Application is hereby made to obtain a permit to do the work and installations as indicated. I certify that no work or installation has commenced prior
to the issuance of a permit and that all work will be performed to meet standards of all laws regulating construction in this jurisdiction. I understand
that a separate permit must be secured for electrical work, plumbing, signs, wells, pools, furnaces, boilers, heaters, tanks, and air conditioners,
etc.

NOTICE: In addition to the requirements of this permit, there may be additional restrictions applicable to this property that may be found in the public
records of this county, and there may be additional permits required from other governmental entities such as water management districts, state
agencies, or federal agencies.
Acceptance of permit is verification that I will notify the owner ofthe property of the requirements ofFlorida Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee at the time of permit submittal. A copy ofthe executed contract is required in order to
calculate a plan review charge and will be considered the estimated construction value ofthe job at the time ofsubmittal. The actual construction value
will be figured based on the current ICC Valuation Table in effect at the time the permit is issued, in accordance with local ordinance. Should calculated
charges figured off the executed contract exceed the actual construction value, credit will be applied to your permit fees when the permit is issued.

Add $189.00 to contract price for addition of `Gold Club' one-year maintenance plan. Includes two
maintenance visits.
The first year maintenance fee is included in the total investment price of your new comfort system.
SYSTEM WARRANTIES
Two (2) year labor warranty; ten (10) year parts warranty and ten (10) year compressor warranty with the
manufacturer.
DUCT AND AIR DISTRIBUTION SYSTEM
The existing duct system shall be reused. The new air handling unit shall be connected to the existing duct
system with R-6 antimicrobial fiberglass duct. All new connections shall be sealed airtight as per Florida
Mechanical Code requirements.
ELECTRICAL SAFETY AND SYSTEM CONTROLS
Existing electrical service and wiring to equipment shall be reused.
Circuit breakers shall be replaced as necessary.
One (1) safety overflow drain line float switch for system shutdown as per mechanical code requirements.

INSTALLATION NOTES
1. Existing mechanical equipment shall be removed and properly disposed of by 4-Seasons.
2. Proposal Includes all necessary copper refrigerant piping, refrigerant, suction line insulation and condensate
drain line with clean -out access.
3. New concrete hurricane pad shall be installed for outdoor unit support. New unit shall be hurricane fastened
to pad.

tolerances.
6. Cleanup will be done at the conclusion of the day's work and all debris will be removed from the premises.
