commenced prior to the issuance of a permit and that all work will be performed to meet standards of all laws regulating construction
in this jurisdiction. I understand that a separate permit must be secured for electrical work, plumbing, signs, wells, pools,
furnaces, boilers, heaters, tanks, and air conditioners, etc.

NOTICE: In addition to the requirements of this permit, there may be additional restrictions applicable to this property that may be
found in the public records of this county, and there may be additional permits required from other governmental entities such as water
management districts, state agencies, or federal agencies.
Acceptance of permit is verification that 1 will notify the owner of the property of the requirements of Florida Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee at the time of permit submittal. A copy of the executed contract is required
in order to calculate a plan review charge and will be considered the estimated construction value of the job at the time of submittal.
The actual construction value will be figured based on the current ICC Valuation Table in effect at the time the permit is issued, in
accordance with local ordinance. Should calculated charges figured off the executed contract exceed the actual construction value,
credit will be applied to your permit fees when the permit is issued.

SCOPE:
This Evaluation Report is issued under Rule 61G20-3 and the applicable rules and regulations governing the use of
construction materials in the State of Florida. The documentation submitted has been reviewed by Robert Nieminen,
P.E. for use of the product under the Florida Building Code and Florida Building Code, Residential Volume. The
products described herein have been evaluated for compliance with the 51h Edition (2014) Florida Building Code
sections noted herein.
DESCRIPTION: GAF Asphalt Roof Shingles
LABELING: Labeling shall be in accordance with the requirements the Accredited Quality Assurance Agency noted
herein.
CONTINUED COMPLIANCE: This Evaluation Report is valid until such time as the named product(s) changes, the referenced
Quality Assurance documentation changes, or provisions of the Code that relate to the product change. Acceptance of
this Evaluation Report by the named client constitutes agreement to notify Robert Nieminen, P.E. if the product
changes or the referenced Quality Assurance documentation changes. Trinity) ERD requires a complete review of this
Evaluation Report relative to updated Code requirements with each Code Cycle.
ADVERTISEMENT: The Evaluation Report number preceded by the words "Trinity) ERD Evaluated" may be displayed in
advertising literature. If any portion of the Evaluation Report is displayed, then it shall be done in its entirety.

4.3.2 QuickStart® Peel & Stick Starter Roll is a is a mineral -surfaced, fiberglass -reinforced, self -adhering SBS modified
bitumen starter strip, nominal 9 -Inch x 33 ft roll, for use with asphalt shingles with exposure of 6 -Inch or less.
5. LIMITATIONS:
5.1 This is a building code evaluation. Neither Trinity IERD nor Robert Nieminen, P.E. are, in any way, the
Designer of Record for any project on which this Evaluation Report, or previous versions thereof, is/was
used for permitting or design guidance unless retained specifically for that purpose.
5.2 This Evaluation Report is not for use in the HVHZ.
5.3 Fire Classification is not part of this Evaluation Report; refer to current Approved Roofing Materials
Directory for fire ratings of this product.
5_4 Wind Classification:
5.4.1 The GAF asphalt shingles noted in Section 4.1 are Classified in accordance with FBC Tables 1507.2.7.1 and
R905.2.6.1 to ASTM D3161, Class F and/or ASTM D7158, Class H, indicating the shingles are acceptable for
use in all wind zones up to V„d = 150 mph (V,,,, = 194 mph). Refer to Section 6 for installation requirements
to meet this wind rating.
5.4.2 The GAF hip & ridge shingles noted in Section 4.2 are Classified in accordance with FBC Tables 1507.2.7.1
and R905.2.6.1 to ASTM D3161, Class F, indicating the shingles are acceptable for use in all wind zones up to
Va,d = 150 mph (V,,,, = 194 mph). Refer to Section 6 for installation requirements to meet this wind rating.
5.4.3 Classification by ASTM D7158 applies to exposure category B or C and a building height of 60 feet or less.
Calculations by a qualified design professional are required for conditions outside these limitations.
Contact the shingle manufacturer for data specific to each shingle.
5.5 All products In the roof assembly shall have quality assurance audit in accordance with the Florida Building
Code and F.A.C. Rule 61G20-3.
6. INSTALLATION:
6.1 Underlayment:
6.1.1 Underlayment shall be acceptable to GAF and shall hold current Florida Statewide Product Approval, or be
Locally Approved per Rule 61G20-3, per FBC Sections 1507.2.3, 1507.2.4 or R905.2.3.
6.2 Starter Shingles or Starter Strip:
6.2.1 Installation of Pro -Start Starter Strip Shingles, WeatherBlocker Premium Eave/Rake Starter Strip and
QuickStart Peel & Stick Starter Roll shall comply with the GAF current published instructions.
6.3 Asphalt Shingles:
6.3.1 Installation of asphalt shingles shall comply with the manufacturer's current published instructions, using
minimum four (4) nails per shingle in accordance with FBC Sections 1507.2 or R905.2, with the following
exceptions:
➢ Camelot, Camelot II, Grand Canyon, Grand Sequoia, Grand Sequoia IR, and Woodland require minimum
five (5) nails per shingle.
Slatellne requires minimum six (6) nails per shingle.
6.3.2 Fasteners shall be in accordance with manufacturer's published requirements, but not less than FBC 1507.2.6
or R905.2.5. Staples are not permitted.
6.3.3 Where the roof slope exceeds 21 units vertical in 12 units horizontal, special methods of fastening are
required. Contact the shingle manufacturer for details.

