NOTICE: In addition to the requirements of this permit, there may be additional restrictions applicable to this property that may be
found in the public records of this county, and there may be additional permits required from other governmental entities such as water
management districts, state agencies, or federal agencies.
Acceptance of permit is verification that I will notify the owner of the property of the requirements of Florida Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee at the time of permit submittal. A copy of the executed contract is required
in order to calculate a plan review charge and will be considered the estimated construction value of the job at the time of submittal.
The actual construction value will be figured based on the current ICC Valuation Table in effect at the time the permit is issued, in
accordance with local ordinance. Should calculated charges figured off the executed contract exceed the actual construction value,
credit will be applied to your permit fees when the permit is issued.

Before me, the undersigned authority, personally appeared Ryan KLessard MGR of HAPPY VALLEY PROPERTIES
L.LC, FLORIDA LIMITED LIABILITY COMPANY, on behalf of the Corporation, ("Affiant"), who being by me first
duly sworn, on oath, depose(s) and say(s) that:
I. Happy Valley Properties LLC, Florida Limited Liability Company ("Buyer"), is purchasing the following
described property from Doris Morales aka Doris Morales Vazquei, to wit:
Lot 12, Less the West 29 feet and the Nest 39 feet of Lot 13, Block 17, 2nd Section San Lanta, according
to the plat thereof recorded in Plat Book 4, Pages 38 through 40, in the Public Records of Seminole
County, Florida.
2. Buyers marital status as reflected in this affidavit and the other closing documents is true and correct. The Buyer is
of legal age and has never been adjudged incompetent. There are no matters pending against the Buyer that could
give rise to a lien that would attach to the property prior to the recording of the interests to be insured, and Buyer has
not and will not execute any instrument (nor permit any action to be taken) that would adversely affect the title or
interests to be insured. There are no judgments or liens against Buyer and no bankruptcy proceedings are currently
pending with respect to Buyer.
I To the best of Buyer's knowledge, information, and belief (a) within the past 90 days there have been no
improvements, alterations, or repairs to the above described property for which the costs thereof remain unpaid, and
that within the past 90 days there have been no claims for labor or material furnished for repairing or improving the
same, which remain unpaid; (b) there are no actual or potential mechanic's, materialmen's, or laborer's liens against
the property; (c) there are no tenancies, leases or other occupancies (oral or written) affecting the property; (d) no
other person or entity has any contract to purchase, option to purchase, right of first refusal, or other potential claim
of right to purchase the property.
a. Buyer knows of no violations of municipal ordinances pertaining to the property. or any action or proceeding
relating to the property which is pending in any court, nor does the Buyer know of any judgment, tax lien, or matter
of any nature whatsoever which could create a lien or charge upon the property. Buyer has no knowledge of any
matters that could or does create a cloud on the title to the subject property.

referenced property and, for the purpose of inducing First Signature Title, Inc. and Old Republic National Title
