Before me, a notary public, appeared JAMI D. THOMAS, as Chief Executive
Officer of J D Thomas Cultural Center, Inc., a dissolved Florida not for profit
corporation (hereinafter referred to as "Affiant"), who after first being duly swom,
deposes and states as follows:
1. That Affiant has personal knowledge of all matters set forth in this Affidavit.
2. That J D Thomas Cultural Center, Inc., a dissolved Florida not for profit
corporation was the previous owner of the below described real property lying and being
in Seminole County, Florida, which real property is currently owned by the City of Sanford,
a Florida municipal corporation, Florida, to wit:
Lots 12 through 16, inclusive, Block 7, Tier 1, of Florida Land
and Colonization Company Limited E. R. Trafford's Map of the
Town of Sanford, according to the plat thereof, as recorded in
Plat Book 1, Pages 56 through 64, inclusive, Public Records of
Seminole County, Florida.
(hereinafter referred to as the "Property").
3. That pursuant to that certain Notice of Reversion Relating to 519 Palmetto
Avenue dated October 3, 2012 and recorded October 5, 2012 in Official Records Book
7870, Page 593, Public Records of Seminole County, Florida, the Property has not been
used for public purposes since January 1, 2012.
4. That J D Thomas Cultural Center, Inc., a dissolved Florida not for profit
corporation, vacated the property on or about October 30, 2012.
5. That all shareholders, directors and officers of J D Thomas Cultural Center,
Inc. a dissolved Florida not for profit corporation, acknowledged and consented to the
reversion of the Property to the City of Sanford pursuant to the terms and provisions of that
certain Special Warranty Deed dated April 13, 1998 and recorded May 12, 1998 in Official
Records Book 3423, Page 91, Public Records of Seminole County, Florida.
6. That Jami D. Thomas, as Chief Operating Officer of J D Cultural Center, Inc,
a dissolved Florida not for profit corporation, is authorized to execute any necessary
documents to wind up the affairs of J D Cultural Center, Inc, a dissolved Florida not for
profit corporation, pursuant to Florida Statute 607.1405 and related provisions of the
Statute.
7. Under penalties of perjury, Affiant declares that he has read the foregoing
document and that the facts stated in it are true.
