G&F RENOVATIONS INC (hereinafter "General Contractor" AND/OR "GC") and

this agreement (hereinafter "Agreement") on this 19 day of I,- , e,yn , , , 20_UI_.
1. Proiects: The Agreement applies to all projects (hereinafter "Project(s)") for which Sub -
Contractor hereinafter provides Work to General Contractor, except for those that are
already subject to another agreement between the parties. The term "Project(s)"
includes the properties on which renovations or construction is occurring.
2. Work: For all Projects, Sub -Contractor agrees to provide all materials, labor, tools,
equipment, supervision, supplies and other items necessary or required to perform the
work that is described in general on the attached Scope of Work and/or Bid Sheet. The
Scope of Work and/or Bid Sheer., together with any signed and written supplements or
modifications thereto for particular Projects, are incorporated into the Agreement, and
shall hereinafter be referred to collectively as the "Work."
I Construction Standards: Sub- Contractor commits to the goal of performing the Work
according to the highest standards of skill and workmanship and with "zero defects."
Sub -Contractor agrees to perform all Work in full compliance with or exceeding the
requirements of all of the following standards (hereinafter "Construction Standards"):
(a) plans, drawings or specifications provided by General Contractor for a particular
Project: (b) applicable federal, state, county and municipal codes, regulations, rules and
laws: (c) applicable manufacturer specifications, directions and suggested methods; (d)
private restrictions and covenants; (e) subdivision or homeowner association rules or

regulations; (f) any written warranty provided or to be provided by General Contractor
to Owner; (g) at least ordinary standards of care under the circumstances; (h)
requirements of the Federal Housing Administration or Veterans Administration; and (i)
standards set forth in the Scope of Work and /or Bid Sheet or any supplements or
modifications thereto.
General Responsibilities: In addition to its other responsibilities under this Agreement,
Sub -Contractor shall have the following responsibilities:
Sub -Contractor shall procure and pay for all permits, licenses, and inspections
required by any governmental authority for any part of the Work and shall
furnish any bonds, security, or deposits required by such authority to permit
performance of the Work. Sub -Contractor shall be also abide by all Regulations
Standards from OSHA, and will be responsible for any penalties, fees, fines
and/or payments of fines imposed if him/her or any of his/her people are in
violation of its regulations.
Sub -Contractor shall comply with, and insure the compliance by its employees,
other subcontractors and agents with, all applicable federal, state and local
laws, ordinances, statutes, rules, and regulations, including those relating to
wages, hours, fair employment practices, nondiscrimination, immigration and
naturalization, occupational safety or health and working conditions. The safety
of Sub -Contractor's employees, subcontractors and agents, whether or not in
common work areas, is the responsibility of Sub -Contractor.
C. Sub -Contractor shall visit each Project before starting the Work to understand
conditions relating to such Project, including access restrictions to the Project.
d. Sub -Contractor shall coordinate the installation of the Work with other
interfacing trades in a courteous and professional manner, so as not to interfere
with them, delay them or damage their work.
e. Sub -Contractor shall conduct the Work so as to avoid causing damage to any
part of a Project. Sub -Contractor shall be responsible for all such damage.
f. Sub -Contractor shall provide a qualified onsite supervisor whenever the Work is
being performed.
g. Sub -Contractor shall keep Projects clean of dirt, debris, trash, and any waste
materials arising from the performance of the Work. Sub -Contractor is
responsible for placing all trash and any construction debris created as a result
of the Work in the area designated by the General Contractor.
h. Sub -Contractor shall not remove or damage silt fencing or other erosion control
devices.

