To whom it may concern,
Based upon our site observation and direction from Greg Clarkson our office will be engineering
the renovation to the project referenced above.
The unit 1601 of the above referenced 4 unit town home has been burned to the extent that the
damaged wood interior framing as well as the second floor and roof framing above the existing
masonry wall needs to be demolished in order to reconstruct the unit. The new area to be rebuilt
is exclusive to the end unit 1601 and the fire wall which separates it from unit 1603. Unit 1605 &
1607 were undamaged by the fire and the only work to occur in these two units is to be
considered cosmetic only and not of a structural nature.

SCOPE:
This NOA is being issued under the applicable rules and regulations governing the use of construction materials.
The documentation submitted has been reviewed by Miami -Dade County Product Control Division and accepted
by the Board of Rules and Appeals (BORA) to be used in Miami Dade County and other areas where allowed by
the Authority Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. The Nliami-Dade County Product Control
Division (In Miami Dade County) and/or the AHJ (in areas other than Miami Dade County) reserve the right to
have this product or material tested for quality assurance purposes. If this product or material fails to perform in
the accepted manner, the manufacturer will incur die expense of such testing.and the AHJ may immediately
revoke, modify, or suspend the use of such product or material within their jurisdiction. BORA reserves the right
to revoke this acceptaA'ce, if it is determined by Miami -Dade County Product Control Division that this product or
material fails to meet the requirements of the applicable building code.

prepared by R.W. Building Consultants, Inc, dated 01-15-01 and last revised on 12-03-01, bearing the Mianhi-
Dade County Product Control Approval stamp with the Notice of Acceptance number and approval date by the
Miami -Dade County Product Control Division.
