homeowners from qualifying as contractors and to express any applicable restrictions and responsibilities.
OWNERS MUST PERSONALLY APPEAR AT THE BUILDING DIVISION TO SIGN THIS DOCUMENT
BY SIGNING THIS STATEMENT, I ATTEST THAT: (Initial to the left of each statement)
I understand that state law requires construction to be done by a licensed contractor and have applied for
an owner -builder permit under an exemption from the law. The exemption specifies that I, as the owner of
the property listed, may act as my own contractor with certain restrictions even though I do not have a
license.
I understand that building permits are not required to be signed by a property owner unless he or she is
responsible for the construction and is not hiring a licensed contractor to assume responsibility.
I understand: that, as an owner -builder, I am the responsible party of record on a permit. I understand that I
may protect myself from potential financial risk by hiring a licensed contractor and having the permit.filed
in his or her name instead of my own name. I also understand that a contractor is required by law to be
licensed in Florida and to list his or her license numbers on all permit and contracts.
I understand that I may build or improve a one -family or two-family residence or a farm outbuilding. I
may also build or improve a commercial building if the costs do not exceed $75,000. The building or
residence must be for my own use or occupancy. It may not be built or substantially improved for sale or
lease.. If a building or residence that I have built or substantially improved myself is sold or leased within
in 1 year after the construction is complete, the law will presume that I- built or substantially improved. it
for sale or lease, which violates this exemption.
I understand: -that, as .the owner. -builder, I must provide direct, onsite supervision of the construction.-
I understand that I may not hire an unlicensed individual person to act as my contractor or to supervise
persons working on my building or residence. It is my responsibility to ensure that the persons whom I
employ have the licenses required by law and by city ordinance.
I understand that it is a frequent practice of unlicensed persons to have the property owner obtain an
owner -builder permit that erroneously implies that the propertyowner is,providing his "or her own labor financial
for any andmaterials. I, as an owner -builder, may be held liable and subjfected to,'serious risk injuries
sustained by an unlicensed person or his or her, employees while working on my properly. My homeowner'
s insurance may not provide coverage for those, injuries. I am, willfully acting as an owner - builder
and am aware of the limits of my insurance coverage for injuries to'workers on my property. I
understand that I may not delegate the responsibility for supervising,.work:to a licensed contractor who is not
licensed to perform the work being done. Any person working"onrmy building who Is not licensed that

comply with laws requiring the withholding of federal income tax and social security contributions under
the Federal Insurance Contributions Act (FICA) and must provide workers' compensation for

2512.1.2 Weep screeds.
A minimum 0.019 nch (0.48 mm) (No. 26 galvanized sheet gage),
corrosion -resistant weep screed with a minimum vertical attachment flange of 3%
inches (89 mm) shall be provided at or below the foundation plate line on exterior
stud wags in accordance with ASTM C 926. The weep screed shall be placed a
minimum of 4 inches (102 mm) above the earth or 2 inches (51 mm) above paved
areas and be of a type that will allow trapped water to drain to Die exterior of the
building. The weather -resistant barrier shell lop the attachment flange. The exterior
lath shag cover and terminate on the attachment flange of the weep screed.

NOTICE OF ACCEPTANCE: STANDARD CONDITIONS
1. SCOPE
1.1 This renews the Notice of Acceptance No. 94-1031.03 which was issued on September 21, 1995. It
approves an aluminum single hung window, as described in Section 2 of this Notice of Acceptance,
designed to comply with the South Florida Building Code, 1994 Edition for Miami -Dade County,
for the locations where the pressure requirements, as determined by SFBC Chapter 23, do not
exceed the Design Pressure Rating values indicated in the approved drawings.
2. PRODUCT DESCRIPTION
2.1 The Series 4000 Aluminum Single Hung Window and its components shall be constructed in strict
compliance with the following document: Drawing No. 137 Sheet I of 4, titled "Single Hung
Elevations," Sheet 2 of 4 titled "Single Hung Layout," Sheet 3 of 4 titled "Comparative Analysis,"
and Sheet 4 of 4 titled "Typ. Single Hung Anchorage," prepared by Vinyl Tech/Progressive Glass
Technology, dated March 17, 1998, except for Sheet 3 of 4 which is dated June 10, 1998, bearing
the Miami -Dade County Product Control Approval stamp with the Notice of Acceptance number
and approval date by the Miami -Dade County Product Control Division. These documents shall
hereinafter be referred to as the approved drawings.
3. LIMITATIONS
3.1 This approval applies to single unit applications only, as shown in approved drawings.
4. INSTALLATION
4.1 The aluminum single hung window and its components. shalt be installed in strict compliance with
the approved drawings.
4.2 The installation of this product will require a hurricane protection system.
5. LABELING
5.1 Each unit shall bear a permanent label with the manufacturer's name or logo, city. state and
following statement: "Miami -Dade County Product Control Approved".
6. BUILDING PERMIT REQUIREMENTS
6.1 Application for building permit shall be accompanied by copies of the following:

6.1.2 Duplicate copies of the approved drawings, as identified in Section 2 .of this Notice of
Acceptance, clearly marked to show the components selected for the proposed installation.

NOTICE ON ACCEPTANCE: STANDARD CONDITIONS
I Renewal of this Acceptance (approval) shall be considered after a renewal application has been filed and the
original submitted documents, including test -supporting data, engineering documents, are no older than eight

2. Any and all approved products shall be permanently labeled with the manufacturer's name, city, state, and the
following statement: "Miami -Dade County Product Control Approval", or as specifically stated in the
specific conditions of this Acceptance.
3. Renewals of Acceptance will not be considered if:
a. There has been a change in the South Florida Building Code affecting the evaluation of this product
and the product is not in compliance with the code changes.
b. The product is no longer the same product (identical) as the one originally approved.
C. If the Acceptance holder has not complied with all the requirements of this acceptance, including the
correct installation of the product.
d. The engineer who originally prepared, signed and sealed the required documentation initially
submitted, is no longer practicing the engineering profession.
4. Any revision or change in the materials, use, and/or manufacture of the product or process shall automatically
be cause for termination of this Acceptance, unless prior written approval has been requested (through the
filing of a revision application with appropriate fee) and granted by this office.
5. Any of the following shall also be grounds for removal of this Acceptance:
a. Unsatisfactory performance of this product or process.
b. Misuse of this Acceptance as an endorsement of any product, for sales, advertising or any other
purposes.
6. The Notice of Acceptance number preceded by the words Miami -Dade County, Florida, and followed by the
expiration date may -be displayed in advertising literature. If any portion of the Notice of Acceptance is
displayed, then it shall be done in its entirety.
7. A copy of this Acceptance as well as approved drawings and other documents, where it applies, shall be
provided to the user by the manufacturer or its distributors and shall be available for inspection,at the. job site
at all time. The engineer needs not reseal the copies.

Failure to comply with any section of this Acceptance shall be cause for termination and removal of
Acceptance.
