commenced prior to the issuance of a permit and that all work will be performed to meet standards of all laws regulating construction
in this jurisdiction. 1 understand that a separate permit must be secured for electrical work, plumbing, signs, wells, pools,
furnaces, boilers, heaters, tanks, and air conditioners, etc.

NOTICE : In addition to the requirements of this permit, there may be additional restrictions applicable to this property that may be
found in the public records of this county, and there may be additional permits required from other governmental entities such as water
management districts, state agencies, or federal agencies.
Acceptance of permit is verification that I will notify the owner of the property of the requirements of Florida Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee at the time of permit submittal. A copy of the executed contract is required
in order to calculate a plan review charge and will be considered the estimated construction value of the job at the time of submittal.
The actual construction value will be figured based on the current ICC Valuation Table in effect at the time the permit is issued, in
accordance with local ordinance. Should calculated charges figured off the executed contract exceed the actual construction value,
credit will be applied to your permit fees when the permit is issued.

'2. All equipment and material are guaranteed by Facemyer Air Conditioning and Heating to be as specified. Ag work will be
completed in a workmanlike manner according to normally accepted practices.
3. Materials and work in addition to that described herein will be famished only on Purchaser's authorization and will be paid by
Purchaser as an extra charge.
4. Upon failure to pay any sums due hereunder. Purchaser agrees to pay Facemyer Air Conditioning and Heating interest at the
rate of one and one half percent (1 %0/9) per month (annual rate of 1896) on all outstanding balances.
S. Facemyer Air Conditioning and Heating shag not be liable for any default caused by events beyond its control, including but not
limited to, fire, flood, strikes, accidents, or delays affecting this work or other operations in which it Is involved, directly or indirectly.
6. Purchaser shall permit Facemyer Air Conditioning and Heating reasonable access to the property on which equipment is to be
Installed. Title to all provided equipment remains with Facemyer Air Conditloning and Heating until all amounts due thereon are
paid in fug, whether such equipment is affixed to the really or not and shag remain personal property and be deemed sever-abte
without injury to the freehold. On any payment default by Purchaser, or if in Facemyer Air Conditioning and Heating's judgment,
reasonably exercised, Its equity appears to be imperiled, then, Facemyer Air Conditioning and Heating may without further notice
enter the premises and remove or resell the equipment, and Purchaser shall be gable for any deficiency or loss sustained by
Facemyer Air Conditioning and Heating in connection therewith.
7. Once equipment is connected at Purchaser's property, Purchaser assumes all risk of loss or damage to such equipment and
shell Insure same fully to protect all interests of Facemyer Air Conditioning and Heating, cost of Insurance to be paid by Purchaser.
Facemyer Alr Conditioning and Heating carries liability insurance and Worker's Compensation Insurance.
8. Facemyer Air Conditioning and Heating provides a one-year limited labor warranty. Equipment or system failure due to lack of
proper maintenance service or abuse Is expressly excluded. Normal maintenance check-ups and fitter replacements are the
responsibility of Purchaser. Ag other warranties, expressed or implied, are the responsibility of the manufacturer of the equipment,
parts, or materials used in connection with the services.
g. There are no warranties, expressed or implied, for existing equipment, ductwork, or other materials not installed by Facemyer
Air Conditioning and Heating
10. All warranty work wig be performed during Facemyer Air Conditioning and Heating's normal working hours, 8:OOAM to 5:OOPM,
Monday through Friday.
11. Purchaser Is responsible for all costs and reasonable attorney fees incurred by Fecemyer Air Conditioning and Heating In
connection with any action or proceeding (including arbitration and appeals) arising out of this agreement, including collection of
any outstanding amounts due, whether or not suit Is filed.
12. Except as provided herein Facemyer Air Conditioning and Heating makes no other representations or warranties, either
express or Implied, including, but not limited to, any implied warranties of merchantability or fitness for a particular purpose
Facemyer Air Conditioning and Heating expressly disclaims all other warranties. Facemyer Air Conditioning and Heating's
maximum liability hereunder shall consist of refunding all moneys paid to it by Purchaser hereunder subject to removal and return
to Facemyer Air Conditioning and Heating of all equipment provided hereunder. Under no circumstances will Facemyer Air
Conditioning and Heating be liable to Purchaser or any other person for any damages, including, without limitation, any indirect,
Incidental, special, or consequential damages, expenses, cost. profits, lost savings or earnings, lost or corrupted data, or other
liability arising out of or related to this agreement, or the services or equipment provided hereunder.
13. This agreement shall be governed and construed solely according to the Internal laws of the State of Florida, without reference
to any conflicts of laws.
14. This agreement is the complete and exclusive statement of the agreement between Purchaser and Facemyer Air Conditioning
and Heating and it supersedes all prior oral and written proposals and any prior or subsequent communications pertaining to the
subject matter hereof.

Facemyer Air Conditioning and Heating proposes hereby to furnish and install the equipment and materials as described above on
the terms and conditions provided herein. This proposal Is good for thirty (30) days from the date hereof, but may be accepted at
any later date at the sole discretion of Facemyer Air Conditioning and Heating

HVAC Permit Application Checklist
All permit application packages must be complete prior to acceptance. You must check each
box to the left or indicate n/a on this submittal. A complete application package shall
include the following:
Building Permit Application completed, signed and notarized. Application must include correct address
and complete parcel I.D. number.
Copy of applicable contractor's license tissued by the State of Florida (if the contractor is the
applicant).
OtiA A site specific notarized power of attorney shall be required from the licensed contractor if
he/she appoints an employee of his/her company to sign the permit application as the contractor.
[ Certificate of insurance indicating worker's compensation insurance coverage and naming the City of
Sanford as certificate holder, or a copy, of a worker's compensation exemption issued by the State of
Florida (must be submitted with each application if contractor is the applicant).
O Na Completed and signed Owner Builder Statement/ Affidavit (if the owner is the applicant).
O N A One (1) copy of equipment sizing calculations —for new construction installations:
o Residential - ACCA Manual ,J-2003 or other approved heating and cooling calculation
methodology.
o Commercial - ACCA Manual , N-2005 or other approved heating and cooling calculation
methodology.
These guidelines were compiled to assist the applicant in preparing a HVAC change out permit application and
may not be complete. The applicant is required to meet all City of Sanford, state, and federal code
