Southern Bell Telephone and Telegraph Company
PRIVATE BRANCH EXCHANGE SYSTEM
The undersigned Subscriber requests the Southern Bell Telephone and Telegraph
Company to provide. at the address designated below, a system of PBX equipment
as listed in Schedule A attached. in accordance with the. Company's lawful rates and
regulations. including the provisions set forth herein:
1. Southern Bell will provide. install, maintain and provide replacement com-
ponents for a period of 180 months from the date of initial service of the equipment
described in Schedule A; thereafter Southern Bell will provide maintenance service
for the system and replacement of like components will be provided at supplemental
charges subject to the availability of equipment.
2. The Subscriber agrees to pay Southern Bell for said equipment a monthly amount
which is the sum of the Tier A price and the Tier B price during the initial service
period and thereafter will pay the Tier B price. The Tier A price will remain unchanged
d~ring the initial service period whereas the Tier B price is subject to change in accor-
dance with tariffs as lawfully filed from time to time by Southern Bell. The Tier A
. price, the initial Tier B price and the initial service period are as listed in the box below.
In the event of termination of all or part of the service prior to the expiration of
the Tier A initial service period. the Subscriber will be required to pay the unpaid balance
of the Tier A charges. At the Subscriber's option, a lump sum payment for the Tier A
balance due may be made. The amount of the lump sum payment will be computed by cal-
culating the present worth of a monthly annuity at the annual effective interest rate
used in determining the Tier A monthly rates.
Charges for central office trunks. stations and other terminal equipment are in
addition to those specified in Schedule A.
3. .i\dditicnal i::quipment and feart.lres offered ur~der the Ccmpan}"'s tari:Ef may be pro-
vided under one of two options:
Option 1: The additional equipment may be included under a new initial
service period commencing with the effective in-service date
of such additional equipment, or
Option 2: Such additional equipment may be included under this agreement
at any time prior to the expiration of the Tier A initial service
period by paying the sum of the Tier A monthly prices applicable
to the additional equipment rateable on a monthly basis over the
remaining portion of the Tier A initial service period.
~ 4. If a part of the service is discontinued and a portion of the equipment listed
in Schedule A is disconnected, it is agreed that the equipment last installed shall be
considered as the equipment first disconnected.
S. The Subscriber agrees to pay any added costs incurred by the Company because
of a change by the Subscriber of the location designated for the equipment prior to
the time it is placed in service.
6. If the service requested by the Subscnoer is cancelled prior to the establishment
of service, the Subscriber agrees to pay the cost incurred by the Company in engineering,
ordering and providing the equipment and disposing of it, less credit obtained through dis-
posal.
This instrument becomes binding when executed by the .Subscriber and accepted by
the Company and shall remain in effect until properly terminated in accordance with the
Companv's lawful rem.liations. The terms and cona3tions rex-caE cannot be varl",d 0";:
