FUNDING OF THE YOUTH EMPOWERMENT AND LEADERSHIP DEVELOPMENT ACADEMY The City and the Contractor formally extend the Agreement between the following parties made and entered the 15T" day of MAY, 2018, (notwithstanding the date of signature thereof) between the City of Sanford, Florida (herein called the "City) a Florida municipal corporation whose address is City Hall, 300 North Park Avenue, Sanford, Florida 32771 and Goldsboro Front Porch Council, Inc. (herein called the "Contractor") whose address is 1213 Historic Goldsboro Boulevard, Sanford, Florida 32771 (the said Agreement being referred to herein as the "Original Agreement") this extension being accomplished nuns pro tune to October 1, 2018 for a period expiring on September 30, 2019, said extension occurring as follows: Section One. Incorporation by Reference. The Original Agreement is incorporated herein by reference, made a part hereof and shall govern the relationship of the parties until amended, supplemented, or modified pursuant to the terms and conditions of the Original Agreement. Section Two. Term: Extension. The term of this Extension Agreement shall commence nuns pro tune on October 1, 2018 and be renewed and extended until September 30, 2019 and shall be subject to successive renewals in accordance with the terms and conditions of the Original Agreement. Section Three. Payment: Funding. It is expressly agreed and understood that the total to be paid by the City under this Extension Agreement shall not exceed the total sum of Thirty Thousand Dollars and No Cents ($34,185.00) as set forth in the attachment "A" Budget. Section Four. Scope of Services. The Contractor, in a manner satisfactory to City, shall perform or cause to be performed all services as implied, described, referred to as set forth in the attachment "B" Scope of Service Section Five. Confirmation/No Additional Changes. The terms, and conditions of the Original Agreement are confirmed, ratified, and approved by the parties and they agree to be bound thereby during the extension period defined in this Extension Agreement. Aside from the foregoing, the Original Agreement is neither amended nor revised. The Contractor agrees and affirms that the City has fully and completely adhered to the terms and conditions of the Original Agreement and that the Contractor has no claim against the City. IN WITNESS WHEREOF, the parties hereto have made and executed this Extension Agreement on the respective dates under each signature. IIPa - e Exhibit "A" SCOPE OF SERVICES YOUTH EMPOWERMENT AND LEADRRSHIP DEVELOPMENT ACADEMY (YELDA) 2019 Section I. SERVICES TO BE RENDERED BY CONTRACTOR Participants Eligibility Criteria The contractor will be responsible for verifying all youth participants are eligible for the services. The Contractor shall provide proof that all enrolled students meet the eligibility criteria as listed: a. Resident of the City of Sanford or adjoining neighborhoods; b. Verification that parent/guardian has a valid identification; c. Verification that student has a valid birth certificate; i. Applicants must be 16-19 years old and enrolled in high school; ii. Parents/Guardians must demonstrate legal custody of child if not the parent on birth certificate; and d. Proper verification for all eligibility criteria must be included in file and shared with CareerSource. e. Preference will be given to students that participated satisfactorily in prior year's programs. f. Verification that the 12 students compensated through the City funding that the household income do not exceed 120% of the area median income and they are residents of the City of Sanford. Seminole County (Orlando, Kissimmee and Sanford MSA) median income $62,900 The contractor certifies that the activity carried out under this Agreement will meet the City's objective of providing summer employment and leadership training and development to at risk or disadvantaged youth. The services to be provided are as follows: • Provide leadership skills, character education, culture enrichment and overall life skills for 12 youth ages 16-19 who are high school students that reside in the City of Sanford or adjoining neighborhoods; • Develop and submit a detailed outline of the 24 hour 4 -day training 7 days prior to the start of the training; • 24 hours Leadership training to occur June 3rd — 7th, 2019 • Provide job readiness, career development and a paid work experience of 20 hours per week for 7 weeks for the 12 students ; • The 7 week work experience to start Monday, June 10th, 2019 and completed by Friday, July 26th, 2019; • Provide a cultural educational day trip for the students; Page 1 of 2 GFPC - YELDA Summer Program 2019 Scope of Services • Develop partnerships with local government, businesses, non -profits and faith based organizations in the community to provide work sites for the students; • Conduct one information meeting for all worksites to explain in more detail expectations, students dos and don'ts, work week, completion and submission of time sheets and etc. • Assume full responsibility for the 15 youth being compensated through the City grant to include maintaining time sheets and payroll processing; • Provide day-to-day supervision and administrative oversight of the 15 youth; • Conduct a ceremony at the conclusion of the program to recognize the students, their parents and businesses that participated in the program; and • Prepare and submit a closeout report within 30 -days of the completion of the program, to include o A survey to be completed by worksite supervisors on recommendations to enhance the program. Goals and Performance Measures The Contractor agrees the funding will be used for the Youth Empowerment and Leadership Development Academy (YELDA) program. The Program will provide fifteen (15) high school students in the City of Sanford with leadership skills, character education, job readiness, work experience, cultural enrichment, money management, career development and overall life skills to assist them in becoming caring, productive and contributing members of society. Performance Measures: • Raise the employability of the students • Prepare students for entering the work force and or college • Increase parental involvement in the student's education life • Increase student's financial competency • Reduced youth's negative contact with law enforcement and juvenile justice system Compliance with Laws and Regulations In providing all services pursuant to this Agreement, Contractor shall comply with applicable Federal (specifically 24 CFR Part 570), State and local statutes, ordinances, rules and regulations pertaining to, or regulating the provision of such services, including those now in effect and hereafter adopted. Any violation of the foregoing statutes, laws, rules, or regulations shall constitute a material breach of this Agreement, and shall entitle the City to terminate this Agreement immediately upon delivery of written notice of termination to Contractor as provided in the Agreement.
