Description of Change: Plumbing (not included in the original scope of services and
agreement)
Reason for Change: Additional plumbing cost for a licensed plumber to remove the water
heater from the closet and them reattach it (3 days later). Also, fix the leak from the valve from
the water line to the water line to the washer machine and inspect the drain from the washer
machine.
Change in Contract Price: $2,519.66

Description of Change: Electrical (not included in the original scope of services and
agreement)
Reason for Change: Due to having to reframe both bathrooms (Men's & Women's). The
electric had to be redone to properly ensure the safety with the new stud walls and everything is
up to code.
Change in Contract Price: $6,244.00

ITEM 3
Description of Change: Framing & Rough Carpentry Labor & Materials (not included in the
original scope of services and agreement)
Reason for Change: Labor costs to properly reframe the Men's and Women's bathroom.
Both bathrooms need to be reframed due to all the metal studs being rusted, corroded, and
damaged. The rusting is so bad on the studs that many of the walls are not even connected to
the bottom plate. Essentially the wall was just hanging in place. By reframing the bathrooms
walls we will safely be able to install the new drywall and tile on and ensure it is safe for years to
come.

Description of Change: Drywall and Associated Materials, Paint, Cove Base & Lighting (not
included in the original scope of services and agreement)
Reason for Change: Materials needed for the extra drywall was take down due to
secondary damage. This includes the drywall in the water heater closet, both walls in the dorm
room area, and both walls in the gym. All materials will match existing colors and will be properly
used. Paint colors differ from the choices made in the original bid.

Description of Change: Contamination - air or surface testing & lab analysis (not included in
the original scope of services and agreement)
Reason for Change: Air and surface testing done inside the Men's and Women's
bathrooms, Hallway and Dorm Room. Test was sent for evaluation by a licensed hygienist and an
official report with the results will be provided.

adjustment of the Contract Price and Contract Time as a result of increases or decreases in cost

Waiver constitutes an agreement between OWNER and CONTRACTOR that the Change Order
represents an equitable adjustment to the Agreement and that CONTRACTOR shall waive all
rights to file a Contract Claim or claim of any nature on this Change Order. Execution of this
Change Order shall constitute CONTRACTOR's complete acceptance and satisfaction that it is
entitled to no more costs or time (direct, indirect, impact, etc.) pursuant to this Change Order.

ACKNOWLEDGMENTS
The aforementioned change, and work affected thereby, is subject to all provisions of the original
Agreement and specifically changed by this Change Order; and