SCOPE:
This Evaluation Report is issued under Rule 61G20-3 and the applicable rules and regulations governing the use of
construction materials In the State of Florida. The documentation submitted has been reviewed by Robert Nieminen,
P.E. for use of the product under the Florida Building Code and Florida Building Code, Residential Volume. The
products described herein have been evaluated for compliance with the Slh Edition (2014) Florida Building Code
sections noted herein.

LABELING: Labeling shall be in accordance with the requirements the Accredited Quality Assurance Agency noted
herein.
CONTINUED COMPLIANCE: This Evaluation Report is valid until such time as the named product(s) changes, the referenced
Quality Assurance documentation changes, or provisions of the Code that relate to the product change. Acceptance of
this Evaluation Report by the named client constitutes agreement to notify Robert Nieminen, P.E. if the product
changes or the referenced Quality Assurance documentation changes. Trinity I ERD requires a complete review of this
Evaluation Report relative to updated Code requirements with each Code Cycle.

any dust and debris prior to application.

Installation Instructions and the requirements for ASTM D226, Type I and II underlayments In FOC Sections 1507 for

work to be completed. We strive to avoid any damage at all, and will seek to access the structure with the least Impact.
Roof Top Services of Central Florida, Inc. Is not responsible for any damages due to piping that may be attached to the underside of the root deck.
Roof Top Services of Central Florida, Inc. cannot be held liable for direct, incidental, coincidental, interior or exterior water damage, property damage, consequential damage, mold or mildew damage or
personal Injury related to the repairing or reroofing of the structure while Job is In progress or after completion. Owner is to carry all necessary Insurance'(11re, tornado, good, etc.) and any other necessary
Insurance.
Disputes arising out of terms and 1 or conditions of this contract are subject to Mediation and Binding Arbitration by both parties. As a member in good standing, Roof Top Services of Central Florida, Inc.
requires that all Mediation and Binding Arbitration be administered by the Better Business Bureau Care Program. Customer Is responsible for any and all attorney fees related to their dispute. Should
Contractor employ an attorney to Institute litigation or arbitration to enforce any of the provisions hereof, to protect its Interest in any matter arising out of or related to this Agreement, Contractor shall be
entitled to recover from the Customer all of Its attorney's tees, costs and expenses Incurred therein, including attorney's fees, costs, and expenses Incurred at mediation, administrative, appellate or
bankruptcy proceedings.
Any alterations or deviation from the agreed specifications involving extra cost of material or labor will only be executed upon written orders for same and will become an extra charge over the sum mentioned
In this contract. All agreements must be made in writing.

Roof Top Services of Central Florida, Inc. retains title to any equipment or material furnished until full and final payment is made. Contractor's Workmanship Warranty will become null and void If contract Is
not paid In full. Payments not rendered In accordance with contract agreement shall be subject to finance charges of 1.5% per month.
There Is a $35.00 N.S.F. Fee for all returned checks. In the event of collection, debtor Is to pay all attorney's fees, costs and expenses Incurred therein regardless of whether Final Judgment Is rendered. These
terms and conditions will be enforced.
Hydraulic Dump Trailers will remain on site until all work Is complete. If homeowner requires the Dump Trailer to be relocated or removed before the completion of the work there will be an additional charge of
$195.00 per move added to the contract price.
All paperwork lPerml% Notice of Commencement, Photographs and all affidavits) posted at job site must remain outside and accessible to the building Inspector to allow for signofl of all Inspections. If
paperwork Is removed before all Inspections have been completed there will be a $100.00 re -Inspection fee assessed to the property owner.
Pricing Is based on 14ayer of roofing. If additional layers are found, there will be an additional charge per layer.
Roof Top Services of Central Florida, Inc. reserves the right to withdraw this proposal if not accepted within (30) days.
Any approval(s) required by the Homeowner's Association is the sole responsibility of the property owner.
Owner and/or Owner's agent agrees to allow Roof Top Services of Central Florida, Inc. use of water and power on site for the duration of the project.
Should Roof Top Services of Central Florida, Inc. employ an attorney to Institute litigation or arbitration to enforce any of the provisions hereof, to protect Its Interest in any matter arising out of or related to
this agreement, Roof Top Services of Central Florida, Inc. shall be entitled to recover from the Customer whether or not a suit Is filed all of (ts attorney's fees, costs and expenses Incurred therein, Including