Sub -Contractor shall keep all vehicles off Projects, including driveways, unless
expressly permitted by General Contractor to drive onto a Project for a limited
purpose.
Sub -Contractor shall not use or permit the use by any of its employees,
subcontractors, suppliers or agents of alcoholic beverages or controlled
substances on or in connection with the Project or the Work.
Sub -Contractor shall not cover any known defect in the Work or in work by
someone else, but shall immediately notify General Contractor of such defect
and obtain permission to continue the pertinent aspect of the Work.
Sub -Contractor shall not allow food or smoking inside of a Project.
M. Sub -Contractor shall not allow shoes to be worn inside of a Project with finished
flooring installed.
n. Sub -Contractor shall not permit profanity, loud conversations or music, or other
behavior likely to disturb others working on a Project or persons living in
proximity to a Project.
o. Sub -Contractor understands that it is not authorized to make changes in the
Work at the request of any third party without obtaining written direction from
the General Contractor per the Agreement. Any requests for changes should be
referred to General Contractor.
P. Sub -Contractor shall limit its interactions with residents or potential tenants and
realtors so as to avoid confusion between Owners and these persons and shall
refer questions from them to General Contractor or Leasing Agent. Sub -
Contractor shall not offer them advice or opinions.
q. Sub -Contractor shall immediately inform General Contractor of any change of its
address or telephone numbers set forth above.
Sub -Contractor is fully responsible, in contract and tort, for the performance of
the Work that it subcontracts to anyone else.
5. Timing of Work: Sub -Contractor agrees to perform the Work in a timely manner and in
accordance with the requirements of any construction schedule that General Contractor
may from time to time develop and submit to Sub -Contractor. Within three (3) calendar
days after being notified by GC. Sub -Contractor shall commence actual construction of
the Work and shall thereafter diligently continue the Work to completion. If, in the
opinion of GC, Sub -Contractor falls behind the schedule, General Contractor shall take
steps as may be necessary to accelerate the Work.

6. Contract Price: The contract price and payment terms are set forth in the Bid Sheet and
shall remain in effect until the parties agree in writing to a new contract price or the
Agreement is terminated. The Bid Sheet is attached as part of the Agreement.

Applications: Sub -Contractor shall submit signed applications for payment to G&F
Renovations Inc., on a schedule adopted by GC, which schedule shall allow applications
at least every other week. The applications shall include an estimate by Sub -Contractor
of the percentage of the Work completed. Contractor's submission of an application
shall constitute a warranty and representation by Sub -Contractor to GC and Owner that
Sub -Contractor has performed that portion of the Work covered by the application in
full compliance with the Agreement, and has paid for all labor, materials, supplies,
equipment, tools and other items used in performance of the Work, and it has obtained
legally binding interim or final lien waivers from all such persons or entities. Builder
may require proof of such payment and of such waivers prior to making payment to
Sub -Contractor. The following individuals are expressly authorized by Sub -Contractor to
sign applications for payment on behalf of Sub -Contractor:

Waiver and Release upon Payment form or Waiver and Release upon Final
Payment form, respectively, as permitted by the State where the "Property" is
being performed "Work" is located.
a. Payments: After reviewing an application, G&F Renovations Inc. shall pay Sub -
Contractor the percentage of the Work that GC determines has, in fact, been
completed. Such payments shall be made according to GC's pay schedule
attached following timely submission of complete and accurate applications.
Untimely or inaccurate applications will cause a delay in payment.
8. Warranty: Sub -Contractor warrants all Work against defects in labor and materials for a
period of one year or, if longer, for a period coinciding with GC's warranty to Owner on
any Project and to any subsequent owner of a Project if GC's Warranty extends to
subsequent owners. Within three (3) working days after being notified of such defect by
GC. Sub -Contractor shall remedy, at its own expense, any such defect, unless the defect
requires emergency attention, in which case Sub -Contractor shall immediately address
the defect, and Sub -Contractor shall pay any damages to GC, Owner or subsequent
owner resulting from such defect. If Sub -Contractor fails to remedy any such defect, GC
may do so and Sub -Contractor shall be liable to GC for all costs associated with
performing such remedy. This warranty shall not limit Sub -Contractor's other
obligations under the Agreement or law.
9. Insurance: Even if not required by law, Sub -Contractor shall maintain, on its own and at
its own expense, the following in rance coverage's: (a) workers compensation

