Application is hereby made to obtain a permit to do the work and installations as indicated. I certify that no
work or installation has commenced prior to the issuance of a permit and that all work will be performed to
meet standards of all laws regulating construction in this jurisdiction. I understand that a separate permit
must be secured for electrical work, plumbing, signs, wells, pools, furnaces, boilers, heaters, tanks, and
air conditioners, etc. .
OWNER'S AFFIDAVIT: I certify that all of the foregoing information is accurate and that all work will
be done in compliance with all applicable laws regulating construction and zoning.
WARNING TO OWNER: YOUR FAILURE TO. RECORD A NOTICE OF COMMENCEMENT MAY
RESULT IN YOUR PAYING TWICE FOR IMPROVEMENTS TO YOUR PROPERTY. A NOTICE
OF COMMENCEMENT MUST BE RECORDED AND POSTED ON THE JOB SITE BEFORE THE
FIRST INSPECTION. IF YOU INTEND TO OBTAIN FINANCING, CONSULT WITH YOUR
LENDER OR AN ATTORNEY BEFORE RECORDING YOUR NOTICE OF COMMENCEMENT.
NOTICE: In addition to the requirements of this permit, there may be additional restrictions applicable to this
property that may be found in the public records of this county, and there may be additional permits required
from other governmental entities such as water management districts, state agencies, or federal agencies.
Acceptance of permit is verification that I will notify the owner of the property of the requirements of Florida
Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee. A copy of the executed contract is required in order
to calculate a plan review charge. If the executed contract is not submitted, we reserve the right to calculate the
plan review fee based on past permit activity levels. Should calculated charges exceed the documented
construction value when the executed contract is submitted, credit will be applied to your permit fees when the
permit is released.

ORDER OF DEMOLITION
2850 SANFORD AVENUE
On June 14, 2010, the City Commission of the City of Sanford issues, under the
authority of Section 18-31 of the City Code and other controlling law, this Order of
Demolition relating to and touching and concerning and running with and burdening the
following described property (hereinafter the "Property"):
Leg Lots 43 + 44 Less Right of Way for Road, 2nd Addition to Park
View, Plat Book 4, Page 5 of the Public Records of Seminole County,
Florida.
FINDINGS OF FACT
1). The City Commission hereby finds the following relative the Property
described above:

2). The City Commission hereby ratifies and affirms the Findings of Fact
made in the Order of Condemnation relating to the Property.
3). The owner of the Property failed to abate the public nuisances found to
exist on the Property, place the Property in a state of sound repair or demolish and
remove the building or structure located on the Property.
4). The property owner did not appear at the hearing.

CONCLUSIONS OF LAW:
1). All notices required by controlling law were provided to the owner of the
Property.
2). The City Commission hereby ratified and affirms the Conclusions of Law
made in the Order of Condemnation relation to the Property.
3). This Order of Demolition touches and concerns the Property.
ORDER:
NOW, THEREFORE, based upon the premises and the evidence submitted for
consideration and being fully advised with regard to this muter, it is ordered by the City
Commission that:
1). The City of Sanford is hereby authorized to abate the public nuisance and
demolish the building or structure located on the Property.
2). This Order of Demolition shall be recorded by the City Manager, or
designee, in the Official Records of Seminole County, Florida (Land Records) and shall
further the effect on the Order of Condemnation recorded as to the Property.
3). This is final action taken as to this matter by the City of Sanford.
Done and Ordered on the date below written.
As approved and authorized for execution by the City Commission of the City of
Sanford on this 14th day of June, 2010.
