Enclosed is the original fully executed Agreement under the Economic Development
Transportation Fund (EDTF) on behalf of the project and company stated above. Additionally,
along with the Quarterly Stares Report w/sample and Request for Modification forms, a Project
Recap Sheet, intended to assist with the administration of this project, is enclosed.
Please make a note of the following:
Paragraph 2.0 Tern: The effective date of this EDTF Agreemere is April 28, 2000. Only
project costs incurred on or after this date and on or prior to the December 21, 2001, termination
date are eligible for reimbursement. The April 28, 2000, date is also important because it signifies
the start date for quarterly project reports that are to be submitted to the Office of Tourism,
Trade, and Economic Deveiopmem (OTTED). Please feel flee to make copies of the enclosed
Quarterly Status Report form. Your first report is due August 13, 2000, for the period April 29
through July 28, and should be submitted even if there has been no activity on the project.
Release of Funds: Under the terms and conditions of the EDTF Agreement, five (5) provisions
must be met and appropriate documentation sent to OTTED before any money can be requested.
These are:
1) A resolution to accept future maintenance and attendant costs in connection with the
project. [Section 6.2(a)]
2) Certification that the business on whose behalf the application was made has secured any
necessary permits and initiated vertical construction of its facility. [Paragraph 6.2(b)]

The City has until October 26, 2000, (180 days) in which to provide OTTED with this
certification. In reviewing the paragraph, please note that OTTED may, at its discretion,
terminate this Agreement if the certification is not provided within the 180-day period
that began when the Agreement was fully executed If American LaFrance Medicmaster
is unable to secure all necessary permits and initiate construction by October 26, 2000,
please inform us in writing of the reasons and request an extension of the default date.
3) A letter certifying compliance with the requirements of Chapter 163, Part II, Florida
Statutes. [Section 6.2(d)]

Certification and a copy of appropriate documentation substantiating that all right-of-way
has been obtained and meets the definition of right-of-way. Please submit a copy of the
deeds of all necessary right-of-way. [Section 6.2(e)]

Written notification of the County's intent to either award the construction of the
transportation project to the lowest and best bidder in accordance with applicable state
and federal statutes, rules and regulations or construct the transportation project utilizing
existing Cotmty employees. [Section 6.2(f)]
Paragraph 16.0 Commencement: The construction commencement date is June 26, 2000, and
final completion date for the transportation project is December 22, 2000.
If construction on the transportation project will not begin by the commencement date
specified in the EDTF Agreement, please complete the enclosedRequest for Modification
form. This will serve as written notice that a modification is requested If a modification
is necessary~ the request must be made and received by OTTED before the construction
commencement or completion date, whichever is to be modified OTTED will then
consider the written request pursuant to the modification provision of the Agreement.