insurance in the amount not less than $1,000,000 ; (b) general liability insurance
minimum of $1,000,000; (c) property damage insurance; and (d) automotive public
liability and automotive property damage insurance in an amount not less than:
1,000,000. (i) $1,000,000 for any one person; (ii) 51,000,000 for bodily injury for any one
occurrence; and (iii) $1,000,000 for property damage for any one occurrence.. Each
Certificate of Insurance shall name G&F Renovations Inc. as certificate holder and shall
state that the insurance will not be canceled without thirty (30) days written notice to
GC. The certificate for the general liability policy shall name G&F Renovations Inc. as an
additional insured. Sub -Contractor shall deliver to GC, without expense to GC,
certificates from Sub -Contractors insurance companies or broker prior to
commencement of work and along with this signed agreement.
10. Indemnification: Sub -Contractor agrees to indemnify and hold harmless Contractor, as
well as its shareholders, directors, members, volunteers, officers and employees, from
any claims, damages, losses, expenses or fines, OSHA fines, including attorney's fees
and legal expenses, arising out of or resulting from either a breach of the Agreement or
any negligent or intentional act or omission by Sub -Contractor or its employees,
subcontractors, suppliers or agents other than for bodily injury to persons or damage to
property caused by or resulting from the sole negligence of Sub -Contractor or its agents
or employees.
11. Dispute Resolution: Except as hereinafter provided, any dispute between General
Contractor and Sub -Contractor arising out of or relating to the Agreement shall be
resolved by mandatory, binding arbitration in the courts of the State where the
"Property" it's being "Worked" at, is located, and in accordance with the rules of
Construction Arbitration Associates, ltd. in effect at the time of the initiation of the
arbitration and the decision of such arbitrator shall be final and enforceable in any court
having jurisdiction. However, if General Contractor is a party in either litigation or
arbitration, before the same or a different arbitration entity, involving claims between
GC and any Owner or Tenant of a Project that arise out of or relate, in whole or in part,
to the Work required by this Agreement or any obligation of Sub -Contractor under this
Agreement, Sub -Contractor agrees that GC may bring Sub -Contractor into such litigation
or arbitration as a third or additional party in order to assert claims against Sub -
Contractor relating to such Work or obligations, including claims for indemnity or
contribution. The results of any such arbitration shall be binding and enforceable in a
court of competent jurisdiction. This paragraph shall apply to disputes covered by it
even if the Agreement is terminated or rescinded.
12. Independent Contractor Status: Sub -Contractor is an independent contractor of
General Contractor and is not an employee of G&F Renovations Inc. Sub -Contractor will
withhold from its payrolls as required by law or government regulation and shall have
full and exclusive liability for the payment of any and all taxes and contributions for
unemployment insurance, worker's compensation, and retirement benefits that may be
required by federal or state governments.
13. Breaches: Sub -Contractor shall be liable to General Contractor for all damages arising
out of or relating to breaches by Sub -Co trac r under the Agreement. General

Contractor is entitled to deduct such damages from payments otherwise due to Sub -
Contractor under the Agreement and to pursue Sub -Contractor for any additional
damages.
14. Termination: If Sub -Contractor breaches the Agreement and fails to cure such breach
within three (3) business days following written notice from General Contractor, GC may
terminate the Agreement completely or may, without terminating the Agreement,
terminate Sub -Contractor from the Work on particular Projects on which such breach
occurred. If GC terminates the Agreement or terminates Sub -Contractor as permitted
herein, it shall be entitled, without waiving any other claim against Sub -Contractor, to
retain so much of any funds that may otherwise be due to Sub -Contractor as are
necessary to remedy such breach or to complete such work through other means.
Either party is free to terminate the Agreement as to future Projects with written notice
prior to the commencement of Work on such Projects.
15. Miscellaneous:
The agreement does not give exclusive rights to Sub -Contractor to work with General
Contractor. G&F Renovations Inc. may create agreements with other parties in the
same trade without affecting the Agreement.
a. The Agreement shall be governed by, and interpreted in accordance with the
laws of the State where the Property it's being "Worked" at, is located.
b. The Agreement is the entire agreement between the parties and can only be
modified or amended in writing.
C. No representation, promise or inducement not included in the Agreement is
relied upon or shall be binding upon either party.
d. Time is of the essence of the Agreement.
e. Notices required in the Agreement shall be in writing to the other party at the
address of such party set forth above and shall be given in person by overnight
delivery service prepaid, by facsimile, or by certified mail, return receipt
requested. Such notice shall be deemed given when received at such address.
f. Each provision of the Agreement is severable from every other provision. If any
provision is determined to be unenforceable, the rest of the Agreement shall
remain valid and enforceable.
g. All plans, drawings and specifications provided by General Contractor to Sub -
Contractor shall remain the property of G&F Renovations Inc.
h. The term "including," shall mean, including but not limited to the specific item
or matter mentioned.

