EXTENSION TO AGREEMENT
THIS EXTENSION AND TO AGREEMENT (the "Extension") is made and
entered into on this Htay of November 2023, nunc pro tunc to October 1, 2023,
and is to that certain "Service Agreement For Workers' Compensation Handling"
made and entered into on the 1st day of October, 2013 (the "Agreement"), as
modified, between the CITY OF SANFORD, a municipal corporation organized and
existing under the laws of the State of Florida (hereinafter referred to as the "CITY"),
whose address is 300 North Park Avenue, Sanford, Florida 32771, and Johns
Eastern Company, Inc., a corporation, authorized to do business in the State of
Florida, whose principal and mailing address is Post Office Box 110259, Lakewood
Ranch, Florida 34211 (hereinafter referred to as "JOHNS EASTERN")
WITNESSETH:
WHEREAS, the CITY and the JOHNS EASTERN entered into Agreement
and now desire to extend the Agreement so as to enable both parties to continue to
enjoy the mutual benefits that the Agreement currently provides, but only until
September 30, 2024; and
WHEREAS, the Agreement and the essential requirements of contract law
provide that any extensions to agreements shall be expressed in writing and duly
executed by the parties; and
WHEREAS, the parties desire to extend the term of the Agreement and for a
specified period of time, as stated above, so as to enable both parties to continue to

enjoy the mutual benefits it provides and this Extension falls within the purchasing
authority of the City Manager of the CITY,
NOW, THEREFORE, in consideration of the mutual understandings and
agreements contained herein, and other good and valuable consideration, the
receipt and sufficiency of which is hereby acknowledged, the parties agree to extend
the Agreement as follows:
SECTION 1. RECITALS. The above stated recitals (whereas clauses) to this
Extension are hereby adopted and form a material part of this Extension and the
consideration hereof upon which the parties have relied.
SECTION 2. EXTENSION AND RENEWAL. The term of the Agreement,
which was to expire on September 30, 2023, in accordance with the provisions of
the Agreement, as modified, is extended such as the Agreement will now terminate
on September 30, 2024 and the Agreement is renewed nunc pro tuns to October 1,
2023
SECTION 3. NO OTHER AMENDMENT; AGREEMENT COMPLIANCE.
Except as specifically stated herein in this Extension, all terms and conditions of the
Agreement shall remain in full force and effect as originally set forth in said
Agreement, as modified and the parties agree that both parties are not in breach
and have fully complied with the provisions of the Agreement, as modified.
IN WITNESS WHEREOF, the parties hereto have executed this instrument
for the purposes herein expressed.

RE: Johns Eastern Company Renewal
Mr. Bonaparte,
Last Thursday, November 2, 2023, it was discovered by both City staff and Johns Eastern
Company (DECO), the Third Party Administrator for the City's workers compensation program,
that the five year agreement between the City and JECO expired September 30, 2023.
Accordingly, Lonnie has prepared the attached agreement that will extend the services of JECO
through September 30, 2024. JECO is agreeable to the agreement as they have already signed
the attached agreement.
Would you please review, and if you approve, sign and return to my attention?
