CHANGES ORDERED ITEM 1 Description of Change: Contract adjustment for As-Built quantities versus original Bid quantities at the Contract unit prices. Reason for Change: Adjustments for Bid items actually installed / constructed from field measurements. Change in Contract Price: Decrease of $888.40. Change in Contract Time: No change, ITEM 2 Description of Change: Additional crew and equipmenttime (2 hours) on 10/31/00 to provide temporary support and excavate around an exiting utility pole at Sta 22+63 RT, including all labor, equipment, fuel, labor burden, overhead, profit, and bond costs. Reason for Change: The Utility Re[ocation Schedule submitted to FDOT included the removal of this utility pole as one of the preconditions for beginning the City's utility relocation work. The utility pole had not been removed by the time of the forcemain installation. In an effort to complete the work within the scheduled timeframe, the Contractor provided temporary support for the utility pole, and continued with the installation of the forcemain. Change in Contract Price: Increase of $553.56. Change in Contract Time: No change. iTEM 3 Description of Change: Additional crew and equipment time (3 hours) on 11/02/00 in conjunction with the water main tie-in at 29th Street (East), including all labor, equipment, fuel, labor burden, overhead, profit, and bond COSTS. Page 2 of 6 Reason for Change: There was an existing 6" water main along the north side of 29th Street that was not identified on the Drawings. This line was connected to the existing WM along the east side of Sanford Avenue that was being removed as part of the relocation work. The Contractor spent additional time to identify the unknown WM and to re-schedule work associated with Connection "C". Change in Contract Price: Increase of $507,49. Change in Contract Time: No change. ITEM 4 Description of Change: Install a 6" plug valve in the existing forcemain along the north side of 27th Street (across from the CattlemaWs Steakhouse), including '"' all material, labor, equipment, fuel, labor burden, overhead, profit, and bond costs. "- Reason for Change: To increase the flexibility of the forcemain system, whereby flows from the Praver Homes lift station (Woodmere Park) can be directed either west along 27~' Street or north along Sanford Avenue. Change in Contract Price: Increase of $2,256.27. .- Change in Contract Time: No change. ITEM 5 Description of Change: Connect existing forcemain from the Prayer Homes lift station to the new forcemain along Sanford Avenue; replace existing 6" forcemain between Mattie Ave. and 27th St. to avoid conflicts with roadway widening work. Reason for Change: in conjunction with Item 4 above, the connection between the existing and the new forcemain at the corner of Mattie Ave. and Sanford Ave. increases the flexibility of the forcemain system, whereby flows from the Prayer Homes rift station (Woodmere Park) can be directed either west along 27~h Street or north along Sanford Avenue. Also, the existing 6" forcemain between Mattie Ave. and 27th St. conflicted with a proposed FDOT drainage structure and had to be removed. ITEM 6 Description of Change: Additional crew and equipment time (1/2 hour) on 12/07/00 due to a broken water service that was incorrectly located, including all labor, equipment, fuel, labor burden, overhead, profit, and bond costs. Reason for Change: A 1" water service line was broken during excavation for the removal of the 10" WM at Sta 26+70 LT. The water service line had been incorrectly located. Change in Contract Price: Increase of $132.90. Change in Contract Time: No change. ,. ITEM 7 Description of Change: Numerous phone cables between Sta 25+40 LT and Sta 26+20 LT slowed the Contractor's progress during removal of the 10" WM. " The number of existing phone cables was not accurately reflected on the Drawings. Additional3-hoursofcrewandequipmenttimeon 12/11/00, including all labor, equipment, fuel, labor burden, " overhead, profit, and bond costs. Reason for Change: There were numerous telephone cables over / around the existing ,- 10" WM. This required that the area be excavated by hand digging to prevent damage to the phone cables during removal of the 10" WM. Change in Contract Price: Increase of $695.64. ,. Work commences on Mettle Street to tie in old 6" transite FM line to the new 12" FM line. Mattie Street is shut down from Sanford Ave, to Grandview, and the Sanford Police are notified of the street closure. While excavating for the tie in, the gas company makes contact to discuss the position of the transite and 6" ductile line at station 24+70 Rt. (See problems below). With the exception of the many changes put in by the City, work proceeds slowly for the tie in. At 2:30 pm, the tie in of the 6" transite line to the 12" FMis complete and 4:00 pm the first part of the tie in of the 6" transite line to the old 6" ductile line is complete. Mettle street remained shut down over night to allow work to continue the next day, and the Sanford Police were notified of the road closure. To help resolve the conflict, Ed (city inspector) is called to site. After an extensive conversation, and contact with the engineer Tom Zinker, the conflict is resolved with a directive from the city to reform the driveways to equal or better than Iwas originally in place. A layout is determined and agreed upon by all parties affected, and the forms put in place. ~Ed approves the form layout. The new size/layout results in an increase of concrete from 50 yards to an estimated t60 yards. The changes to the driveways result in an additional 24 manhours labor, plus the additional !work required by the concrete subcontractor.
