apply to and govern all work under this Change Order.

Description of Change: Additional Pipe Removal and 12" Gate Valve
Reason for Change: 511 LF of existing 6" FM, 150 LF of existing 12" FM and 711 LF of
existing 24" WM was previously taken out of service but was
unable to be removed during the original CR46A Phase III
construction. This existing piping must be removed during the
roadway construction therefore it needs to be added to the CR
46A / Airport Boulevard Utility Relocations construction. A 12"
gate valve at Larkwood Drive will also be added at the City's
Request to allow the force main flow to continue down CR46A
during construction and minimize force main flows along Upsala
Road.

Description of Change: Relocation of 6" RWM along Vinewood Drive
Reason for Change: Approximately 180 LF of the City of Sanford's existing 6"
reclaimed water main is in conflict with the County's proposed
structure S -112 and proposed curbline along Vinewood Dr. and
needs to be relocated. (see attached sketch)

WAIVER This Change Order constitutes full and mutual accord and satisfaction for the
adjustment of the Contract Price and Contract Time as a result of increases or decreases in
cost and time of performance caused directly and indirectly from the change. Acceptance of
this Waiver constitutes an agreement between OWNER and CONTRACTOR that the Change
Order represents an equitable adjustment to the Agreement and that CONTRACTOR shall
waive all rights to file a Contract Claim or claim of any nature on this Change Order. Execution
of this Change Order shall constitute CONTRACTOR's complete acceptance and satisfaction
that it is entitled to no more costs or time (direct, indirect, impact, etc.) pursuant to this Change
Order.
APPROVAL AND CHANGE ORDER AUTHORIZATION
ACKNOWLEDGMENTS
The aforementioned change, and work affected thereby, is subject to all provisions of the
original Agreement and specifically changed by this Change Order; and
