OBSERVATIONS:
On March 23, 1996, a representative of Universal Engineering Sciences (UES) was present at the above project
site as requested to perform reinspections on bolt torques which were previously not in accordance with
specifications. All bolted connections with 3/4 inch Type A-325 bolts were found to be torqued to 28 kips as
specified in AISC Table No. 3. Other locations that had been noted with regard to the length of bolts were replaced•
with the proper bolts as specified in the project plans. Final inspection verifies structural steel bolted connections
to be as per project plans and specifications.

Under current guidelines, property that is being used exclusively. for normal
operations of the airport is tax exempt.
The new terminal, concourse, departure building and welcome center fall
within these guidelines and would therefore be exempt from taxation.
This office requires that the Airport Authority provide updated information
regarding exemption status and use of property on an annual basis. This
assists in ensuring proper compliance with current laws and regulations.

At your request, the substitution of fiber mesh as specified below is approved
for use on this project. If you have any further questions, please do not
hesitate to call me.
Synthetic fiber reinforcement shall be 100% virgin polypropylene in
fibrillated form. Fiber length shall be 1-1/2 " long added at the rate of 1.5
lbs. Per cubic yard of concrete. Fibers shall be added directly to the
concrete mixing -"system and mixed per manufacturer's recommendations. Fiber
shall comply with A.S.T.M. C-1116, 4.1.3 and applicable building codes.
Material shall be equal to Forta UP or Fibermesh.

within the masonry walls of the above building. Due to the size and proximity
of the clearance necessary of (3/4 ") from the surrounding masonry, a larger
rod than called for on the plans is acceptable. The use of #6 reinforcing
bars in lieu of #5 rods is acceptable.

On October 6, 1995, a representative of Universal Engineering Sciences, Inc. (UES) was present at the above
project site as requested to perform the referenced work. A UES representative was present to observe proof rolling
of the southwest parking lot. All deleterious material had been removed (i.e., trees, stumps, roots, grass) from
construction area.
Observed rolling operation of roller with vibratory in operation more than 100 feet from the existing building.
The operation went from east to west and north to south with a minimum of 30 percent of roller overlapping the
preceding pass. During observation of the rolling operation, the technician did not see any yielding of material.
The proof rolling was done according to job plans and specifications.
