meet standards of all laws regulating construction in this jurisdiction. I understand that a separate pertnit
must be secured for electrical work, plumbing, signs, wells, pools, furnaces, boilers, heaters, tanks, and
air conditioners, etc.

Acceptance of permit is verification that I will notify the owner of the property of the requirements of Florida
Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee. A copy of the executed contract is required in order
to calculate a plan review charge. If the executed contract is not submitted, we reserve the right to calculate the
plan review fee based on past permit activity levels. Should calculated charges exceed the documented
construction value when the executed contract is submitted, credit will be applied to your permit fees when the
permit is released.

homeowners from qualifying as contractors and to express any applicable restrictions and responsibilities. /
OWNERS MUST PERSONALLY APPEAR AT THE BUILDING DIVISION TO SIGN THIS DOCUMENT
BY SIGNING THIS STATEMENT, I ATTEST THAT: (Initial to the left of each statement)
I understand that state law requires construction to be done by a licensed contractor and have applied for
an owner -builder permit under an exemption from the law. The exemption specifies that I, as the owner of
Cthe property listed, may act as my own contractor with certain restrictions even though I do not have a
license. _
I understand that building permits are not required to be signed by a property owner unless he or she is
responsible for the construction and is not hiring a licensed contractor to assume responsibility.
I understand that, as an owner -builder, I am the responsible party of record on a permit. I understand that I
may protect myself from potential financial risk by hiring a licensed contractor and having the permit filed
in his or her name instead of my own name. I also understand that a contractor is required by law to be
licensed in Florida and to list his or her license numbers on all permit and contracts.
I understand that I may build or improve a one -family or two-family residence or a farm outbuilding. I
may also build or improve a commercial building if the costs do not exceed $75,000. The building or
residence must be for my own use or occupancy. It may not be built or substantially improved for sale or
lease. If a building or residence that I have built or substantially improved myself is sold or leased within
in 1 year after the construction is complete, the law will presume that I built or substantially improved it
for sale or lease, which violates this exemption.
I understand that, as the owner -builder, I must provide direct, onsite supervision of the construction.
I understand that I may not hire an unlicensed individual person to act as my contractor or to supervise
persons working on my building or residence. It is my responsibility to ensure that the persons whom I
employ have the licenses required by law and by city ordinance.
I understand that it is a frequent practice of unlicensed persons to have the property owner obtain an
owner -builder permit that erroneously implies that the property owner is providing his or her own labor
and materials. I, as an owner -builder, may be held liable and subjected to serious financial risk for any

homeowner's insurance may not provide coverage for those injuries. I am willfully acting as an owner -
builder and am aware of the limits of my insurance coverage for injuries to workers on my property.
I understand that I may not delegate the responsibility for supervising work to a licensed contractor who is
not licensed to perform the work being done. Any person working on my building who Is pot licensed
must work under my direct supervision and must be employed by me, which means that I must
i comply with laws requiring the withholding of federal income tax and social security contributions
under the Federal Insurance Contributions Act (FICA) and must provide workers' compensation
for the employee. I understand that my failure to follow these laws may subject me to serious financial
risk.

Revenue Service, the United States Small Business Administration, the Florida Department of Financial
Services, and the Florida Department of Revenue. I also understand that I may contact the Florida
Construction Industry Licensing Board at 1-850-487-1395 or at www.myflorida.com/dbpr/pro/cilb/ for
more information about licensed contractors.
I am aware of, and consent to, an owner -builder building permit applied for in my name and 'understand
that I am the party legally and financially responsible for the proposed construction activity at the address
listed below.
I agree to notify the building department immediately of any additions, deletions; or changes to any of the
information that I have provided on this disclosure or in the permit application package.
Licensed contractors are regulated by laws designed to protect the public. If you contract with a person
who does not have a license, the Construction Industry Licensing Board, the Department of Business and
Professional Regulation and the building department may be unable to assist you with any financial loss
that you sustain'as a result of a complaint. Your only remedy against an unlicensed contractor may be in

civil court. It is also important for you to understand that, if an unlicensed contractor or employee of an
individual or firm is injured while working on your property, you may be held liable for damages. If you
obtain an owner -builder permit and wish to hire a licensed contractor, you will be responsible for verifying
whether the contractor is property licensed and the status of the contractor's workers' compensation
coverage.

