This Work Authorization is conditional acceptance by PNC Bank, National Association (the "Owner") of
Construction Manager's bid for Construction Management services for the above -referenced project. Owner's
conditional acceptance of Construction Manager's bid is subject to the terms and conditions of a master construction
agreement (hereinafter, the "Agreement") entered into by Owner and the Construction Manager. Pursuant to the
Section 2.2.6 of the Agreement, the Owner and Construction Manager hereby amend the Agreement to establish a
Guaranteed Maximum Price and Contract Time for the Project as set forth below. The terms and conditions of the
Agreement are incorporated herein as if fully set forth below and the Construction Manager hereby re -affirms its
representations and warranties made therein. In order for this Work Authorization to be valid, there must be an
unexpired Agreement currently in effect:
This Work Authorization' is also subject to, among other things, Construction Manager's execution of this Work
Authorization and Construction Manager's delivery to Owner of the items described below and/or in the
Agreement. Prior to commencement of work on the above Project, Construction Manager MUST provide to Owner
the following:
l) Current certificate of insurance, in compliance with attached Exhibit C, referencing this project name, including
the waiver of subrogation language, and listing PNC Bank N.A. as an additional insured (send insurance to:

We are pleased to submit our Plumbing and Mechanical price in the amount of SEE PAGE 2 on the
above referenced project as described below.
The price is based solely on Attachment "A", and the further designations, clarifications and
qualifications stated on the Attachment "A" and the attached "Conditions of Proposal'.
This proposal is conditioned upon either the usage of the "AGC/ASA/ASC" Standard Form
Construction Subcontract" or a subcontract form otherwise accepted to Master Craft Plumbing Contractors Inc.
Specifically, this price is predicated upon the conditions that we shall receive prompt monthly payment
for the value of work properly performed and material delivered, and that payment can be withheld only to the

This information transmitted in this proposal supersedes any previously communicated information to
the contrary. Further, we reserve the right to modify our price and terms at any time prior to the time for
submitting your bid to the owner to reflect adjustments resulting from our continuing review.
Finally, and unless otherwise extended in writing from Master Craft Plumbing Contractors, Inc., this bid
including any such modifications shall remain effective and will not be further modified or withdrawn for 30
days from this bid.