M Certification: Sub -Contractor certifies that it: (a) has read the Agreement and
understands it; (b) has the skills and qualifications necessary to perform the Work in
accordance with the Agreement, and to otherwise comply with the Agreement; (c) has
all licenses, registrations and similar governmental authorizations necessary to perform
the Work and to comply with the Agreement; (d) is familiar with all of the Construction
Standards and applicable laws, rules, regulations, codes, documents and other matters
mentioned in the Agreement; and (e) has the authority to sign the Agreement. The
individual signing on behalf of Sub -Contractor represents and certifies that he is
authorized to sign this Agreement on behalf of Sub -Contractor.
17. Protection of People and Property: Sub -Contractor shall be responsible for initiating,
maintaining and supervising all safety precautions and programs in connection with the
performance of the Contract. The Sub -Contractor shall take reasonable precautions for
safety of, and shall provide reasonable protection to prevent damage, inquiry or loss to
employees, the work materials and equipment to be incorporated therein; and the
Property at the Site or adjacent thereto. Sub -Contractor shall abide by all rules and
regulations of OSHA and will be responsible for any penalties, fees, and/or any fines
imposed by this organization and release the GC of any responsibilities regarding
violations of its regulations.
18. The Sub -Contractor shall give notices and comply with applicable laws, ordinances,
rules, regulations, and lawful orders of public authorities bearing on safety of persons
and property and their protection from damage, injury or loss. The Sub -Contractor
shall promptly remedy damage and loss to property caused in whole or in part by the
Sub -Contractor, or anyone directly or indirectly employed by any of them, or by anyone
for which the Sub -Contractor is responsible.

EARLY START AUTHORIZATION CONDITIONS
City of Sanford and the Owner/contractor listed agree to the following:
I . A complete building permit application and plans shall be submitted at the time of the request for Early Start Authorization.
2. This Early Start Authorization is for interior work or other work as determined by the Building Official.
3. Work must comply with any and all other local, state and federal agencies related to the development and construction
proposed and compliance with asbestos NESHAP regulations must occur for all demolition work.
4. Work shall not be concealed and must remain open for all necessary inspections. At the time of inspections, any work
concealed shall be uncovered.
5. The contractor acknowledges that all subcontractors will be properly licensed and have current worker's compensation
coverage.
6. All subcontractors are responsible for pulling their own permits.
7. Inspections of work for any construction trade will not be made until a permit has been issued for the trade.
8. The Early Start Authorization does not guarantee that construction plans will be approved as submitted. All work done prior
to the issuance of the required permits shall be at the Owner's/Contractors risk.
9. The Owner/Contractor acknowledge that additional fees, including but not limited to impact and zoning fees, may be due at
the time of building permit issuance, and or prior to Certificate of Occupancy.
10. The Owner/Contractor acknowledge that additional site improvements, including but not limited to installation of a grease
trap, accessible parking and landscaping, may be required'at the time of building permit issuance.
11. The Owner/Contractor agree to indemnify and hold the City of Sanford/Agents free and harmless from any and all claims,
causes of action, damages, losses penalties or costs, including but not limited to, all attorneys fees (whether from litigation or
administrative proceeding, including cost and fees on appeal), with respect to any person or government authority arising out
of, either directly or indirectly, the construction or operation at the premises covered by the Early Start Authorization,
whether the liability, loss or damage is caused by, or arises out of, the negligence or the City of Sanford/Agents or its
officers, agents, employees, or otherwise.
12. If the City of Sanford/Agents shall be subject to any claim, demand or penalty or become a party to any suit or other judicial
or administrative proceeding by reason of any claimed act or omission by any party, or by reason of any act occurring on the
subject premises, or by reason of any omission with respect to the construction or operation on the subject premises, the
Owner/contractor shall indemnify and hold City of Sanford/Agents harmless against all judgments, settlements, penalties and
expenses, including attorney's fees, court costs and other expense of litigation of defense relating to such claim or litigation
or administrative proceeding, at the election of the City of Sanford/Agents, the Owner/Contractor shall also defend the City
of Sanford.
13. It is understood and hereby acknowledged between the parties hereto that the City of Sanford/Agents shall not be liable for
any act or other obligation to the Owner/Contractor.
14. This Early Start Authorization will terminate upon the issuance of a Building Permit for property covered under this Early
Start Authorization; however, this agreement shall remain in effect for all events occurring prior to the issuance of the
Building Permit.
By signing this Early Start Authorization Application, the undersigned acknowledges and agrees to condition 1 through 14.

