Addenda Extending Agreements with North Seminole Family Practice
Pursuant to the approval of Consent Agenda Item I., Approval of Nunc Pro Tunc Extensions of
Agreements with North Seminole Family Practice Associates, P.A., by the City Commission in
the December 14, 2009 City Commission Meeting, please find fully executed originals of the
above referenced documents.
You will note that even though the agreements include signature lines for Drs. Monette, Quinn and
Schefsky, we only have Dr. Quinn's signature. I spoke to Lonnie Groot, City Attorney, and he is
good with having the one signature, says it is still binding. Note his signature on the agreements,
accordingly.

The City and the Contractor formally extend the Agreement entered October 1,
2006, as renewed on November 9,2007 nunc pro tunc to October 1,2007 for a renewal
term expiring on September 30, 2008, this extension being accomplished nunc pro tunc
to October 1, 2008 for a period expiring on May 31,2010, said extension occurring as
follows:

Section One. Incorporation bv Reference. The Agreement dated October 1,
2006 and executed by the Contractor on October 16, 2006 and the City on October 10,
2006 (hereinafter the "original Agreement") is incorporated herein by reference, made a
part hereof and shall govern the relationship of the parties until amended,
supplemented, or modified pursuant to Section 6.3 of the original Agreement.
Section Two. Term. The term of this Extension Agreement shall commence
nunc pro tunc on October 1, 2008 and be renewed until May 31, 2010 and subject to
successive renewals in accordance with Section 6.3 of the original Agreement.
Section Three. Confirmation/No Additional Chanaes. The agreements,
terms, and conditions of the original Agreement are confirmed, ratified, and approved by
the parties and they agree to be bound thereby during the extension period defined in
this Extension Agreement. Aside from the foregoing, the Agreement is neither amended
nor revised.
IN WITNESS WHEREOF, the parties hereto have made and executed this
Extension Agreement on the respective dates under each signature.
