furnaces, boilers, beaters, tanks, and air conditioners, etc.

NOTICE: In addition to the requirements of this permit, there may be additional restrictions applicable to this property that may be
found in the public records of this county, and there may be additional permits required from other governmental entitles such as water
management districts, state agencies, or federal agencies.
Acceptance of permit is verification that I will notify the owner of the property of the requirements of Florida Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee at the time of permit submittal. A copy of the executed contract is required
in order to calculate a plan review charge and will be considered the estimated construction value of the job at the time of submittal.
The actual construction value will be figured based on the current ICC Valuation Table in effect at the time the permit is issued, to
accordance with local ordinance. Should calculated charges figured off the executed contract exceed the actual construction value,
credit will be applied to your permit fees when the permit is issued.

ROOF REPLACEMENT
We hereby submit Specifications for:
DEMOLITION AND PREPARATION FOR INSTALLATION OF NEW SHINGLES:
1.1. Remove existing one (1) layer shingle roof and felt paper underlayment to expose wood roof deck.
1.2. Inspect wood decking, fascia, and trusses and repair defective or decayed wood at the additional rate of
$70.00/ man hour plus materials.
1.3. Re -fasten roof sheathing as mandated with .113" ring shank 8D decking nails and install simplex
fasteners at 6" on -center spacing in shingle felt laps and 12" on -center staggered in the field as a
secondary water barrier in accordance with current `My Safe Florida Home' standards.
SHINGLE ROOF INSTALLATION:
2.1. Install new shingle roof as follows:
A. Secure underlayment to comply with ASTM D226, Type II, or D4869, Type IV or D6757 shingle
underlayment to deck as dry -in and roof system vapor barrier.
'Shingle underlayment to be fastened with: a) 3" minimum laps on --26° pitched slopes; b) 16"
minimum laps on --16° pitched slope.
B. Install new valleys using ASTM D1970 modified bitumen self -adhering roof membrane and
closed cut shingle method.
C. Fasten shingles with 1 '/d" roofing nails in accordance with manufacturer's written specifications
and local building codes.

2.2. Remove and replace all existing lead plumbing vent stacks.
2.3. Remove and replace all paint finished galvanized gooseneck vents (color:' ac browq, or white).
2.4. Remove existing drip edge flashing and replace with new 6" galvanized eave drip flashing, excludes
ex' drip edge flashings entrapped by gutters that are not being replaced (color: black, brown, or
hite).
2.5. Remove and replace three (3) factory paint finished galvanized off ridge vents (colorblack, brown, or
white).

3.1. Upon completion, all work shall be inspected for soundness and, all job-related debris,will be removed
from the job site..
3.2. This Proposal includes all permit fees and inspections required by corresponding. city• or county-,
municipality.
Florida Universal Roofing, Inc. will furnish a Five (5) Year Warranty against all leaks under normal wear and tear. Florida

All material is guaranteed to be as specified. All work to -be completed -in a workman like manner according to standard practices. Any alteration or
deviation from above specifications or any necessary work to correct unforeseen conditions involving extra costs will become an extra charge over and
above the estimate and will be billed on a time and material basis at an hourly rate of $70.00 per man hour plus materials. If "Additional Insured" certificate
of insurance is required, an additional $200.00 will. be added to the contract price. All agreement are contingent upon strikes, accidents, or delay beyond
our control. • A finance charge of 1.5% per month will be`assessed on all unpaid balance§ after 30 day's. This proposal it subject to acceptance within 30 days
and is void thereafter at the option of the undersigned. All terms and conditions on the reverse side of this Proposal Contract are part of this Contract.
ACCORDING TO FLORIDA'S CONSTRUCTION LIEN LAW (SECTIONS 713.001-713.37, FLORIDA STATUTESVTHOSE WHO WORK ON YOUR.
PROPERTY OR PROVIDE MATERIALS AND SERVICES AND ARE NOT PAID IN FULL HAVE A RIGHT TO ENFORCE THEIR CLAIM FOR.
PAYMENT AGAINST YOUR PROPERTY. IF YOUR CONTRACTOR OR A SUBCONTRACTOR FAILS TO PAY THEIR SUBCONTRACTORS,
SUB -SUBCONTRACTORS; OR MATERIAL SUPPLIERS, THOSE'PEOPLE WHO ARE OWED MONEY MAY LOOK TO YOUR PROPERTY FOR -
PAYMENT, EVEN IF YOU HAVE ALREADY PAID YOUR CONTRACTOR IN FULL. IF YOU FAIL TO PAY YOUR 'CONTRACTOR, -YOUR
CONTRACTOR MAY ALSO HAVE A LIEN ON YOUR PROPERTY. THIS MEANS IF A LIEN IS FILED, YOUR PROPERTY COULD BESOLD
AGAINST YOUR WILL TO PAY FOR LABOR, MATERIALS, OR OTHER SERVICES THAT YOUR CONTRACTOR OR A SUBCONTRACTOR
MAY HAVE FAILED TO PAY. TO PROTECT YOURSELF, YOU SHOULD STIPULATE IN THIS CONTRACT THAT BEFORE ANY PAYMENT IS

CONTRACT AGREEMENT.— TERMS AND CONDITIONS
1. Any representations or other communications not written in this contract are agreed to be immaterial and not relied on by either
party. No agreements other than those specifically set forth in this contract shall be recognized and do not survive the'execution
of this contract. The entire understanding and contract agreement of the parties is contained in this contract agreement
document(s).
2. Any and all modifications, alterations, or deviations of the contract scope of work involving extra expense%st will be executed
only upon written orders or change orders and will become an extra expense / cost charge additional to this corltract agreement.
This contract both front and back constitutes the entire understanding of the parties and no other understanding shall be binding
unless in -writing and signed by both parties.
3. It is understood and agreed that this contract shall not become binding upon Florida Universal Roofing, Inc./Contractor
(hereinafter referred to as 77UR1 � until it is approved and accepted b_y FURI or an authorized officer of FUR), or until
performance commences; or whichever occurs first. -
4. Performance of the work scope described in this contract will commence as soon as possible, and to be. completed as soon as
possible, This is an' approximation/estimate and is subject to scheduling difficulties, labor, and/ojr material shortages, of weather-
related issues, or any other circumstances beyond our control, or other events not foreseen by FURL.
5. FURL shall not m
assue liability or be held liable for any damages to personal property or physical injury as a result of vibrations .
caused by hammering or walking on structures or any other normal work operations necessary for completion of the workscope
and shall not assume liability or be held• liable for any damage to improperly maintained or improperly constructed structures
resulting from normal work operations as contemplated on the face of this contract.
6. FURI assumes rio responsibility for damage incurred to property or groundresuhit►g from delivery of supplies•or* service '
equipment or any vehicles. .
7. Through the duration of the work scope, the customer will be responsible- for any interior damages, etc. provided that FURI
follows normal/standard dry -in procedures.to protect the property during the operation$ of the work scope., ,
8. Any and all left over materials are the property of FUR), and all materials remain the property of FUR] until paid in full.
9.r FURI will not be held liable or responsible for any reasonable wear and tear to driveways, parking lots, walkways, lawns, shrubs,
floral or vegetation. caused by the movement or the normal operations of tnrcks; equipment, materials, workforce, or debris.
10. FURI will reasonably clean up and remove work scope/job-related debris'created by FURL.
11. Payment will be made as noted on'thq face of this contract. In the event'payment'is ndimade'as noted on the face' of this coniiact,'
or upon presentatioh of a final invoice,' the_amoun'i or remaining balAnce' due and owing shall bear interest at the rate of eighteen
(I 896i percent per annum until. paid in.full. .
12. In the event FURI is required to institute legal action, mediation, and/or arbitration to enforce, construe, or interpret the terms or
conditions of this contract, FURI shall be entitled to its reasonable attorneys fees and costs at any trial court or appellate level and '
for any mediation or arbitration.
13. The purchasers) represents and warrants that he or she is (or they are) the owners, or legal .power of attorney, or, legally
authorized to approve contracts for the improvements or restorations•on.the address of the land •and• premises located on the face
of this -contract.,
14. All proposals/estimates are void a'ft4-30 days of proposal/estimate date located on the facc'of this document, unless otherwise
accepted by F'URI.or an authorized officer of FURI.
15. Buyer's right to cancel: If purchaser(s).determined within three .(3) business.days to cancel this contract he/she or they must.do so
by providing written notice to FURI by certified or registered mail, or telegram prior to midnight of the third business day.:'
16. After the three (3) business -day cancellation term, customers) may be subject to fifteen (15%) percent of the contract amount as
a liquidation damage fee.
17. Insurance authorization' agreement and terms: Customer hereby request FURI to negotiate with their insurance company for the
purpose of acquiring funds to cover the expense/costs' of.suah •repairs stated on the -face of this document. Customer authorizes
FURI to make any necessary repairs at the price negotiated between FURI arid the insurance company, and at no expense/costs to
the customer other than upgrades, add-ons, extras, wood replacement and deductible. The final price agreed on between FUR[
and the insurance company shall become. the final contract price. FURI. reserves the right to fileforany supplemental insurance
claims for the situations br circumstances due to measurement shortages or mistakes; material'and/or labor price increases, or any
other overlooked and/or unforeseen situations or circumstances, at no "additional expenselcosti to the customer. In the event that
the customer refuses to fulfill their obligation under these terms, the customer agrees to pay FURI the amount of tidenty (20%)*
percent of such insurance proceed -to FURI•as compensation for utilizing their time, efforts, skill, knowledge, and expertise in
acquiring payment on behalf for -the customer/insuied;,plus any•out.of pocket expenses/costs incurred by FURI in relation'to the
workscope which is set forth on the faceof this document:.lf this document is being used to obtain insurance proceeds for the
stated work scope on the face of this'document, FUR] will not start work until work is approved by the customer's insurance
company.
18. Customer(s) reads this agreement, fully understands its content, and agrees to be bound by the terms, conditions and stipulations
contained herein.