The OWNER agrees to pay to the CONTRACTOR for the satisfactory performance and completion of the Work and all of the
duties, obligations and responsibilities of CONTRACTOR under this Agreement, the contract amount listed above, which shall be
paid upon completion.
Payments made within 30 days of receipt of invoice provided that CONTRACTOR shall have fulfilled all of the satisfactory
obligations to be performed by CONTRACTOR under this Agreement and the Owner or the Owner's Authorized Agent has
accepted the work.
CONTRACTOR agrees to furnish all labor, materials and equipment necessary to perform the work in accordance with their
attached proposal. Contractor's proposal is attached only to serve as a description of the work, no other provisions of the proposal
shall be allowed to take precedence over the terms of this agreement unless it offers warranty or guarantee periods in excess of
those required by this document.
SCOPE OF WORK: See attached proposal
Restore building to pre -fire condition according to Revised Insurance Adjuster scope of work. Work to include apartments
8-301 and 8-302. All exterior including roof, trusses, siding, breezeways and staircases.
Liquidated damages of $24.36 per unit per day for loss of rent if contractor does not complete project on given date according
to schedule. Number based on current rent $731.00 per month divided by 30 days.
Payment schedule is as follows:
30% - $42,555.76 - upon Mobilization
300/9 - $42,555.00 - 50% of completion
30% - $42,555.00 - 75% of completion
10% - $14,185.00 - 30 days after completion

