This Contract Amendment is made and entered into this + day of "
d by and between the STATE OF FLORIDA DEPARTMENT OF TRANSPORTATION,
hereinafter called the "Department" and The City of Sanford, hereinafter called the "UAO,"
WITNESSETH:
WHEREAS, the Department and the UAO on January 23, 2007 entered into an agreement titled
Design/Build Utility Agreement, hereinafter referred to as the "Agreement"; and
WHEREAS, the parties have determined that it is in their mutual best interests to amend the
Agreement;
NOW, THEREFORE, the parties hereby amend the Agreement as follows:
As to the sections of the UAO's 20" Reclaimed Water Main located from Stations 25 +28(MP
5.412)(98.50' LT, elev. 6.80) to 26 +16(MP5.427)(108.15' LT, elev. 6.49) across Old Geneva Road and
stations 73 +89.66(MP 6.357)(72.54' LT, elev. 6.57) to 73 +94.7(18.75' LT) to 77+98.46(4.04'RT) to
80 +17.29(MP 6.467x25.35' RT, elev. 8.20) the UAO shall be entitled to leave those sections of pipe in
place without any further obligation of the UAO and the provisions of Section 7 of the Agreement will
not apply to those sections of pipe
Except as hereby modified, amended, or changed, all of the terms and conditions of the
Agreement and any supplements and amendments thereto will remain in full force and effect.
IN WITNESS WHEREOF, the parties have executed this Contract Amendment
