building Code and Florida Building Code, Residential Volume. The
products described herein have been evaluated for compliance with the 51h Edition (2014) Florida Building Code
sections noted herein.
DESCRIPTION: Owens Corning Asphalt Roof Shingles
LABELING: Labeling shall be in accordance with the requirements the Accredited Quality Assurance Agency noted
herein.
CONTINUED COMPLIANCE: This Evaluation Report is valid until such time as the named product(s) changes, the
referenced Quality Assurance documentation changes, or provisions of the Code that relate to the product change.
Acceptance of this Evaluation Report by the named client constitutes agreement to notify Robert Nieminen, P.E. if the
product changes or the referenced Quality Assurance documentation changes. TrinityJERD requires a complete
review of this Evaluation Report relative to updated Code requirements with each Code Cycle.
ADVERTISEMENT: The Evaluation Report number preceded by the words "TrinityI ERD Evaluated" may be displayed in
advertising literature. If any portion of the Evaluation Report is displayed, then it shall be done in its entirety.
INSPECTION: Upon request, a copy of this entire Evaluation Report shall be provided to the user by the manufacturer or

4.1.2 Berkshire are fiberglass reinforced, 4-tab asphalt roof shingles.
4.1.3 Devonshire' are fiberglass reinforced, 5-tab asphalt roof shingles.
4.1.4 Duration, TruDefinitioe Duration, Duration Premium Cool, TruDefinitioe Duration Designer Color
Collection, TruDefinitioe Oakridge , Oakridge and WeatherGuard HP are fiberglass reinforced, laminated
asphalt roof shingles.
4.2 Berkshire Hip & Ridge Shingles, High Ridge, WeatherGuard* HP Hip & Ridge Shingles, ProEdge Hip & Ridge
Shingles and DuraRidge'" Hip & Ridge Shingles are fiberglass reinforced, hip and ridge asphalt roof shingles.
4.3 Starter Strip Shingle, Starter Strip Plus and Starter Shingle Roll are starter strips for asphalt roof shingles.

5.4 Wind Classification:
5.4.1 All Owens Corning shingles noted herein are Classified in accordance with FBC Tables 1507.2.7.1 and
R905.2.6.1 to ASTM D3161, Class F and/or ASTM D7158, Class H, indicating the shingles are acceptable for
use in all wind zones up to Vasd = 150 mph (Vun = 194 mph). Refer to Section 6 for installation requirements
to meet this wind rating.
5.4.2 All Owens Corning hip & ridge shingles, Starter Strip Shingle and Starter Strip Plus noted herein are Classified
in accordance with FBC Tables 1507.2.7.1 and R905.2.6.1 to ASTM D3161, Class F, indicating the shingles are
acceptable for use in all wind zones up to Vasd = 150 mph (V k = 194 mph). Refer to Section 6 for installation
requirements to meet this wind rating.
5.4.3 Classification by ASTM D7158 applies to exposure category B or C and a building height of 60 feet or less.
Calculations by a qualified design professional are required for conditions outside these limitations. Contact
the shingle manufacturer for data specific to each shingle.
5.4.4 Refer to Owens Corning published information on wind resistance and installation limitations.
5.5 All products in the roof assembly shall have quality assurance audit in accordance with the Florida Building
Code and F.A.C. Rule 61G20-3.
6. INSTALLATION:
6.1 Underlavment:
6.1.1 Underlayment shall be acceptable to Owens Corning and shall hold current Florida Statewide Product
Approval, or be Locally Approved per Rule 61G20-3, per FBC Sections 1507.2.3, 1507.2.4 or R905.2.3.
6.2 Asphalt Shingles:
6.2.1 Installation of asphalt shingles shall comply with the manufacturer's current published instructions, using
minimum four (4) nails per shingle in accordance with FBC Sections 1507.2 or R905.2, with the following
exceptions:
Berkshire shingles require minimum five (5) nails per shingle.
WeatherGuarde HP shingles require minimum six (6) nails per shingle.
Devonshire' shingles require minimum six (6) nails per shingle.
Starter Strip Shingle and Starter Strip Plus require minimum five (5) nails per strip.
Refer to Owens Corning published information on wind resistance and installation limitations.
6.2.2 Fasteners shall be in accordance with the manufacturer's published requirements, but not less than FBC
1507.2.6 or R905.2.5. Staples are not permitted.
6.2.4 Where the roof slope exceeds 21 units vertical in 12 units horizontal, special methods of fastening are
required. See figures below for details.

6.3.1 Installation of Berkshire Hip and Ridge Shingles, High Ridge, WeatherGuarde HP Hip and Ridge Shingles and
ProEdge Hip & Ridge Shingles shall comply with the manufacturer's current published instructions, using four
4) nails per shingle. Installation of DuraRidgeTm Hip & Ridge Shingles shall comply with the manufacturer's
current published instructions, using two (2) nails per shingle. Refer to Owens Corning published
information on wind resistance and installation limitations, including the use of hand -sealing for wind
warranties.
6.3.2 Fasteners shall be in accordance with the manufacturer's published requirements, but not less than FBC