General Conditions
A. CONTRACTOR hereby agrees that the Work will be performed in strict accordance with the specifications on the Contract Documents herein defined.
CONTRACTOR agrees to perform the Work to the NII satisfaction of the OWNER and/or his authorized agent. CONTRACTOR shall commence the
Work immediately when notified by OWNER and shall execute said WORK in a good and workmanlike manner with due diligence and without delay.
The OWNER reserves the right to have any portion of the Work, for which the CONTRACTOR has not been directed to proceed, accomplished by his
own forces or by others.
B. Time is of the essence in the performance of the Work referred to in this Agreemem, in as much as failure of the CONTRACTOR to commence and
complete the Work as and when required by the OWNER may cause grave injury and damage to the OWNER by virtue of loss of sales and good will,
delayed receipt of income from rental units, extension of overhead costs and otherwise. Accordingly, the CONTRACTOR agrees to prosecute the Work
with due diligence and will not in any manner delay or otherwise interfere with the work of the OWNER or other Subcontractors.
C. CONTRACTOR acknowledges that no liens shall be attached to the real estate by virtue of any work done hereunder by the CONTRACTOR or by any
suppliers, employees, material men, or other Subcontractors employed by him; and the CONTRACTOR warrants that all sucb parties shall be advised of
same and certifies to the OWNER that they are aware thereof and bound thereby. OWNER has the right to request Waivers of Lien to be relinquished
by CONTRACTOR as payments are received. Full Waives of Lien and Affidavits may be required from the CONTRACTOR prior to final and !lull
Payment.
D. CONTRACTOR shall maintain at all times, while obligated to perform under the terms of the contract, the following insurance coverage:
1. A policy of workmen's compensation and employer's liability insurance, with a limit of no less than Five Hundred Thousand dollars S500.000.00)
for the protection of CONTRACTOR's employees, including, without limitation, executive, managerial and supervisory employees, engaged in
the work.
Il. Proof of insurance for any and all vehicles, with a minimum of no less than One Million dollars (31.000.000.001 to be used by the CONTRACTOR
in order to perform his services.
III. A policy of general liability insurance including completed operations covering loss resulting from the CONTRACTOR'S direct and indirect
activities hereunder, whether performed by CONTRACTOR or by its Subcontractors or suppliers, and covering injury and damage to any person
employed by CONTRACTOR and its Subcontractors and suppliers, and all others who might be injured or damaged as a result of performance of
the work, and covering damage to property. The minimum bodily injury coverage (including death) shall not be less than One Million dollars
(SI.000.000.001 for each occurrence. The property damage coverage required herein shall not be less than One Million dollars (SI".000.00) for
each occurrence and shall protect the work, and any improvements on the property adjacent to the area where the work is being performed. If said
bodily injury and property damage coverage are combined, total amount of coverage is to be One Million dollars (SI.000.00.00)
IV. Coverage requirements are to be evidenced by providing a Certificate of Insurance naming the OWNER and TWC Seventy -Sox, Ltd. As
Certificate Holder and shall provide that such policy may not be (i) candid, (o) allowed to expire, or (iii) reduced in its coverage limits without
thirty (30) days prior written notice to the Certificate Holder.
V. CONTRACTOR SHALL, CONCURRENTLY WITH ITS EXECUTION HEREOF, DELIVER TO THE CERTIFICATE HOLDER THE
CERTIFICATE OF INSURANCE REFLECTING INFORMATION OUTLINED ABOVE.
E. Provide the Community with a copy of MSDS for any and all materials used on the property.
F. The OWNER, or his representative, may order changes in the work, consisting of additions, deletions, or other revisions, the Contract price and the
Contract time being adjusted accordingly. The OWNER shall authorize all such changes in the work in writing.
G. CONTRACTOR shall indemnify and hold the OWNER. OWNER's agents and to Property frce of and harmless from any and all actions or causes of
action, claims, demands, liabilities, losses, damages or expenses of any kind and name, including attorney's fees, which the OWNER, OWNER'S
agents or the Property shall or may at any time sustain or be liable for in consequence of any injury or damage to person or property which may arise
directly or indirectly from the performance of this Contract by the CONTRACTOR and its subcontracts, material men, supplies, and anyone directly or
indirectly employed by or acting for any of them.
H. CONTRACTOR shall at all times keep the premises free from accumulation of waste materials or rubbish caused by its performance of the work.
1. The CONTRACTOR shall be responsible to the OWNER for the axis and omissions of all CONTRACTOR's employees and all Subcontractors, their
agents and employees, and all other persons performing any of the work for said CONTRACTOR. The CONTRACTOR shall be responsible for any
and all damages caused by his employees, agents, or representatives and shall repair all damages to the condition that existed before said changes took
place or reimburse OWNER for making repairs.
J. The CONTRACTOR shall obtain all permits and license that may be necessary for the proper performance of this Work. The CONTRACTOR, and all
acting under is direction or control, shall take all necessary precautions for the safety of employees performing the work and shall at all times comply
with all Local, State and Federal statutes, ordinances, rules and regulations, as well as those of other public body having authority concerning the Work.
The CONTRACTOR shall hold and save the OWNER harmless and indemnified against all claims arising out of any violations or noncompliance with
the provisions of the article.
IC The CONTRACTOR shall pay all Sales Taxes, F.I.C.A. and Unemployment Compensation Taxes upon the material and labor fumished under this
Contract, as required by the United States Government and the State in which this work is performed.
L. ALL CONTRACTOR's personnel must wear uniforms with identifying nametags or embroidery telling the name of the company, as well as the
employee's name.
M. All work is to be performed at a time and in a manner that will least disrupt the Residents of the Community. No employee will be them before 8:00
a.m. or after 6:00 p.m. — Monday through Saturday unless authorized by Owner.
N. This Contract shall not be assigned by the CONTRACTOR without first obtaining permission in writing from the OWNER.
0. CONTRACTOR shall be responsible for the protection of the OWNER's property or public or private adjacent to the Property at all times.

