The City of Sanford requires payment of a plan review fee at the time of permit submittal. A copy of the executed contract is required
in order to calculate a plan review charge and will be considered the estimated construction value of the job at the time of submittal.
The actual construction value will be figured based on the current [CC Valuation Table in effect at the time the permit is issued, in
accordance with local ordinance. Should calculated charges figured off the executed contract exceed the actual construction value,
credit will be applied to your permit fees when the permit is issued

Conditions: We hereby propose to furnish labor, materials, supervision, equipment, insurance, taxes, permits and dump
fees in accordance with state and local building codes to the above referenced property as follows: Shingle roof
replacement main residence: Removal of
entire existing damaged roof and transport of waste to an approved construction debris dumpsite. We will
inspect decking board for damage and replace rotten wood charged in addition to price herein. We charge $50
per board, which includes labor and materials. We will properly nail decking according to current Hurricane Mitigation
codes. Dry in
using Midstates brand Peel & Stick underlayment as a secondary water barrier. We will install new 2 Wdrip edge
metal and off ridge vents. We will replace all ventilation stacks with new products. We will
replace flashing metal on the fireplace and install a "Cricket" water diverter. We will replace wall flashing on the
front slope wall and install a "kick -out" water diverter. installation of
Tamko brand Heritage dimensional shingles according to manufacturer instructions and building code regulations
with 30 years Limited Warranty. Tamko Heritage shingles have a 15-year Sure Start warranty which pays
for labor and materials for defects in the product. Beginning the 16th through 301h year, the warranty is pro
rated. Photographs of every phase of construction will document our progress. As of the date of this proposal, work
scope and products specified meet Florida Building Code 5th Edition 2014 Requirements —SEC 201 Hurricane
Mitigation Provisions effective June 30, 2015. Our Price:
S7,500.00 (Seven thousand five hundred dollars) Larry Meyers Roofing owns all materials until the balance ofthe contract is paid in full. Although
we wRl make every effort to avoid damage, Larry Meyers Roofing cannot be held responsible for minor peripheral damage caused by construction activity to
grass, plants, driveways, sprinklers, air conditioning units or items inside buildings. The owner will be liable for all costs incurred from non-payment of this contract allowed
bylaw. Invoices not paid within 30 days of bill date will accrue interest at the rate of 1,55E per month and be subject to Florida's Construction Uen Law. ACCORDING
TO
FLORIDA S CONSTRUCTION LIEN LAW (SECTIONS 713.001— 713.37, FLORIDA STATUTES), THOSE WHO WORK ON YOUR PROPERTY OR PROVIDE MATERIALS AND
SERVICES AND ARE NOT PAID IN FULL HAVE A RIGHT TO ENFORCE THEIR CLAIM FOR PAYMENT AGAINST YOUR PROPERTY. THIS CLAIM IS KNOWN AS
