Aluminum Structures Permit Application Checklist
All permit application packages must be complete prior to acceptance. You must check each box
to the left or indicate n/a on this submittal. A complete application package shall include the
following:
Building Permit Application completed, signed and notarized. Application must include
correct address and complete parcel I.D. number.
21 -/ Copy of a contract, signed by the contractor and the property owner, indicating the documented
construction value of the project.
P-,-�Copy of applicable contractor's license issued by the State of Florida.
O A site specific notarized power of attorney shall be required from the licensed contractor if
1J�Pr he/she appoints an employee of his/her company to sign the permit application as the
contractor.
Certificate of insurance indicating worker's compensation insurance coverage and naming the
City of Sanford as certificate holder, or a copy of a worker's compensation exemption issued
by the State of Florida (must be submitted with each application if contractor is the applicant).
O Completed and Signed Owner Builder Statement / Affidavit (if the owner is the applicant).

$ Two (2) copies of the signed and sealed engineered drawings. Wind design data required on
drawings per FBC 1603.1.4 to meet 129 mph ultimate design wind speed for risk category 1
buildings OR Two (2) copies of previously mastered filed plans. Indicate all details and options
that apply.
B-'-�Two (2) copies of a site specific layout for the structure with minimum 1/8" per foot scale.
Include all framing sizes and types. Include any electrical devices to be added.
Two (2) copies of Florida Product Approval, including manufacturer installation instructions
and product evaluation reports, for composite or elite roofing systems. (If applicable)

NIA accordance with FBCR R301.2.1.1.2

Aluminum self -tapping screws shall conform to ASME B 18.6.4 specification.

Maximum fastener spacing shall not exceed (3+20t) where "t" is the member thickness in inches.
For roofing and siding connection, use minimum #12 screws for end and side laps spaced at 12" max for side lap
and end lap fasteners shall bno more than 2" from the end of overlapping sheets.
Bots:
Bolts and other fasteners shall be aluminum, stainless steel, hot -dip or electro -galvanized steel. Double cadmium
plated steel bolts may also be used. Bolt holes diameter shall not exceed 1/16" larger than the bolt diameter and shall
be spaced at a minimum of 2. times the bolt diameter with minimum edge distance of 1.5 times the bolt diameter.

This Product Evaluation Report is being issued in accordance with the requirements of the Florida
Department of Business and Professional Regulation (Florida Building Commission) Rule Chapter 61 G20-
3.005, F.A.C., for statewide acceptance per Method 2(b). The product noted above has been tested
and/or evaluated as summarized herein to show compliance with the Florida Building Code Fifth Edition
(2014) and is, for the purpose intended, at least equivalent to that required by the Code. Re-evaluation of
this product shall be required following pertinent Florida Building Code modifications or revisions.

indicated in the span schedule(s) on its respective Product Evaluation Document (i.e. engineering
drawing).
Installation
Each product listed above shall be installed in strict compliance with its respective Product Evaluation
Document (i.e. engineering drawing), along with all components noted therein.
Each product component shall be of the material specified in that product's respective Product Evaluation
Document (i.e. engineering drawing).
Limitations & Conditions of Use:
Use of this product shall be in strict accordance with the Product Evaluation Document (i.e. engineering
drawing) as noted herein for outdoor patio construction only.
All supporting host structures shall be designed to resist all superimposed loads and shall be of a material
listed in this product's anchor schedule. Host structure conditions which are not accounted for in this
product's anchor schedule shall be designed for on a site-specific basis by a registered professiona'