General Conditions
P. CONTRACTOR agrees to conform to the progress schedule, which the OWNER shall issue.
Q. All equipment. materials and articles used in the work covered by this contract shall be of most suitable grade of their respective kinds for the purpose.
R. The CONTRACTOR shall promptly notify the OWNER in writing of any or all conditions which. in his opinion, will affect the satisfactory execution of
his Work or endanger its permanence. Upon beginning work, the contractor shall be liable for correcting all work and materials which produce an
unsatisfactory installation according to standard methods, techniques, and practices within the industry or that do not conform to applicable
specifications, drawings, building codes, or government regulations.
S. The CONTRACTOR shall supervise and direct the work. The CONTRACTOR shall notify the Property Manager 48 hours in advance of manning the
job and shall at all times enforce strict discipline and good order among his employees. The CONTRACTOR shall have present on the property, a
designated Forman as his representative who shall be responsible for the work and the CONTRACTOR's employees.
T. The CONTRACTOR shall be responsible for initiating maintaining and supervising appropriate safety precautions in connection with his wort. He
shall take all reasonable precautions for the safety of his employees, agents and residents of the property. The CONTRACTOR shall comply with all
appropriate OSHA regulations and procedures, and shall make all reasonable efforts to protect his work in progress.
U. As to Permits and Cerificates, the CONTRACTOR shall comply with the rules and regulations and requirements of all legally constituted authorities.
The certificates of approval of the authorities shall be delivered to the Property Manager before payment will be authorized. Failure of die Property to
demand these approvals shall not constitute a waiver of this clause.
V. CONTRACTOR guarantees that all the Work shall be free from defects in worknumship and materials for a minimum period of one (1) year from the
date OWNER accepts the Work and promptly upon OWNER's request will correct by repair or replacements, without charge, any such defects (and any
damage to other property, including without limitation the work of other Subcontractors resulting therefrom or from the correction thereof) which may
appear in the Work during that period In the event that Contractor's attached proposal offers guarantees or warrantees in excess of one (1) year, that
specific provision of the Contractor's proposal shall take precedence over this minimum requirement.
W. The OWNER, at its sole discretion, may terminato this Agreement at any time by giving at least thirty (30) days prior written notice of such termination
to the CONTRACTOR with a written notice delivered to the CONTRACTOR via Certified Mail. Upon any termination of this Agrccmeny and subject
to all the terms and provisions herein contained, the CONTRACTOR shall be entitled to payment at the contract price for all accepted work furnished or
installed by him. However, the OWNER may retain from any monies due to the Contractor, an amount sufficient to cover CONTRACTOR's
obligations under the guarantee of materials and workmanship as contained herein. Upon the expiration of such obligations, the balance of such
amount if any, shall be paid to the CONTRACTOR. The CONTRACTOR upon termination of this Agreement, shall forthwith peaceably and quietly
surrender to the OWNER all premises, facilities, machinery, and equipment by or belonging to the OWNER.
X. This Contract shall be construed and enforced in accordance with the laws of the State where Property is located.

each box to the left or indicate n/a on this submittal. A complete application package shall
include the following:
Building Permit Application completed, signed and notarized. Application must include correct address
and complete parcel I.D. number.
Gam Copy of the contractor's license issued by the State of Florida (if contractor is applicant).
p� A site specific notarized power of attorney shall be required from the licensed contractor if
he/she appoints an employee of his/her company to sign the permit application as the contractor.
Certificate of insurance indicating worker's compensation insurance coverage and naming the City of
Sanford as certificate holder, or a copy of a worker's compensation exemption issued by the State of
/Florida (must be submitted with each application if contractor is the applicant).
GY Completed, signed, and notarized Property Owner Builder Disclosure Statement Affidavit (if owner is
applicant).
O Approval letter from sanitary sewer provider (if other than the City of Sanford).
O Copy of the onsite sewage disposal system construction permit issued by Seminole County Health
Department for new or existing septic systems, grease interceptors, etc. (if applicable).
