Forensic Laboratory Services Division may issue simplified reports for crime scene and
laboratory services provided by the Laboratory personnel for the Sanford Police Department.
The following information will be included e simplified report:
~ Atitle (Crime Scene Report, Latent Print Comparison Repor.atc.).
• The name and address of the Laboratory,
* The case number,
• The name and address of the contributor
* The title and name,
^ A description of all items recoived, including items that were not tested (exornined),
items created that were or could be tested,
° Results oftests and/or examinations conducted,
* Initial database entry (e.g. BIS, |AF|S. N|B|N), if applicable,
w Anassociation resulting from adatabase search, ifapplicable,
* Disposition ofthe evidence,
w Statement regarding opinions and interpretations,
• Statement regarding simplified reports (external agencies cases only),
w Statement specifying that the report will not be reproduced except in full without
approval of the laboratory. This can provide assurance that parts of report are not
taken out ofcontext, and
* VVhena na|avant, o statement hothe effect that the results relate only to the items tested.
• Pagination to allow for recognition of complete report and clear identification of the
end.
The following information will not be included a e|nno|Jk*d report but are located within the
laboratory case record and are available upon request:
• Identification of the method(s) used, additions to, cleviclations or exclusions from the
method and
^ The date ofreceipt ofthe test items and the date of the
