Application is hereby made to obtain a permit to do the work and installations as indicated. I certify that no
work or installation has commenced prior to the issuance of a permit and that all work will be performed to
meet standards of all laws regulating construction in this jurisdiction. I understand that a separate permit
must be secured for electrical work, plumbing, signs, wells, pools, furnaces, boilers, heaters, tanks, and
air conditioners, etc.

be done in compliance with all applicable laws regulating construction and zoning.

Acceptance of permit is verification that I will notify the owner of the property of the requirements of Florida
Lien Law, FS 713.
The City of Sanford requires payment of a plan review fee. A copy of the executed contract is required in order
to calculate a plan review charge. If the executed contract is not submitted, we reserve the right to calculate the
plan review fee based on past permit activity levels. Should calculated charges exceed the documented
construction value when the executed contract is submitted, credit will applied to your permit fees when the
permit is released. ,

We propose to provide all labor and equipment necessary to execute the roofing work at the above referenced project according
to the following procedure.

INSTALLATION SCOPE OF WORK:
• Remove single layer of existing 3 -tab shingles down to the wood deck.
• Inspect and replace rotted or deteriorated decking.
• Furnish and install a single layer of 30# underlayment felt over the roof deck.
• Furnish and install roof to wall flashings.
• Install new drip edge (color - and starter shingle roll around the roofs perimeter.
• Install new 16" wide 26 gauge metal flashing underneath shingles in the valleys.
• Install new lead pipe boots and hardware on plumbing pipes.
• Install Shingles Type _Certainteed Landmark Architectural Color
• Cleanup —daily; magnetically sweep job site; haul away all job related debris.
• Provide a full time superintendent who shall be on site at all times when work is being performed.
WARRANTY:
Upon completion of the above scope of work Solidway Construction, Inc. will furnish a (5) year workmanship
warranty.

Terms & Conditions
• The replacement of a roof is a very aggressive and noisy process. You can expect hammering, banging and
vibrations throughout your home until the job is completed. You may wish to take steps to secure certain outdoor
and indoor items that could be damaged due to this type of activity. If you have a skylight(s), please take the
necessary precautions because debris may drop into the house when the skylight(s) is removed or replaced.
Solidway Constriction, Inc. will do everything possible to protect your property.
• Please notify Solidway Construction, Inc. prior to commencement of work, if you have any exposed or cathedral
ceilings in your home that may be susceptible to protruding nails. This should include any porches, screened
rooms, sun rooms, patios, Florida rooms and the like that are included in the area to be re -roofed. With prior
notification, Solidway Construction, Inc. can take the necessary steps to roof these areas. Solidway Construction
Inc assumes no responsibility for damage to these types of ceilings if we are unaware of their existence.
• In order to complete the re -roof process Solidway Construction, Inc. requires access to within a couple of feet of
the roof structure of your home. A dump trailer or dumpster is usually parked on the driveway. But it is sometimes
necessary to position a dumpster on a lawn to obtain this access. Solidway Construction, Inc. assumes
responsibility for damage caused by its own workers while using their vehicles or dump trailers. We encourage the
homeowner to park their vehicles on the street during the roofing project to prevent nails in tires.
• Solidway Construction, Inc. assumes no liability for damage caused by roofing nails being shot into electrical,
refrigerant, water, PVC or any other types of lines that may come into contact with the underside of the roof
decking material.
• Solidway Construction, Inc. assumes no liability for damage to the driveway or lawn caused by the shingle
suppliers' truck or equipment. If the supplier requests a waiver to be signed, this will be the responsibility of the
owner.
• If the flashing where the roof meets a wall or a chimney needs to be replaced, Solidway Construction, Inc. will not
assume the responsibility to replace surfaces like accucrete, stucco, and hardee board.
• Owner grants Solidway Construction, Inc. the right to use their address and photos of residence for advertisement
or marketing purposes and to post a yard sign in the front yard. No customer's information will be given to
telemarketing.
• When lead times become long there is the possibility that increases in the cost of materials may cause an increase
in your proposed price after you have signed with our company. Solidway Construction, Inc. will do everything
possible to maintain the stated proposal price for your roofing job. If an increase is required, you will be given the
option to either accept the increase or cancel your contract.
• Any claims that you want to make for damage must be submitted to Solidway Construction, Inc. within ten days
from the date that your roofing job was completed.
