Application is hereby made to obtain a permit to do the work and installations as indicated. I certify that no work or installation has
commenced prior to the issuance of a permit and that all work will be performed to meet standards of all laws regulating construction
in this jurisdiction. I understand that a separate permit must be secured for electrical work, plumbing, signs, wells, pools,
furnaces, boilers, heaters, tanks, and air conditioners, etc.

NOTICE: In addition to the requirements of this permit, there may be additional restrictions applicable to this property that may be
found in the public records of this county, and there may be additional permits required from other governmental entities such as water
management districts, state agencies, or federal agencies.
Acceptance of permit is verification that I will notify the owner of the property of the requirements of Florida Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee at the time of permit submittal. A copy of the executed contract is required
in order to calculate a plan review charge and will be considered the estimated construction value of the job at the time of submittal.
The actual construction value will be figured based on the current ICC Valuation Table in effect at the time the permit is issued, in
accordance with local ordinance. Should calculated charges figured off the executed contract exceed the actual construction value,
credit will be applied to your permit fees when the permit is issued.

HVAC CONTRACT
THIS HVAC CONTRACT (hereinafter referred to as the "Contract") is made on by and between ALL STAR
HEATING AND COOLING OF CENTRAL FLORIDA, LLC of 3705 Seminole Drive, Orlando, FL 32812
hereinafter referred to as the "Contractor"), and LAHAM, FEDERICO, of 9131 DOLLANGER CT
ORLANDO, FL 32819 (Owner/Owner agent/Managing member, hereinafter referred to as the "Client")

WHEREAS, Contractor is engaged in the business of providing HVAC services to individuals and
companies; and
WHEREAS, Client desires to hire the Contractor under the terms and conditions specified herein;
NOW, THEREFORE, in consideration of the mutual covenants contained in this Contract, and other good
and valuable consideration, the parties agree as follows:
1. DESCRIPTION OF WORK
Contractor will furnish all labor and materials for HVAC work at the Client's job site at 2615
Mohawk Ave, Sanford, FL 32773, Rear unit, as requested by Client. Contractor shall furnish the
following HVAC services for Client:
Open and complete mechanical permit. Supply and install 1x 2 ton Goodman 14 seer AC system,
with aux heat per breaker size, run new copper lineset, clean drain line/test operation, install new
AHU float switch, install metal frame AHU stand, run new low voltage lines as needed, install new
ac pad/anchor kit, supply and install new digital programmable thermostat. Start system, balance
charge, test operation, register equipment for 10 year parts, 1 year labor warranty. Supply and
install new duct work, boots, and grills per mechanical plans, strap and seal all connections.
Contractor is not responsible for any drywall, concrete or roof penetrations, cutting, patching,
painting, flashing, or sealing of any kind for any reason. Contractor is not responsible for supplying
or fabricating any supplemental steel for any reason.
TERM
The services specified hereunder shall begin at Client's request and/or Contractor's earliest
availability and shall take approximately 1 DAY per phase of job to complete, or unless otherwise
described in this contract or in text or email correspondence
3. PAYMENT
Client will pay the Contractor as follows: In full, upon job phase completion, or as otherwise
described herein or in email correspondence. All invoices are due and payable upon receipt. Any
balance or payments that are not received by no later than Thirty (30) days past original due date
will be subject to interest at a rate of 3.5% of overdue amount, or $75.00, whichever is greater,
per month, thus putting Client in default. If Client defaults, Client will be in breach of contract and
the sum of all moneys paid to Contractor, as well as all materials, equipment, plans, permits, and
any other job documents will remain in possession of Contractor with no obligation to repay
Client, complete project or perform any implied warranty repairs of any kind until past and current
payments, including interest, are paid in full. No inspections will be scheduled until payment has
been made as agreed. If client cancels job after deposit payment has been made at no fault of the
Contractor, there will be a non-refundable restocking fee charged to Client in the amount of 20%
total project cost, in addition to any additional labor and/or material costs incurred by contractor
as a result of removing equipment and supplies from job site. Payment plans require extended
labor warranty purchase and a working credit card to be held on file to be ran on or about the first
of each month or as described below.