A violation of this exemption is a misdemeanor of the first degree punishable by a term of imprisonment
not exceeding 1 year and a $1,000.00 fine in addition to any civil penalties. In addition, the local
permitting jurisdiction shall withhold final approval, revoke the permit, or pursue any action or remedy
for unlicensed activity against the owner and any person performing work that requires1icensure under
the permit issued. '

SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction
materials. The documentation submitted has been reviewed by Miami -Dade County Product Control Division
and accepted by the Board of Rules and Appeals (BORA) to be used in Miami Dade County and other areas
where allowed by the Authority Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Miami -Dade County Product Control
Division (In Miami Dade County) and/or the AHJ (in areas other than Miami Dade County) reserve the right
to have this product or material tested for quality assurance purposes. If this product or material fails to
perform in the accepted manner, the manufacturer will incur the expense of such testing and the AHJ may
immediately revoke, modify, or suspend the use of such product or material within their jurisdiction. BORA
reserves the right to revoke this acceptance, if it is determined by Miami -Dade County Product Control
Division that this product or material fails to meet the requirements of the applicable building code.
This product is approved as described herein, and has been designed to comply with the Florida Building
Code, including the High Velocity Hurricane Zone.

APPROVAL DOCUMENT: Drawing No. JW0509-03-01, titled "Wood Edge Impact Glazed Steel Door
6'-8" Outswing Single or Double", sheets 1 through 6, of 6 prepared by 0. M. Engineering Services, Inc.,
dated 12/17/04 with revision "C" dated 02/05/08, signed, sealed and dated 06/07/08 by Nimesh Bhaysar,
P.E., bearing the Miami -Dade County Product Control Revision stamp with the Notice of Acceptance number
and expiration date by the Miami -Dade County Product Control Division.

LABELING: Each unit shall bear a permanent label with the manufacturer's name or logo, city, state and
following statement: "Miami -Dade County Product Control Approved", unless otherwise noted herein.
RENEWAL of this NOA shall be considered after a renewal application has been filed and there has been
no change in the applicable building code negatively affecting the performance of this product.
TERMINATION of this NOA will occur after the expiration date or if there has been a revision or change
in the materials, use, and/or manufacture of the product or process. Misuse of this NOA as an endorsement of
any product, for sales, advertising or any other purposes shall automatically terminate this NOA. Failure to
comply with any section of this NOA shall be cause for termination and removal of NOA.
ADVERTISEMENT: The NOA number preceded by the words Miami -Dade County, Florida, and followed
by the expiration date may be displayed in advertising literature. If any portion of the NOA is displayed, then
it shall be done in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to the user by the manufacturer or its
distributors and shall be available for inspection at the job site at the request of the Building Official.
This NOA revises NOA # 05-1215.01 and consists of this page 1 and evidence pages E-1 and E-2, as well as
approval document mentioned above.

Panel Construction: The active and inactive panels are cons"ed from 24 GA
0.021' min) galvanized steel. The face sheets at top and bottom are bent 90" over
the top and bottom rails. The top tall measures 1.0"high x 1.668" wide. The steel
bottom rail measures 1.21' high x 1.688' wile. The sides of the face sheets are roll
formed into the latch and hinge stiles which measure 1.00"high x 1.688" wide. The
interior cavity is filled with expanded polystyrene. The face sheets are glued to the
expanded polystyrene. The panels are cutout for the two-piece ODL Aluminum fits
frame. Units are glazed with tempered and laminated impact 1"IG. On double door
applications the inactive door is fitted with an extruded aluminum astragal of
6063-T5 alloy, manufactured by Endure.
Frame Construction: The frames are constructed of linger jointed pine jambs
measuring 4.9116"x 1-114' The head jambs are mortised and bult)oined to
the side iambs, and attached with three (3) 16GA 7I16' crown x 2" long
staples on each side. The threshhold Is attached to the side jambs with
three (3) 16GA 7116 ' crown x 2" long staples on each side.
Glazing., ODL insulated fife Is 1"overall thickness, consisting of one (1)
pane of 0.125'tempered glass; 0.5" air space; two (2) panes of a 125'
annealed with .090" PVB intedayer by Solutia. AN tiles are sandwiched
glazed into the panels.

