The City of Santord requires payment of a plan review lee at the time 01'permit submittal. A copy of the executed contract is required
in order to calculate a plan review charge and will be considered the estimated construction value of the job at the time of submittal.
The actual construction Valtte will be fi12,ured based on the current ICC Valuation Table in effect at the time the permit is issued, in
accordance with local ordinance. Should calculated charges figured off the exrcttted contract exceed the actual construction value,
credit will be applied to your permit fees when the permit is issued.

1. Iurchaser hereby accepts the equipment and service described above and agrees to pay Facemyer Air Conditioning and Pleating the price
sltotvn above.
2. All equipment and material are guaranteed by1Facctayer Air Conditioning and Heating it)beas specified. All work will be connpleted in it
workmanlike manner according.to normally accepted practices.
3. Materials and work ill addition'to that (Iescril)cd herein will be lurnished only on Purchaser's authorization and will be paid by Purchaser
as an extra charge.
4. Upon failure to pay any suns due hereunder. Purchaser agrees to pity Facemyer Air Conditioning and Healing interest at the rate of one
and one halfpercem (1 Y%) per month (annual rate ol' 18%) on all outstanding balances.
5. F=acemyer Air Conditioning and Pleating shall not be liable for anydefaultcaused by events beyond its control. including but not limited to,
fire, hood, strikes; accidents, or delays aficctiingthis work or other operationsin which it is involved, directly or indirectly. 6.
Purchaser shall permit Faceniver Air Conditioning and Healing reasonable access to the property on which equipment is to be installed. Title
to all provided equipment remains with Facemyer Air Conditioning and Heating until all ,amounts title thereon are paid in full. Xvhciller such
equipment is alfixed tothe realty or not, and shall remain personal property and be deemed sever -able without injure to the freehold. On any payment
default by Purchaser, or it in 1'-accmyer Air Conditioning and Healing's judgment, reasonably exercised. its equity appears to be imperiled, then,
F;acenyerAir Conditioning and Healing may without further notice enter the pretniscs and removeor resell the equipment, and Purchaser shall
be liable for qmy deficiency or loss sustained by Facemyer Air Conditioning and heating in connection therewith. 7. Once equipment
is, connected ill'Purchascr's property. Purchaser assunus all risk of loss or damage to such equiptiacnl and shall insure same fully to
protect all interests of Facemyer Aar Conditioning urnd I lcating, cost of insurance to he paid by Purchaser. Facemyer Air Conditioning and Heating
carries liability insurance and \Vorker's Compensation Insurance. 3. Facemyer Air
Conditioning and Heating provides at one-year limited labor warranty. L'AlUipmcnt or systemfailure due to lack of proper maintenance service or
abuse is expressly excluded. Normal maintenance cheek -ups and filter replacements are the responsibility of Purchaser. All (ether
warranties, expressed or implied, ale the responsibility ol'the nanul'adUrcr ol'the equipnnett. parts. or materials used in connection with the
services. 9. There are
no warranties: expressed or implied, I' Or existing equipment, ductwork, or other materials not installed by Facemyer Air Conditioning and Heating
10. All warranty
work will be performed during Facemyer Air Conditioning and Hcating's normal working hours, 8:00A\4 to 3:0011i'v1, Monday through Friday.
11. Purchaser is
responsible fair all costs and reasonable attorney fees incurred by Faccnnyer Air Conditioning and Healing in connection with any action or
proceeding (including arbitration ,and apilcals) arising out of this agreenncnt_ including collection of any outstanding amounts file, whether ornot
suit is filed. 12. Except ,as
provided herein Facemyer Air Conditioning and I leafing makes no other representations or warranties, either express or implied, including, but
not limited to, any implied vvarrawies of nnerehanlability or fitness liar it particular purpose Facemyer Air Conditioning and heating
expressly disc faints all other warranties. Facemyer Air Conditioning and Beating's maximum liability hereunder shall consist of
refunding all rnoneys paid to it by Purchaser hereunder subject to removal and return to 1'acennyer Air Conditioning and Heating ofall equjpnncnt
provided hereunder. Under no circunnstanccs will Facemyer Air Conditioning and Heating be liable to Purchaser or any other person
for any damages, including, without limitation, any indirect. incidental, spccial, or conscquential dama cs. expenses, cost. profits, lost saving'
or earnings, lost or corrupted data_ or other liability arising out of'or related to this agreement. or the services or equipment provided hereunder.
13. This agreement
shall be governed and construed solely according to the internal laws of the Slate of Florida. without referencc to any conflicts of'laws.
14. This agreement
is the complete and exclusive statement ofthe agreement beuvean Purchaser ;and f <accnayer Air Conditioning and locating and it supersedes

injured residential consumers from the Florida Homeowners Recovery Fund for both Division I and Division 11 scopes ol'work. Financ
Effective July I, 2016, all licensed contractors, both Division I and Division 11, must ensure that the Florida Homeowner Construction
Recovery Fund Notice is contained in their residential contracts.
According to section 489.1425(I ): Each agreement or contract for repair, restoration, improvement, or construction to residential real
property must contain a written statement explaining the consumer's rights under the recovery fund, except where the value of all labor and
materials does not exceed $2,500. The written statement must be substantially in the following form:
