Acceptance of permit is verification that 1 will notifyt e owner of the property of the requirements of Florida Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee at the time of permit submittal. A copy of the executed contract is required
in order to calculate a plan review charge and will be considered the estimated construction value of the job at the time of submittal.
The actual construction value will be figured based on the current ICC Valuation Table in effect at the time the permit is issued, in
accordance with local ordinance. Should calculated charges figured off the executed contract exceed the actual construction value,
credit will be applied to your permit fees when the permit is issued.

Scope of work: The work of this division consist of Nmishing all plant labor, materials, equipment, supervision, and
performing all operations required to complete all items of work in accordance with these specifications, applicable
drawings and all other applicable portions of the complete contract documents. lob Address: 804 E 8i1 St, Sanford,

' Specification: Proposal is based on the submitted plans/electrical drawings as requested by
the building manager/owner/general contractor. With revision as indicated with the 2011
NEC.
The installation of:
- Repair and upgrade electrical services (main panel) to 150 Amps.
- NOTE: an electrical permit.will be pulled from the building department prior to start this job. The
authority having jurisdiction over this job( building inspectors) might/might not request extra jobs/code
violation corrections, are considered outside our scoop of work, and if that happen will be treated as
extra, and in order to proceed with extra work, a change order need to be approved and signed by home
owner.
"Warranty: We guarantee for (1) year against defects in material and workmanship. Failure due to misuse, vandalism,
fire, damage and/or natural causes is not covered by this warranty."
We hereby propose to furnish material and labor — complete in accordance with the above specifications, for the
sum of:
$1,500.00
Payments are to be made as follow: $ 750.00 sign this contract and start the job, and $750.00
with a final inspection approved., Any deviations from the terms this contract must be recorded on a change order
form which is signed and approved by the hometbusiness owner. Work is to be invoiced upon completion. Payment is
due five (5) days from receipt of the invoice. A finance charge of 1.5% per month will be charged on all invoices not
paid within 30 days.
All work is to be done In professional manner according to standard practices. Any
