meet standards of all laws regulating'-6 nsduction iri this jurisdiction. I understand that a separate permit
must be secured for electrical work, plumbing, signs, wells, pools, furnaces, boilers, heaters, tanks, and
air conditioners, etc.
OWNER'S AFFIDAVIT: I certify that all of the foregoing information is accurate and that all work; will
be done in compliance with all applicable laws regulating construction and zoning.
WARNING TO OWNED: YOUR FAILURE TO RECORD A NOTICE OF COMMENCEMENT MAY
RESULT IN YOUR PAYING TWICE FOR IMPROVEMENTS TO YOUR PROPERTY. A NOTICE
OF COMMENCEMENT MUST BE RE' CORDED AND POSTED ON THE JOB SITE BEFORE THE
FIRST INSPECTION. IF YOU INTEND TO OBTAIN FINANCING, CONSULT WITH YOUR
LENDER OR AN ATTORNEY BEFORE E'CORIDING YOUR NOTICE OF COMMENCEMENT.
NOTICE: In addition to the requirements of this permit, there may be additional restrictions applicable to this
property that may be found in the public records of this county, and there may be additional permits required
from other governmental entities such as water management districts, state agencies, or federal agencies.
Acceptance of permit is verification that I will notify the owner of the property of the requirements of Florida
Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee. A copy of the executed contract is required in order
to calculate a plan review charge. If the executed contract is not submitted, we reserve the right to calculate the
plan review fee based on past permit activity levels. Should calculated charges exceed the documented
construction value when the executed contract is submitted, credit will be applied to your permit fees when the
permit is released.

2.7 Tho Subcontractor shall at all times keep the prnjart Site free from accumulation of waste
materials and rubbish arising out of the Subc antractnes operations, and areas of such
operations shall be kept broom swept. Such waste materials and rubbish must be disposed
daily by the Subcontractor, at its sole cost and expense. In the event that the Subcontractor
fails to comply with this provision, the Contractor may, but shall not be obligated to, dispose of
such waste materials and rubbish and charge the Subcontractor for the Contractor's direct
cost for such disposal. The Subcontractor agrees that such charge may be off -set against any
sums due, of to become due, to the Subcontractor under this Subcontract.
2.8 Prior to the commencement of the Subcontract Work, the Subcontractor shall deliver to the
Contractor a detailed list of the Subcontractor's subcontractors, vendors and material
suppliers The Contractor may, in its sole discretion, object to any of the Subcontractor's
subcontractors, vendors or material suppliers by issuing written notice of objection to the
Subcontractor. In the event that the Contractor objects to any subcontractor, vendor or
material supplier, the Subcontractor shall propose a substitute subcontractor, vendor or
material supplier acceptable to the Contractor_ The Subcontractor shall have a continuing
obligation to notify the Contractor of any changes or additions to the Subcontractor's list of
subcontractors, vendors and material suppliers during the term of the Subcontract, and the
Contractor shall have the right to object to any such subcontractor, vendor or material supplier
in accordance with this paragraph.
3.0 THE SUBCONTRACT WORK
3.1 The uubcontractor shall perform the £uboontract Work in £tHot and ontiro conformity with this
Subcontract, the Prime Contract and applicable Laws (as herein defined).
3.2 The Subcontract Work is described on the Drawings, addendums, and in Specifications
identified on Exhibit A attached hereto and made a part hereof. The incorporation of Exhibit
A into this Subcontract shall net be construed by the Subcontractor to be a limitation on the
Subcontractor's obligation to perform all portions of the Subcontract Work that may be
described elsewhere in the Prime Contract.
4.0 SUBCONTRACT WORK
4.1 The Contractor agrees to pay the Subcontractor, for performance of the Subcontract Work,
according to the terms set forth in Section 22.0; the Eked lump sum of Nineteen Thousand

not be obligated to make any payments until, and at such time, that the Contractot receives
payment from the Owner, and such payment from the Owner to the Contractor shall be
a condition precedent to any payment from the Contractor to the Subcontractor. 3
