All permit application packages must be complete prior to acceptance. You must check each box to the
le/ft or indicate n/a on this submittal. A complete application package shall include the following:
r7 Building Permit Application completed, signed and notarized. Application must include correct address
and complete parcel I.D. number.
I/ Copy of applicable contractor's license issued by the State of Florida (if the contractor is the
applicant).
�i A site specific notarized power of attorney shall be required from the licensed contractor if
he/she appoints an employee of his/her company to sign the permit application as the contractor.
61 Certificate of insurance indicating worker's compensation insurance coverage and naming the City of
Sanford as certificate holder, or a copy of a worker's compensation exemption issued by the State of
Florida (must be submitted with each application if contractor is the applicant).

Installation Method
Manufacturer's General Installation Instructions:
• Product to be applied directly to a clean dry surface, free from any moisture, ice, dust, loose nails, protrusions,
voids or any other debris.
• Store product in the carton in a dry place until ready to use. Keep product out of direct sunlight. Do not lay rolls
on their sides.
• Prime surface as recommended by manufacturer.
• Adhere membrane only in fair weather when the air, surface and membrane are at a minimum temperature of
40° F. For installation below 40° F, mechanically fasten membrane to deck with nails until fully adhered.
• Start at the low point of the deck, align the membrane and press into place onto roof deck.
• Adhesive is pressure -activated, so press firmly into place using uniform pressure with a weighted roller.
• Overlap end laps a minimum of 6". Side laps a minimum of 3". Drip edges a minimum of 3/8".
• For valley and ridge installation, cut product to desired length, apply pressure from the middle working to the
outside edges.
• QUIK-STICK HT end laps must be sealed with a 1/8" layer of SBS mastic. This is also recommended for the other
products.
• Allow product to completely relax prior to any fastening.
• Do not expose QUIK-STICK HT to direct sunlight for more than 90 days.
• Minimum slope shall be 2:12 or greater.

Underlayment Application: For roof slopes between 2 units horizontal to 12
units vertical (17%) and 4 units vertical to 12 units horizontal (33%), two
layers of MSA UL 15#, MSA UL 30#', MSA ASTM D226.159, MSA ASTM
D226 309, MSA Z-15, or MSA Z-30 must be applied to the roof in the following
manner.
-Apply 19 inch -wide strip (483mrn) of underlayment felt parallel with and
starting at the eaves.
-Starting at the eave, apply 36 -inch wide (914mm) sheets of underlayment
overlapping successive sheets 19 inches (483mm). Distortions in the
underlayment shall not interfere with the ability of the shingles to seal.
-End laps shall be offset by 6 feet (I 829mm).
-Corrosion-resistant fasteners are applied along the overlap at a maximum
spacing of 36 inches (914mm) on center.

3. Unless otherwise noted, insulation may be any one layer or combination of polyisocyanurate, polystyrene, wood fiberboard, perlite, GlasRoc Roof Board or gypsum -based roof board that
meets the QA requirements of F.A.C. Rule 61G20-3 and is documented as meeting FBC 1505.1 and, for foam plastic, FBC 2603.4.1 or 2603.8, when installed with the roof cover.
4. Minimum 200 psi, minimum 2 -inch lightweight insulating concrete may be substituted for rigid Insulation board for System Type D (mechanically attached base sheet, bonded roof cover),
whereby the base sheet fasteners are Installed through the LWIC to engage the structural steel or concrete deck. The structural deck shall be of equal or greater configuration to the steel and
concrete deck listings.

7. Bonded polyisocyanurate insulation boards shall be maximum 4 x 4 ft.
8. For mechanically attached components or partially bonded insulation, the maximum design pressure for the selected assembly shall meet or exceed the Zone 1 design pressure determined in
accordance with FBC Chapter 16, and Zones 2 and 3 shall employ an attachment density designed by a qualified design professional to resist the elevated pressure criteria. Commonly used
methods are RAS 117 and FM LPDS 1-29. Assemblies marked with an asterisk* carry the limitations set forth in Section 2.2.1.5.1(a) of FM LPDS 1-29 for Zone 2/3 enhancements.
9. For fully bonded assemblies, the maximum design pressure for the selected assembly shall meet or exceed critical design pressure determined in accordance with FBC Chapter 16, and no
rational analysis is permitted.
30. For mechanically attached components over existing decks, fasteners shall be tested in the existing deck for withdrawal resistance. A qualified design professional shall review the data for
comparison to the minimum requirements for the system. Testing and analysis shall be in accordance with TAS 105 or ANSI/SPRI FX -1.

adhesive, and the existing roof system (for recover) shall be capable of resisting project design pressures on its own merit to the satisfaction of the AH), as documented through field uplift

Evaluation Report This product evaluation is limited to compliance with the structural requirements of
