is to certify that on October 29, 1999 this firm performed a field survey on the
formboards for Building 13. The constructed forms location appears to be in substantial
compliance to the "Civil Design" plans, as received by this firm on May 10,
1999 from Burkett Engineering, for the Plantation Lakes Apartments. The top
of forms elevation' varies from 67.24 to 67.26, and the proposed finished floor elevation
is 67.25. The above finished floor elevations meet or exceed the requirements set

requested, a Universal Engineering Sciences (UES) threshold representative was at the above referenced
project to perform a visual inspection of reinforcing steel for Building #13 shear walls and
elevated slab, 21 floor wall lines 9.1 through 1.1. All steel placed meets project plans and specifications.
The
UES representative monitored concrete placement in the same locations. During the concrete
placement, the UES representative checked batch time of 3 truck mixers that had been idle
on the job site for one hour. All three trucks allotted time of 90 minutes from batch time had expired.
Charles Hudgens was notified and agreement was made that trucks would be sent away and
not placed. Charles called the concrete supplier and was told by truck dispatcher that trucks were
not expired and that the 90 minutes starts when truck arrives at job site, not from batch time. Charles
took advice from dispatcher and made a set of test cylinders from the first truck to return. Fifty
gallons of water was added to the mix to achieve 3" minimum slump and the temperature was
recorded at 96 degrees. Placement began at wall line 5 in wall and in slab. Approximately 3
cubic yards of concrete was placed when the concrete started to harden in slab and in placement
bucket. A cold joint formed between wall line 5 and 4 and the rest of the load was not placed.

IMPORTAN?'. This detector must be lested and maintained
regularly following NFPA 72 requirements. This detector
should be cleaned at least once a year.
General Doscrlptlon
Model 2100(A) is a 2-wire photoelectronic smoke detector
that uses a state-of-the-art optical sensing chamber. This
detector Is designed to provide open area protection and to
be used with compatible UL-listed panels only. Model
2100T(A) features a restorable. built-in, fixed- tempera! tire
135"F) thermal detector.
Installation of these detectors is simplified by Ilie use of a
mounting bracket and a plug-in screw terminal block Ihat
can be prewited to the system, allowing the det Oct, to be
easily installed or removed for cleaninj. The detector's sen•
silkily can be tested in place using the MOg400R Tesl
Module. An LED on the detector provides a local visual in-
dication of the detector's status. If power Is applied to the

OUTDOOR OPTION
Fire alarm hom/strobe operates from 24 Vdc and can be selected for
temporal pattern or steady tone output. The unique microprocessor
based hom is completely self -synchronized when set to temporal
signal and does not require external synch -control modules. A
moveable jumper provides a choice for high (97 dBA) or low (91
dBA) output.
The hom/strobe is listed for indoor and outdoor installations. A =12
AWG terminal block is provided for connection to signal circuit and
all models mount to standard North American 2-gang elect
minimum 2-3/4** (69 mm) deep. The plastic front plate has
attractive textured finish. The synchronized strobe is suppliM,
FIRE** (wall orientation) as the standard lens markma. Color -
matched surface boxes, homs. and other audible!visible srgnals are
also available.
