meet standards of all laws regulating construction in this jurisdiction. 1 understand that a separate permit
must be secured for electrical work, plumbing, signs, wells, pools, furnaces, boilers, heaters, tanks, and
air conditioners, etc.

NOTICE: In addition to the requirements of this permit, there may be additional restrictions applicable to this

Acceptance of permit is verification that I will notify the owner of the property of the requirements of Florida
Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee. A copy of the executed contract is required in order
to calculate a plan review charge. If the executed contract is not submitted, we reserve the right to calculate the
plan review fee based on past pennit activity levels. Should calculated charges exceed the documented
construction value when the executed contract is submitted, credit will be applied to your permit fees when the
permit is released.

All-Tite Roofing, Inc. is pleased to submit this contract/proposal for your approval. This price
includes all materials, tax and labor to complete the following:
Will tear off and replace 600 sq. ft of flat carport.
Replace with Certainteed Modified Bitumen, smooth or granulated.
Will inspect deck for rotten wood. Wood replacement @ $55.00 per man hour plus
materials. Re -nail decking if necessary.
43 Ib. asphalt base sheet underlayment.
Will provide roofing permit.
Will clean up and remove all roofing debris.
Total materials and labor will not exceed............ $2,000.00
The manufacturer will warranty the Modred Bitumen material for 12 years. All-Tite Roofing, Inc.
will warranty the labor for 3 years from completion date.