WARRANTY OF FITNESS OF EQUIPMENT AND MATERIALS
Contractor represents and warrants to Client that all equipment and materials used in the work,
and made a part of the structures on the work, or placed permanently in connection with the
same, will be new unless otherwise specified, of good quality, free of defects, and in conformity
with the Contract. It is understood between the parties that all equipment and materials not so in
conformity are defective. Contractor provides a minimum warranty for a period of 1 year parts
and labor, 10 year parts for new residential system installations, and 30 days on repairs or
other HVAC services provided to Client. In some cases, drywall will be cut for access to ductwork.
Contractor's warranties are limited to the cost of labor and materials only, and exclude ordinary
wear and tear or abuse by others. Contractor is not responsible for any additional repairs such as
drywall damage or cuts, or paint related to installation of equipment, registers, or duct work,
thermostat, or any other work typically associated with HVAC as described herein Contractor not
responsible for damage resulting from roof leaks of any kind for any reason. All permit fees paid by
Client in addition to job cost. Any failure on existing system(s) due to normal procedures as
described or discussed in this scope of work, including but not limited to compressor failure,
refrigerant leaks, motor failure, electrical components, wiring, etc is not covered by warranty of
any kind, nor is contractor responsible for cost of repairs outside of original scope if needed.
Warranty does not include: clogged drain lines, dirty filters, dirty coils, acts of nature, and all non -
manufacturer parts, including refrigerant.
TITLE TO MATERIAL
Title to the material to be furnished by Contractor shall be and remain in possession and
ownership of Contractor until all sums of money called for are fully paid. In case of default in
payment of any of the installments provided for in this contract, Contractor is authorized to obtain
legal aid to obtain access to jobsite and remove material and equipment at any time after the
default, with no responsibility to pay for repairs incurred by entering jobsite or removal of
equipment and supplies.

6. CHANGE ORDERS
All change orders must be in writing and signed by both parties. Client agrees that changes
resulting in the furnishing of additional labor or materials will be paid for prior to the
commencement of the extra work.
7. SITE CONDITIONS
Client acknowledges that this Contract is based upon the Contractor's observation of conditions.
Conditions which could not be known by a reasonable inspection, such termite damage, hidden
water damage, hidden code violations, or other concealed conditions, may require extra labor or
materials, which are not part of this Contract. If such hidden conditions are discovered, Contractor
will notify Client and will attempt to reach an agreement for a change order to this Contract that
addresses those problems.
8. SEVERABILITY
If any provision of this Contract shall be held to be illegal, invalid, or unenforceable under present
or future laws, such provisions shall be fully severable, this Contract shall be construed and
enforced as if such illegal, invalid or unenforceable provision had never comprised a part of this
Contract; and, the remaining provisions of this Agreement shall remain in full force and effect.
9. NO WAIVER
The failure of either party to this Contract to insist upon the performance of any of the terms and
conditions of this Contract, or the waiver of any breach of any of the terms and conditions of this
Contract, shall not be construed as subsequently waiving any such terms and conditions, but the
same shall continue and remain in full force and effect as if no such forbearance or waiver had
occurred.
10. GOVERNING LAW
This Contract shall be governed by, construed, and enforced in accordance with the laws of the
State of FL.
11. NOTICES
Unless provided herein to the contrary, any notice provided for or concerning this Contract shall be
in writing and shall be deemed sufficiently given when sent by certified or registered mail if sent to
the respective address of each party as set forth at the beginning of this Contract.
12. BINDING ARBITRATION
Notwithstanding the foregoing, and anything herein to the contrary, any dispute under this
Contract shall be required to be resolved by binding arbitration of the parties hereto. If the parties
cannot agree on an arbitrator, each party shall select one arbitrator and both arbitrators shall then
select a third. The third arbitrator so selected shall arbitrate said dispute. The arbitration shall be
governed by the rules of the American Arbitration Association then in force and effect.
13. ENTIRE AGREEMENT
This Contract shall constitute the entire agreement between the parties and any prior
understanding or representation of any kind preceding the date of this Contract shall not be
binding upon either party except to the extent incorporated in this Contract.
14. MODIFICATION OF CONTRACT

Any modification of this Contract or additional obligation: assumed by either party in connection
with this: Contract shall be binding only if placed in writing and signed by each party Or an
authorized representative of each party.
is. ASSIGNMENT OF RIGHTS
The rights of each party under this Contract are personal to that party and may not be assigned or
transferred to any other person, firm, corporation, or other entity without the prior, express, and
written consent of the. other party..
16. COUNTERPARTS
This Contract may be executed in any number of counterparts, each of which shall be deemed to
be an original, but all ofwhich together shall constitute but one and the same instrument.
17, COMPLIANCE WITH LAWS
In performing. under this Contract, all, applicable governmental laws, regulations, orders, and
other rules of duly-constituted:authority will be followed and complied with in all respects by
both parties.
