NOTICE: In addition to the requirements of this permit, there may be additional restrictions applicable to this'property that may be
found in the public records of this county, and there may be additional permits required from other governmental entities such as water
management districts, state agencies, or federal agencies. .
Acceptance of permit is verification that I will notify the owner of the property of the requirements of Florida Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee at the time of permit submittal. A copy of the executed contract is required
in order to calculate a plan review charge and will be considered the estimated construction value of the job at the time of submittal.
The actual construction value will be figured based on the current ICC Valuation Table in effect at the time the permit is issued, in
accordance with local ordinance. Should calculated charges figured off the executed contract exceed the actual construction value,
credit will be applied to your permit fees when the permit is issued.

Scope of work as follows:
Includes:
Utility Room: (Added Utility Room)
Remove (Demo) (1) existing 150-amp 120/240-volt 1-phase main breaker load center
and associated circuit breakers.
Supply and install (1) 200-amp 120/240-volt 1-phase 120/240-volt 30/40-circuit
surface mount main breaker load center and necessary accessories (Ground bars,
Screws, Panel cover, etc.) (Note: Main breaker to be 150-amp to match existing)
Supply and install the following circuit breakers: (Note: New circuit breakers to match
existing FPE circuit breakers that were removed.)
12) 1-pole 20-amp circuit breakers. (Note: Existing load center has (9) 1-pole 20-
amp circuit breakers. There is one 20-amp 1-pole circuit breaker that has two # 12 wire
terminated & there is what appears to have been a 2-pole 30-amp circuit breaker
previously with the handle broken off and is currently being utilized as (2) 1-pole circuit
breakers. HLE mad provisions to supply and install (2) 1-pole 20-amp circuit breakers in
lure of the (2)1-pole 30-amp circuit breakers per to the NEC.)
1) 2-pole 20-amp circuit breaker. (Note: There is (1) 2-pole 20-amp breaker in a
breaker enclosure next to the existing FIRE panel. HLE made provisions to re-route to
new panel and eliminate breaker enclosure.)
1) 2-pole 50-amp circuit breaker. (Note: Existing load center has (2) 2-pole 50-amp
circuit breakers with two wires terminated ((1) # 12 & (1) # 10 on A -phase and (1) # 12 &
1) # 10 on B-phase. It appears that the # 10 wire is serving dryer outlet located under the
panel and the # 12 wire was serving the AC condensing unit that has been removed. HLE
made provisions to provide and additional 2-pole 20-amp circuit breaker & a 2-pole 30-
amp circuit breakerto separate to the loads being served)
4) 2-pole 30-amp circuit breaker.
Supply and install necessary PVC conduit and fittings to sleeve existing branch circuit
wiring from the new load center to the location that it enters the ceiling space above
the load center.

Supply and install necessary PVC conduit, PVC fittings, #12 THHN copper wire, 4-
square junction boxes, 4—square blank plates from the location of the new load
center to the location that the existing wiring enter the house through the exterior
block wall that is serving the garbage disposal and dishwasher located in the kitchen
area under the sink. (Note: Existing wiring is not installed per NEC.)
Remove (Demo) all other wiring in the Utility Room (Added Utility Room) not being
used.
Supply and install (1) 1-gang 3-hole weather-proof junction box, 1' of/Z Caro -flex
conduit, (2) caro-flex conduit straight connector, necessary # 12 THHN copper wire,
1) 15/20-amp weather resistant tamper -proof GFCI receptacles, (1) in -use type
weather-proof cover plate and necessary fasten materials. (Note: GFCI receptacle to
be installed under the new load center. '
Supply and install necessary W or 1" PVC conduit, %" or 1" PVC fittings, (1) 6x6x 4
PVC junction box and necessary fastening materials to sleeve existing wiring from the
new load center located in the utility room (Added utility room) the sub panel located
in the master bedroom (??) closet area.
Grounding per NEC.
Hot check and label new Panel. (Note: HLE will hot check and label panel if power has been
turn on. HLE made no provisions to repair existing circuitry or wiring. )
Exterior:
Supply and install (2) galvanized ground rods, 15' of # 4 Bare copper ground wire and
2) acom type glround rod clamps if required. (Note: Existing utility meter is located /
mounted to the exterior wall of the utility room (Added Utility Room). HLE made no provisions to
relocate or re -wire. HLE made provisions to install grounding per NEC only at the existing utility
meter.)
Note: HLE made no provisions to perform any other work other than what is listed above.
HLE made no provisions to replace or repair existing circuitry and or existing wiring. If any
problems are found at time of hot check HLE will notify THI of the issue observed for THI to
forward to whom it may concern.
Exclude:
Any other work other than what is listed above.
Patching and or painting of drywall and or concrete.