directly on braced wood construction spaced at a maximum of
24" o.c. or over sheathing with vertical solid wood backing.
Please contact Nichiha regarding installation over steel frame

installing Sierra Premium Shake and Smooth boards, it is recommended
that an APA tared /16" OS13 or phwood Sheathing
be used. Boards may be installed over non-structural shcathrno
such as foam, fiber builder board, and N-psum type iancls.
Non-structural sheathing thickness shounot exceed I". Lie to
the compressive nature 450me nun -structural sheathings, care mast
be taken not to overdrive the fastener and compress the sheathing.
The fastener length must be increased by the thickness of
tic non structural`sheathing. Ensure all
weather resistive barriers arc in place in accordance with local
code. Framing misalignment
or irregularities in the sheathing application should
he corrected prior to siding installation. These irregularities may
mirror through the finished installation. Fastening Corrosion
resistant
fastcncrs. such as hot -di ed galvanized nail and screws
that are appropriate to local building codes and practices must
be used. Stainless steel fastcncrs arc highly recommended in
high -humidity and high -moisture regions. Nichiha is
not liable for corrosion resistance of fasteners. Do not use aluminum
fasteners, staples. clipped head or T-head nails, or fasteners that
are not rued or desirmed for intended use. Corrosion resistant.

Starter Strip &
First Course (Fi,;. l) A starter
strip is required to set the first course on the proper angle and
to create a drip edge at dhc bottom course of siding. The starter
strip material may he a cut piece of Sierra Premium, FS 1010 ti"
Nichifia Spacer, PV(`., or pressure treated lumber. Do not use untreated
hnubcr, phwood, OSB, or other engineered wood I)roducts
as a starter strip. nstall the
starter strip level with the buuom of the wail sheathing or mutdsill.
Ensure the
first course of boards is level as it will be the guide for entire installation,
iise a level and dtalk line to establish a level and straight
reference point for the first course. Continue to check for
level lines throughout installation. The first

or plywood sheathing is used, boards must be secured with a corrosion

For better appearance, stagger the second course 16". For the third course,
Rip the board (or reverse the pattern) and stagger' back another 16" from the
second course. Please note these dimensions oplily to 16" o.c. framing. For
other spacing plan accordingly: Avoid creating a 'step" or "ladder" pattern.
Shake kevw ys from one course to the next should not line up vertically.
Layout of the application may be nccessary to avoid stacked keyways.
Joint Treatment
Joint Installation (Fig. j)
The use of a joint flashing behind the bull joints is required.
Suitable materials for joint flashing include anodized or PVC coated coil
stock and cods; approved water resistive barriers.
1'he joint flashing should be at least 3" in width and long enough
to extend above the board width a minimum of 1-1/2".
fnstall Sierra Premium" Shake and Smooth boards over joint flashing with a
minimum I/S" gap between board ends. This gap ma}}' be filled with a
quality exterior grade caulk. Ensure caulk meets AS ri 183.1 or AS'fhi C-9 0
requirements and is approved for use at butt joints.

Where board ends and edges meet trim at inside and outside corners, door,
window and other trim attachments, leave a minimum 1/'8" gap and caulk
with a quality' caulk as described above.
Prim attachments must be of sufficient thickness, 91ic overlapped boards
must not extend beyond the face of the trim. A continuous solid shim or
build out" behind the trim may be necessarv.
A flashed I/-i" clearance must exist over hoiIzontal trim.
Alternative Corner Treatments (Fig. 8-9)
Sierra Premium Shake and Smooth boards may also be installed with
mitered corners. For best results, use a compound miter saw to accurately
cut the 45 degree miter cut and slopxe. an le.
Sierra Premium ShakeShale and Smooth ` roams may also be installed in a laced
or sul-nered corner application.
Nichiri recommends installing a galv:mized, coaled aluminum. or vin
PVC flashing over the building corner prior to beginning a mitered or J'
